Standard Elements in Studies of Adverse Events and Medical Error (SESAME): explanation and elaboration.

Studies that evaluate adverse events and medical error often suffer from incomplete and inadequate reporting, which affects the interpretability and reproducibility of their findings. To address this, we developed the 44-item Standard Elements in Studies of Adverse Event or Medical Error (SESAME) checklist using a rigorous process following the recommendations of the Enhancing the QUAlity and Transparency Of health Research Network. The aim of this manuscript is to explain the scope and aim of the checklist by providing a detailed explanation of each item in the SESAME checklist, illustrated b

Bayesian robust symmetric regression for medical data with heavy-tailed errors and censoring.

Bayesian symmetric regression offers a principled framework for modeling data characterized by heavy-tailed errors and censoring, both of which are frequently encountered in medical research. Classical regression methods often yield unreliable results in the presence of outliers or incomplete observations, as commonly seen in clinical and survival data. To address these limitations, we develop a robust Bayesian regression model that incorporates symmetric error distributions such as the Student-t and Cauchy, providing improved resistance to extreme values. The model also explicitly accounts fo

Suitability of Hair Testing in Medical Dispensing Errors: A Fatal Case of Gliclazide Poisoning.

A patient was prescribed a new treatment, 40 mg furosemide. Her pharmacist mistakenly dispensed 40 mg gliclazide instead. After 3 weeks of treatment, the patient was found in a coma, was hospitalised and died after a week. Hair sample was collected during post-mortem examination and tested for gliclazide on three 2 cm sections, starting at the root end. Hair strands were decontaminated, segmented and incubated in the presence of diazepam-d5, and a solid-liquid extraction has been performed. Finally, toxicological analyses were performed by UFLC Shimadzu Prominence - MS/MS Sciex 6500 QTRAP. Gli

Inadequacies of Physical Examination as a Cause of Medical Errors and Adverse Events: A Collection of Vignettes.

BACKGROUND: Oversights in the physical examination are a type of medical error not easily studied by chart review. They may be a major contributor to missed or delayed diagnosis, unnecessary exposure to contrast and radiation, incorrect treatment, and other adverse consequences. Our purpose was to collect vignettes of physical examination oversights and to capture the diversity of their characteristics and consequences. METHODS: A cross-sectional study using an 11-question qualitative survey for physicians was distributed electronically, with data collected from February to June of 2011. The

Nurse-Led Interventions to Mitigate High-Alert Medication Errors in Critical Care: A Systematic Review of Effectiveness and Implementation Strategies.

BACKGROUND: Intensive care units (ICUs) are high-risk environments where nurses administer the majority of high-alert medications, making them the final checkpoint in the medication-use process. However, evidence on nurse-led interventions specifically targeting high-alert medication errors has not been systematically synthesized across all ICU populations. AIM: This systematic review evaluated the effectiveness of nurse-led interventions in reducing high-alert medication errors in neonatal, paediatric and adult intensive care units and examined implementation strategies that enhance patient

Two decades of medication administration errors related to pain management: Hospital admissions for nonopioid analgesics, antipyretics, and antirheumatics in Australia.

The rate of medication poisoning is constantly increasing globally, increasing the number of hospital admissions and the burden on the health system. In Australia, nonopioid analgesics, antipyretics, and antirheumatics medications cause a high rate of accidental poisonings. This study aims to study hospital admissions trends related to medication administration errors (MAEs; poisoning by, adverse effect of and underdosing of) of nonopioid analgesics, antipyretics, and antirheumatics-related in Australia from 1998 to 2022. This ecological study used hospital admissions data from the National Ho

Unintentional Incorrect Route of Drug Administration: An Analysis of Spontaneous Reports of Medication Errors in Children and Adolescents from Germany.

BACKGROUND AND OBJECTIVE: Administering a drug unintentionally via an incorrect route, for example intravenously instead of orally, can lead to serious adverse drug reactions. Although individual cases of incorrect routes of drug administration (IRDA) in children and adolescents have been described in the literature, a systematic analysis of a larger number of cases is lacking. The aim of this exploratory study was to analyse unintentional IRDA errors related to chemically defined drugs in two spontaneous reporting databases focusing on the drugs, dosage forms, patients involved and the clinic

Medication Preparation Errors Involving Very Small and Difficult-to-measure Volumes in a Pediatric Hospital: A 14-year Descriptive Study.

PURPOSE: Medication preparation errors are a major source of preventable harm in pediatric hospitals. Oral liquid medications, especially those involving difficult-to-measure or very small volumes, may increase the risk of such errors, yet supporting evidence is limited. This study aims to determine whether difficult-to-measure and very small oral liquid medication volumes are associated with higher rates of preparation errors in a pediatric hospital pharmacy. METHODS: We conducted a retrospective cohort study of all oral liquid medication preparations produced between January 2010 and Decemb

A pharmacist-led information technology intervention for medication errors (PINCER): a multicentre, cluster randomised, controlled trial and cost-effectiveness analysis.

BACKGROUND: Medication errors are common in primary care and are associated with considerable risk of patient harm. We tested whether a pharmacist-led, information technology-based intervention was more effective than simple feedback in reducing the number of patients at risk of measures related to hazardous prescribing and inadequate blood-test monitoring of medicines 6 months after the intervention. METHODS: In this pragmatic, cluster randomised trial general practices in the UK were stratified by research site and list size, and randomly assigned by a web-based randomisation service in blo
