High risk channeling to improve Medicaid maternal and infant care.

To improve the medical care access of the poorest women and newborns, South Carolina began a High Risk Channeling Project since April 1986. The Project directs physicians to screen all Medicaid-eligible pregnant women and newborns for specified clinical high risk factors. High risk patients are channeled to designated clinics for prenatal and newborn care. Channeled pregnant women are directed to deliver their babies at regional referral hospitals. For the first two years of the Project, about two-thirds of pregnancies and 60% of newborns were actually screened. Channeled women were much more

Postpartum contraceptive use among Medicaid beneficiaries following Idaho's abortion ban.

OBJECTIVES: Abortion bans have been associated with decreased reproductive health care access. We examined the association between Idaho's abortion ban and postpartum contraceptive use among Medicaid beneficiaries. STUDY DESIGN: This retrospective cohort study used Medicaid claims data from Idaho and Oregon with a difference-in-differences approach. We included Medicaid-enrolled women aged 15-44 who delivered a live birth between July 1, 2021, and September 30, 2023. Idaho's abortion ban, implemented August 25, 2022, criminalized abortion from fertilization with narrow exceptions; Oregon serv

Postpartum Medicaid extensions and follow-up care after pregnancy complications.

INTRODUCTION: For women with pregnancy complications, such as hypertensive disorders or gestational diabetes, the postpartum period is a critical, yet underutilized opportunity to prevent both pregnancy-related morbidity and longer-term chronic disease. Loss of insurance coverage postpartum is a barrier to treatment and prevention strategies. METHODS: We examined if extending Medicaid through 12 months postpartum increased postpartum health care utilization for women with pregnancies complicated by diabetes or hypertension. We analyzed 2019-2023 deliveries in Medicaid claims data from New Yor

SEER-Medicaid and SEER-Medicare data for neuro-oncology research.

PURPOSE: The Surveillance, Epidemiology, and End Results (SEER) program is a robust resource for U.S. cancer surveillance, capturing cancer patient data on diagnosis, treatment, survival, and demographics across diverse population-based cancer registries. The linkage of SEER data with Medicare and, more recently, Medicaid, enhances its utility for understanding treatment access, quality, and outcomes, as well as health policy impacts, on patients with brain tumors. This review describes the SEER-Medicaid and SEER-Medicare data resources and provides practical guidance for researchers to suppor

Medicaid Expansion and Overall Mortality Among Women With Breast Cancer.

IMPORTANCE: Medicaid expansion under the Patient Protection and Affordable Care Act (ACA) was designed to improve access to care and reduce health disparities. Its association with breast cancer mortality and related disparities remains unclear. OBJECTIVES: To evaluate the association between Medicaid expansion and overall mortality among women with breast cancer and to assess whether survival differs by race and ethnicity, disease stage, income, and treatment modality. DESIGN, SETTING, AND PARTICIPANTS: This retrospective, hospital-based cohort study used a National Cancer Database cohort o

Timing of Medicaid Enrollment during Pregnancy and Associated Infant Outcomes.

OBJECTIVE: To describe the timing of Medicaid enrollment during pregnancy among Medicaid-insured women in Texas, examine patterns of healthcare utilization following enrollment, and evaluate associations between enrollment timing and infant outcomes. METHODS: Retrospective cohort study of 913,229 deliveries to women aged 15-44 years enrolled in Texas Medicaid between January 1, 2010, and December 31, 2014. Women were classified into five mutually exclusive groups based on timing of first Medicaid enrollment: before conception, first, second, or third trimester, or delivery hospitalization. We

Medicaid Unwinding: Association With New and Ending Buprenorphine Treatment Episodes.

IMPORTANCE: Medicaid is an important source of coverage for buprenorphine treatment for opioid use disorder (OUD). The loss of Medicaid coverage through Medicaid unwinding may substantially impact individuals' ability to initiate and continue buprenorphine, but little is known about how Medicaid unwinding is associated with buprenorphine OUD treatment. OBJECTIVE: Examine the association of Medicaid unwinding overall and by magnitude of unwinding with changes in buprenorphine OUD treatment. DESIGN, SETTING, AND PARTICIPANTS: Retrospective cohort study using 2021-2023 national retail pharmacy

Medicaid Expansion and Survival Outcomes Among Men With Prostate Cancer.

INTRODUCTION: Prostate cancer stands as one of the most diagnosed malignancies among men worldwide. With the recent expansion of Medicaid under the Affordable Care Act (ACA), millions more Americans now have health insurance coverage, potentially influencing healthcare access and subsequent outcomes for various illnesses, including prostate cancer. Yet, the direct correlation between Medicaid expansion and cancer-specific survival among individuals with prostate cancer remains an area warranting comprehensive exploration. OBJECTIVE: This study aims to determine the impact of the implementatio

Utilization and patient experiences of telehealth among Medicaid expansion enrollees.

Telehealth can promote access to health care, especially among individuals enrolled in Medicaid. However, we currently lack data on patient experiences of telehealth from low-income individuals. Using data collected in 2021 and 2022 from a statewide survey of enrollees in the Heathy Michigan Plan, Michigan's Medicaid expansion program, we conducted an analysis to study patients' experiences of using telehealth. Approximately one-third of enrollees reported a telehealth visit within the past year. Individuals reporting barriers to primary care were more likely to have had a telehealth visit com

Medicaid Waivers to Address Homelessness: Political Development and Policy Trajectories.

This article reviews the role of Medicaid waivers in homeless policy and their emerging role as a mechanism to address homelessness. The authors evaluate the political development of waivers in housing and homeless policy over the past thirty years, and they investigate the status of current and approved waivers targeting homelessness. They then consider how waivers may shape homeless policy governance going forward (including the success of existing systems), and they address implementation and efficacy questions related to the role of health care payers in solutions to homelessness. The auth

Medicaid expansion and palliative care for advanced-stage liver cancer.

BACKGROUND: Medicaid expansion (ME) has contributed to transforming the United States healthcare system. However, its effect on palliative care of primary liver cancers remains unknown. This study aimed to evaluate the association between ME and the receipt of palliative treatment in advanced-stage liver cancer. METHODS: Patients diagnosed with stage IV hepatocellular carcinoma or intrahepatic cholangiocarcinoma were identified from the National Cancer Database and divided into pre-expansion (2010-2013) and postexpansion (2015-2019) cohorts. Logistic regression identified predictors of pallia

Medicaid Expansion and Mortality Among Persons Who Were Formerly Incarcerated.

IMPORTANCE: Since 2014, Medicaid expansion has been implemented in many states across the US, increasing health care access among vulnerable populations, including formerly incarcerated people who experience higher mortality rates than the general population. OBJECTIVE: To examine population-level association of Medicaid expansion with postrelease mortality from all causes, unintentional drug overdoses, opioid overdoses, polydrug overdoses, suicides, and homicides among formerly incarcerated people in Rhode Island (RI), which expanded Medicaid, compared with North Carolina (NC), which did not

Medicaid Enrollment and Intergenerational Transfers of Wealth Among Older Adults.

BACKGROUND AND OBJECTIVES: Medicaid look-back periods are meant to prevent Medicaid applicants from gifting assets to meet eligibility requirements. These policies have the potential to affect families across generations given their ability to restrict the transfer of assets between parent and child. RESEARCH DESIGN AND METHODS: Using 2008-2018 data from the Health and Retirement Study, this study analyzed the estate planning and familial wealth transfer behaviors of a cohort of older adults aged 65 and older who became Medicaid recipients during a 10-year period. RESULTS: There were 8,347 r

Polypharmacy among medicaid-insured children with and without documented obesity.

OBJECTIVE: Polypharmacy increases the risk of drug-drug interactions and adverse drug events. As obesity and rates of obesity-associated comorbid chronic conditions continue to rise, an improved understanding of whether children with obesity experience higher risk of polypharmacy is needed. This study aimed to compare chronic medication polypharmacy prevalence among children with and without a diagnosis of obesity. METHODS: We performed a cross-sectional examination of prescription data for children aged 2-18 years prescribed ≥1 chronic medication using the 2019 Marketscan Medicaid database.

Do Medicaid Expenditures Increase After Adults Exit Permanent Supportive Housing?

The effects of homelessness and permanent supportive housing (PSH) on health care utilization have been well documented. Prior research on the association between PSH entry and Medicaid expenditures have indicated that such housing support could result in savings to Medicaid programs; however, whether changes occur in health care use and expenditures after individuals exit PSH is unknown. If efficiency gains from PSH persist after the individual leaves PSH, the savings to payers such as Medicaid may continue even after the costs to provide housing for a PSH recipient have ended. We used linked

Medicaid Expansion and Contraceptive Use Among Female High-School Students.

INTRODUCTION: Access to effective contraception prevents unintended pregnancies among sexually active female youth. Potentially impacted by the Affordable Care Act's Medicaid-related policies, contraception use increased among sexually active high-school students from 2013 to 2019. METHODS: Analyses conducted in 2021 assessed state-level Youth Risk Behavior Survey data among female students in grades 9-12 who reported being sexually active. States that expanded Medicaid were compared with other states in 2013 (baseline) and 2019 (after expansion). Measured outcomes included self-reported use

Prenatal Syphilis Screening Among Medicaid Enrollees in 6 Southern States.

INTRODUCTION: The rates of syphilis among pregnant women and infants have increased in recent years, particularly in the U.S. South. Although state policies require prenatal syphilis testing, recent screening rates comparable across Southern states are not known. The purpose of this study is to measure syphilis screening among Medicaid enrollees with delivery in states in the U.S. South. METHODS: A total of 6 state-university research partnerships in the U.S. South developed a distributed research network to analyze Medicaid claims data using a common analytic approach for enrollees with deli

Drug costs of Medicaid-covered therapies for pemphigus vulgaris treatment.

Pemphigus vulgaris is the most common form of pemphigus affecting an estimated 30,000-40,000 people in the United States. Costs of systemic and immunoglobulin therapies for pemphigus vulgaris have remained persistently high. Herein, we address the current costs and changes in costs of immunosuppressive treatments, anti-inflammatory treatments, and immunoglobulin treatments covered by Medicaid for pemphigus vulgaris from 2013-2020.

Medicaid Policy and Liver Transplant for Alcohol-Associated Liver Disease.

BACKGROUND AND AIMS: In some states, liver transplantation (LT) for alcohol-associated liver disease (ALD) is covered by Medicaid only with documentation of abstinence and/or alcohol rehabilitation. Different Medicaid policies may affect the distribution of LT for ALD, particularly post-2011, as centers have adopted early (i.e., specific abstinence period not required) LT practices. APPROACH AND RESULTS: We surveyed Medicaid policies in all states actively performing LT and linked state policies to prospectively collected national registry data on LT recipients from 2002 to 2017 with ALD as t
