Electronic Medical Record Context Signatures Improve Diagnostic Classification Using Medical Image Computing.

Composite models that combine medical imaging with electronic medical records (EMR) improve predictive power when compared to traditional models that use imaging alone. The digitization of EMR provides potential access to a wealth of medical information, but presents new challenges in algorithm design and inference. Previous studies, such as Phenome Wide Association Study (PheWAS), have shown that EMR data can be used to investigate the relationship between genotypes and clinical conditions. Here, we introduce Phenome-Disease Association Study to extend the statistical capabilities of the PheW

Documentation-related diagnostic safety concerns in primary care: a structured medical record review of 1116 patient records.

BACKGROUND: Diagnostic safety concerns are among the most consequential yet least visible threats to patient safety in primary care. Non-specific symptoms potentially associated with cancer represent a high-risk context, where delayed diagnosis may have serious consequences despite a low probability of serious disease. OBJECTIVE: To identify diagnostic safety concerns and areas for improvement in assessing patients presenting with non-specific symptoms in primary care. METHOD: A structured medical record review combining quantitative assessment and qualitative thematic analysis was conducted

From data entry to digital transformation: Allied health perspectives on standardised electronic medical records data.

BACKGROUND: Electronic medical records (EMRs) currently rely on standardised data fields to support secondary data use for clinical care, performance monitoring, and system-level reporting. However, utilisation of standardised data capture and reporting within allied health remains underdeveloped in practice. Greater understanding of how allied health clinicians and managers perceive the purpose, value, and impact of standardised data collection is needed to support advancement in data maturity and transformation of utilisation. OBJECTIVE: To explore the perceived benefits, challenges and fut

Preconceptual counselling for women with epilepsy - is it happening? A retrospective observational medical records review.

Epilepsy in pregnancy is common, but it remains a major risk factor for maternal morbidity and mortality. Over the last decade, neurological causes have remained in the top three indirect causes of maternal death in the UK. National reports, including the recent maternity care bundle, have emphasised the importance of preconceptual counselling to optimise maternal health and reduce complications for epileptic women during pregnancy and postpartum. This study is a retrospective medical record review, examining records of epileptic women of childbearing age, taken from a general practitioner (GP

Leveraging health records to identify diagnoses associated with recurrent pregnancy loss across two medical centers.

Recurrent pregnancy loss (RPL), defined as 2 or more pregnancy losses, affects 5-6% of ever-pregnant individuals. Approximately half of these cases have no identifiable explanation. In this study, we aim to identify diagnoses associated with RPL and generate hypotheses about RPL etiology utilizing electronic health record (EHR) data. We implemented a case-control study comparing the history of over 1,600 diagnoses between RPL and live birth patients, leveraging the University of California San Francisco (UCSF) and Stanford University EHR databases. In total, our study includes 8,496 RPL (UCSF:

Exploring Technological Solutions for Interoperability Between Patient Electronic Medical Records and Clinical Registries: Scoping Review.

BACKGROUND: The use of electronic medical records (EMRs) and clinical registries has transformed health care delivery by improving data management, care coordination, and research capacity. However, the full potential of these technologies can only be realized through effective interoperability, thereby reducing the burden of manual data entry and enhancing the use of real-world clinical data. OBJECTIVE: This review examines technologies that enable automated data extraction and transfer, which promote interoperability between EMRs and clinical registries. METHODS: A search of PubMed, CINAHL

Cancer Cachexia Prevalence Is Underestimated in Medical Records of Patients in a Regional Tertiary Hospital.

BACKGROUND: Widespread lack of awareness and limited real-world prevalence evidence have impeded cachexia care and research. We hypothesized that healthcare professionals may identify the term cachexia, leading to International Classification of Diseases (ICD) coding for this term, with or without records of body weight loss for diagnosing cancer cachexia, and that frequently, ICD coding does not accurately reflect weight data. METHODS: We assessed cachexia prevalence in patients diagnosed with cancer, using the Clinical Data Warehouse of a French hospital containing two types of real-world d

Treating chronic atrophic gastritis: identifying sub-population based on real-world TCM electronic medical records.

BACKGROUND AND OBJECTIVE: Chronic atrophic gastritis (CAG) is a complex chronic disease caused by multiple factors that frequently occurs disease in the clinic. The worldwide prevalence of CAG is high. Interestingly, clinical CAG patients often present with a variety of symptom phenotypes, which makes it more difficult for clinicians to treat. Therefore, there is an urgent need to improve our understanding of the complexity of the clinical CAG population, obtain more accurate disease subtypes, and explore the relationship between clinical symptoms and medication. Therefore, based on the integr

Discrepancies in Skin Cancer Screening Reporting Among Patients, Primary Care Physicians, and Patient Medical Records.

Skin cancer incidence in the United States has risen rapidly in recent decades, underscoring the need for accessible and effective prevention practices. The full-body skin examination (FBSE) is the quintessential tool for secondary skin cancer prevention, but the US Preventive Services Task Force (USPSTF) states there is insufficient evidence to recommend the examination for the general or at-risk population. Variable performance of FBSEs among primary care providers (PCPs) is a barrier to accurate studies, and variability in measurement of that performance can be a major impediment to assessm

MLEE: A method for extracting object-level medical knowledge graph entities from Chinese clinical records.

As a typical knowledge-intensive industry, the medical field uses knowledge graph technology to construct causal inference calculations, such as "symptom-disease", "laboratory examination/imaging examination-disease", and "disease-treatment method". The continuous expansion of large electronic clinical records provides an opportunity to learn medical knowledge by machine learning. In this process, how to extract entities with a medical logic structure and how to make entity extraction more consistent with the logic of the text content in electronic clinical records are two issues that have bec

Developing an operational definition of housing instability and homelessness in Veterans Health Administration's medical records.

The main objective of this study was to examine how homelessness and housing instability is captured across data sources in the Veterans Health Administration (VHA). Data from 2021 were extracted from three data repositories, including the Corporate Data Warehouse (CDW), the Homeless Operations Management System (HOMES), and the Homeless Management Information System (HMIS). Using these three data sources, we identified the number of homeless and unstably housed veterans across a variety of indicators. The results showed that the use of diagnostic codes and clinic stop codes identified a large
