Primary mediastinal tumors in children and adults: a clinicopathological spectrum.

BACKGROUND: Mediastinal tumors are a heterogenous group of unrelated neoplasms that share the mediastinal anatomical location. There are no local publications regarding mediastinal masses, as such the clinical and pathological characteristics of this group of tumors in our population has been overlooked. The study aims to describe the mediastinal masses clinicopathological spectrum in children and adults. METHODS: With the objective to compare the clinicopathological spectrum of mediastinal tumors among children and adults we retrospectively review the medical charts and pathology reports of

Mediastinal germ cell tumors.

Mediastinal germ cell tumors are among the most common tumors in the anterior mediastinum. This tumor should be considered strongly in the differential diagnosis of anterior mediastinal masses. Benign teratomas are common in this site, and complete surgical resection results in cure of nearly all patients. Malignant mediastinal seminomas are highly curable and either radiotherapy (mediastinal disease) or cisplatin-based chemotherapy (metastatic disease) will result in long-term survival in 80% or more of patients. Mediastinal nonseminomatous germ cell malignancies represent a clinically and bi

Successful resection of a giant primary liposarcoma of the posterior mediastinum.

Primary liposarcomas of the mediastinum are extremely rare neoplasms, comprising less than 1% of all mediastinal tumors. These tumors occur most commonly in the lower extremities (75%) and less frequently in the retroperitoneum. We present a case involving a successful radical resection of a large myxoid liposarcoma and its recurrence, both located in the posterior mediastinum.

Pulmonary cytopathology of an invasive thymoma.

The cytopathology of an invasive thymoma that penetrated the bronchus is presented. As is typical of these neoplasms histologically, both epithelial cells (thymocytes) and lymphocytes were seen cytologically. However, the benign appearance of the thymocytes precluded a neoplastic diagnosis initially. Only with the knowledge that an anterior mediastinal mass is present roentgenographically can this diagnosis be safely made cytologically.

Metastatic HPV-related head and neck squamous cell carcinoma to the lung and mediastinal lymph nodes in aspirated cytology material: A diagnostic pitfall.

BACKGROUND: Although HPV-related head and neck squamous cell carcinoma (HNSCC) has good prognosis, a small proportion of patients develop distant metastases and have worse outcomes. Such metastases can be particularly difficult to diagnose in the lung and mediastinum, where they show extensive morphologic overlap with primary pulmonary neoplasms. This case series discusses metastatic HPV-related HNSCC in lung and mediastinal fine needle aspiration (FNA) cytology. METHODS: The cytopathology archives were searched for lung and mediastinal FNA specimens of patients with HNSCC. Specimens were inc

[Mediastinal involvement in the course of proliferative diseases in children in the materials of one institution].

UNLABELLED: The mediastinum region may be the primary or secondary localization of neoplasms. The aim of our study was the assessment of clinical symptoms, histopathology and outcome of mediastinal tumours in children. MATERIALS AND METHODS: A group of 27 children, chosen from the group of 128 patients with malignancies, which were treated in the years 2000-2004 in the Oncology and Haematology Unit of the Paediatric Department in Katowice, has been studied. In this group there were 14 boys and 13 girls at the age between 2 and 12 years. We analysed the clinical symptoms prior to the diagnosis

Torsion of extralobar pulmonary sequestration: a case report.

Extralobar pulmonary sequestration (ELS) with torsion is rare in adolescents and mimics posterior mediastinal tumours, posing diagnostic challenges. An 11-year-old boy presented with acute chest pain. Contrast-enhanced computed tomography revealed a non-enhancing homogeneous mass with pleural effusion, while magnetic resonance imaging showed isointense T1 and hypointense T2 signals. Despite initial uncertainty regarding neoplasm, the acute presentation contrasted with the indolent course of posterior mediastinal tumours. History of abnormal prenatal ultrasonography facilitated diagnosis of tor

Thoracic follicular dendritic cell sarcoma - an outlandish presentation of a rare tumour with review of literature.

Follicular dendritic cell sarcoma is a rare low grade malignant neoplasm that arises from follicular dendritic cells in lymphoid tissue germinal centres and accounts for 0.4% of all soft tissue sarcomas. It is extremely rare to have pulmonary follicular dendritic cell sarcoma with endobronchial extension and as an anterior mediastinal mass with mediastinal lymph node involvement. We present the case of a 34-year-old male non-smoker who had been experiencing chest pain for three months. A lobulated left peri-hilar mass with endobronchial spread into the left main bronchus and mediastinal lympha

Sclerosing thymoma: A rare case report and brief review of literature.

RATIONALE: Sclerosing thymoma is an extremely rare mediastinal neoplasm; it was recognized for the first time in 1994 and to date only 15 cases have been reported. PATIENT CONCERNS: The present study report a case of a 65-year-old man who was incidentally found to have an anterior mediastinal nodule, without clinical symptoms including fever, chest pain, and myasthenia gravis. DIAGNOSES: The chest computed tomography (CT) revealed the nodule was 4.9 × 4.2 × 3.0 cm in size. And the microscopic and immunohistochemical findings indicated that the final diagnosis was sclerosing thymoma. INTERVE

Atypical intermediate-grade mediastinal carcinoid. Case presentation.

BACKGROUND: Approximately 25% of carcinoid tumors develop in the respiratory system. Neuroendocrine carcinoids represent ~5% of all mediastinal tumors and 1-5% of all intrathoracic neoplasms. They contain numerous neurosecretory granules that synthesize, store and release neurohumoral substances that can induce the carcinoid syndrome. CLINICAL CASE: A 21-year-old male presented with a rapidly progressive paraneoplastic syndrome unleashed by an acute urethritis. Two left mediastinal masses were identified and resected. Postoperative evolution has been uneventful during the first year. CONCLUS

When rarity meets thoracic cancers: a narrative review from ITMIG 2024.

BACKGROUND AND OBJECTIVE: Rare thoracic cancers (RTCs) comprise a heterogeneous group of malignancies characterized by low incidence, high histological diversity, and significant clinical challenges. Their rarity often results in delayed diagnoses, lack of standardized therapeutic approaches, and limited prospective clinical trials. The absence of robust data is compounded by the fragmentation of expertise across institutions, underscoring the need for centralized, multidisciplinary management and international collaboration. This article aims to provide an overview of three representative RTC

Advancement in Diagnostic Imaging of Thymic Tumors.

Thymic tumors are rare neoplasms even if they are the most common primary neoplasm of the anterior mediastinum. In the era of advanced imaging modalities, such as functional MRI, dual-energy CT, perfusion CT and radiomics, it is possible to improve characterization of thymic epithelial tumors and other mediastinal tumors, assessment of tumor invasion into adjacent structures and detection of secondary lymph nodes and metastases. This review aims to illustrate the actual state of the art in diagnostic imaging of thymic lesions, describing imaging findings of thymoma and differential diagnosis.

Tuberculosis presenting as Anterior Mediastinal Mass: A Case Report.

The mediastinum, located between the pleural sacs, has three compartments. The anterior mediastinum spans anteriorly from the sternum to the pericardium and brachiocephalic vessels posteriorly. Common lesions in this area include thymomas, lymphomas, teratomatous neoplasms, and thyroid masses. A mediastinal mass in the setting of tuberculous meningoencephalitis is an uncommon presentation of tuberculosis. We present a case of a 20-year-old girl with fever and headache diagnosed with tuberculous meningoencephalitis. A thorough workup revealed an anterior mediastinal mass, histopathologically di

[Primary pharyngolaryngeal carcinoma, pulmonary metastases and carcinoma of the bronchus (author's transl)].

On the basis of 112 cases of pulmonary or mediastinal neoplasms observed during the treatment or follow-up of 952 carcinomas of the pharynx and larynx, the authors study in detail 45 cases of metastases. They draw the following conclusions:--metastases present clinically relatively early in the treated course of pharyngolaryngeal carcinomas: 85% before 18 months.--cannonball lesions are the commonest endo-thoracic clinical metastases: 6 out of 10.--carcinomas of the hypopharynx are at the origin of 2/3 of cannonball lesions (alpha less than or equal to 0.02) and 1/4 of mediastinal lymphadenopa

Carcinoid tumor of the anterior mediastinum in a 38-year-old woman.

Mediastinal neuroendocrine tumors (NETs) are very rare. They have been estimated to account for approximately 2%-4% of all anterior mediastinal neoplasms. Carcinoid tumors are a type of NET that can occur in a number of locations. They arise from endocrine amine precursor uptake and decarboxylation cells that can be found in organs such as the lungs. In general, they are slow growing tumors but are nevertheless capable of metastasizing. We present the case of a carcinoid tumor of the anterior mediastinum in a 38-year-old woman presented to our hospital with a 4-month history of worsening breat

Hemangioma in the anterior mediastinum.

Anterior mediastinal hemangiomas are very rare neoplasms in mediastinal tumors. A 58-year-old woman was revealed to have a mass measuring 4 x 3 cm in size in the anterior mediastinum with calcification on computed tomography. It was initially suspected to be a thymoma. We performed tumor extirpation in November 1998. The tumor was close to the thymus and slightly adhered to the superior vena cava, ascending aorta and right phrenic nerve, however, it did not invade any surrounding organs. Histopathologically, it was diagnosed to be a venous type hemangioma composed of vessels covered by smooth

Breath on the Brink: Navigating Anaesthesia Management in a Case With a Mediastinal Mass.

Mediastinal teratomas are rare, often asymptomatic, but clinically significant neoplasms that can manifest with a spectrum of symptoms, frequently attributed to the compression of surrounding critical anatomical structures. Here, we present the case of a 19-year-old male with respiratory distress and chest pain attributed to a large anterior mediastinal mass, ultimately diagnosed as a benign mature teratoma of the thymus. Radiological imaging revealed a large, partially cystic mass compressing the pulmonary arteries, aortic arch, and left main bronchus. Surgical excision was deemed necessary d

Right video-assisted thoracoscopic enucleation of an upper thoracic esophageal leiomyoma.

Leiomyomas are rare benign tumors of the esophagus that account for about 4% of all esophageal tumors. Recently, the great improvements in minimally invasive techniques have made the resection of mediastinal and pulmonary neoplasms more feasible through video-assisted thoracoscopic or robotic surgery. Our video tutorial describes our approach for a right-sided 3-port thoracoscopic resection of an upper esophageal leiomyoma.

Paediatric mesenchymal tumors of the mediastinum.

Paediatric mesenchymal tumors of the mediastinum are extremely rare tumors forming approximately 2% of all mediastinal neoplasms. There are subtle differences in the clinical and histomorphological profile of these tumors from their adult counterparts. The management of these tumors in the children is primarily surgical. This review was done to perform a comprehensive analysis of these mesenchymal tumors in the paediatric age group.
