Does date stamping ICD-9-CM codes increase the value of clinical information in administrative data?

CONTEXT: Comorbidity measures are designed to exclude complications when they map International Classification of Diseases (ICD-9-CM) codes to diagnostic categories. The use of data fields that indicates whether each secondary diagnosis was present at the time of hospital admission may lead to the more accurate identification of preexisting conditions. OBJECTIVE: To examine the rate of misclassification of ICD-9-CM codes into diagnostic categories by the Dartmouth-Manitoba adaptation of the Charlson index and by the Elixhauser comorbidity algorithm. DATA SOURCE: Analysis of 178,838 patients

The acute abdomen: the role of computer-assisted diagnosis.

CAD has been used in over 20,000 patients in an Accident and Emergency Department. Structured documentation has replaced normal case notes and a patient record is created which includes investigations and management. A copy of the documentation acts as an immediate communication to the general practitioner. The information is held on a computer and is readily accessed to produce feedback and to allow audit of any number of parameters. The results show that an improvement in diagnostic accuracy and a reduction in admissions and unnecessary laparotomies continues over 15 years. Problems have occ

Physician self-audit: a scoping review.

INTRODUCTION: Self-audit involves self-collection of personal performance data, reflection on gaps between performance and standards, and development and implementation of learning or quality improvement plans by individual care providers. It appears to stimulate learning and quality improvement, but few physicians engage in self-audit. The purpose of this study was to identify how self-audit has been operationalized; factors influencing self-audit conduct and outcomes, including program design; and issues warranting further research. METHODS: A systematic review of quantitative and qualitati

Introducing a Modified Grenoble Classification for induction of labour with an audit at a regional maternity unit in Australia.

OBJECTIVE: Rates of induction of labour (IOL) have dramatically risen in recent years. Currently no universally adopted classification system for IOLs exists. A comprehensive classification system would enable analyses of trends and benchmarking of practices. A previously proposed classification attempted to address this issue, but it did not appear to be fully comprehensive, as it did not account for IOL for preterm prelabour rupture of membranes. Furthermore, without subcategorization to reflect some common indications the utility of the classification system is limited. We have aimed to cre
