Surgery for patients with endometrioid-type endometrial cancer: is lymphadenectomy above the inferior mesenteric artery necessary?

OBJECTIVE: The primary objective of this study was to identify the risk of metastasis to lymph nodes above the inferior mesenteric artery (IMA) in endometrioid-type endometrial cancer (EC) and the factors that influence metastasis. METHODS: The study included patients who had been operated on for endometrioid-type EC in three gynecological oncology centers between 2007 and 2023. The supramesenteric lymph node (SM-LN) is the region between the left renal vein and the IMA, whereas the inframesenteric lymph node (IM-LN) is the region between the IMA and the aortic bifurcation, as determined by t

Arteriocolonic fistula of inferior mesenteric artery aneurysm: A case with lower gastrointestinal and intra-abdominal hemorrhage.

INTRODUCTION: Arteriocolonic fistula of Inferior Mesenteric Artery Aneurysm (IMAA) refers to a spontaneous formation of pathological communication between the artery and the adjacent gastrointestinal tract. It is a rare, life-threatening condition primarily manifesting as abdominal pain, gastrointestinal bleeding, abdominal pulsating masses. However, its clinical manifestations are usually atypical with a difficult diagnosis and treatment. CASE PRESENTATION: We report a rare case of a 50-year-old male with a hemorrhagic shock due to primary arteriocolonic fistula of IMAA. Instead of sigmoidec

Impact of the Patency of Inferior Mesenteric Artery on 7-Year Outcomes After Endovascular Aneurysm Repair.

PURPOSE: The impact of preoperative patent inferior mesenteric artery (IMA) on late outcomes following endovascular aneurysm repair (EVAR) remains unclear. This study aimed to investigate the specific influence of IMA patency on 7-year outcomes after EVAR. MATERIALS AND METHODS: In this retrospective cohort study, 556 EVARs performed for true abdominal aortic aneurysm cases between January 2006 and December 2019 at our institution were reviewed. Endovascular aneurysm repairs performed using a commercially available device with no type I or type III endoleak (EL) during follow-up and with foll

Delayed bowel necrosis due to inferior mesenteric artery occlusion in Stanford type A acute aortic dissection.

Although inferior mesenteric artery occlusion due to acute aortic dissection sometimes occurs, it is usually not considered an important finding. Herein, we present an extremely rare case of delayed bowel ischaemia due to inferior mesenteric artery occlusion in Stanford type A acute aortic dissection that highlights the need for cardiac surgeons to be mindful of inferior mesenteric artery occlusion in patients with superior mesenteric artery dissection or vascular anomalies in the mesenteric arteries.

Preoperative carbon nanoparticle injection improves inferior mesenteric artery lymph node retrieval in patients with rectal cancer.

BACKGROUND: Inferior mesenteric artery lymph node (station 253 node) metastasis occurs in approximately 0.3% to 13.9% of rectal cancer patients. This study examined whether carbon nanoparticles could aid in harvesting more station 253 nodes and evaluated the relationship between the number of station 253 nodes retrieved and station 253 node metastasis. METHOD: A total of 480 consecutive rectal cancer patients were enrolled in this prospective cohort study between August 2014 and October 2018. Ninety-one patients (18.96%) received a preoperative submucosal injection of carbon nanoparticles (CN

Short- and long-term follow-up after transarterial embolization of a giant inferior mesenteric artery aneurysm.

Among the cases of visceral artery aneurysms, those that involve the inferior mesenteric artery are the most uncommon, with a prevalence of <1%. This study reports a giant symptomatic inferior mesenteric artery aneurysm and its endovascular treatment by means of percutaneous embolization with the implantation of pushable coils. After complete occlusion of the aneurysm, adequate maintenance of arterial supply of the left colon and resolution of the symptoms were observed. In the 2-year follow-up, a significant reduction in the aneurysm's volume and maintenance of the clinical condition were obs

[Prevention of colonic ischemia following abdominal aortic aneurysmectomy by measurement of inferior mesenteric artery stump pressure].

To prevent postoperative colon ischemia, the inferior mesenteric artery (IMA) stump pressure measurement was performed in 60 patients undergoing unruptured abdominal aortic aneurysmectomy. IMA stump pressures were more than 40mmHg and IMA/systemic pressure ratios were more than 0.4 in 58 patients. The IMA was ligated in all of these patients. Colon ischemia developed in two of these 58 patients. In one, vigorous retractive trauma to the colon and the mesenterium seemed to be causative, and in the other, division of the branches of the IMA was causative. None of the other 56 patients developed

Atherosclerotic inferior mesenteric artery stenosis resulting in large intestinal hypoperfusion: a paradigm shift in the diagnosis and management of symptomatic chronic mesenteric ischemia.

Symptomatic chronic mesenteric ischemia results from intestinal hypoperfusion and is classically thought to result from involvement of two or more mesenteric arteries. The celiac artery and superior mesenteric artery are most frequently implicated in this disease process, and their involvement usually results in symptoms of small intestinal ischemia. Symptomatic chronic mesenteric ischemia resulting predominantly from inferior mesenteric artery involvement has largely been overlooked but does gives rise to its own, unique clinical presentation with symptoms resulting from large intestinal isch

Preoperative Risk Stratification for Sac Enlargement and Reintervention Using Inferior Mesenteric Artery Patency and Lumbar Artery Burden After Endovascular Aneurysm Repair.

OBJECTIVE: To determine the optimal number of residual lumbar arteries (LAs) according to inferior mesenteric artery (IMA) patency and assess their associations with sac enlargement and reintervention after endovascular aneurysm repair (EVAR). METHODS: This retrospective single-center study included 335 patients who underwent EVAR for abdominal aortic aneurysm. Patients were stratified by postoperative IMA patency and residual lumbar artery (LA) count. Kaplan-Meier analyses with three prespecified dichotomizations were used to determine optimal risk-stratification cutoffs. RESULTS: Among 335

Aberrant Left Testicular Artery Originating from the Inferior Mesenteric Artery Identified on Angiography in a Patient with Gastrointestinal Bleeding: Case Report.

Background and Clinical Significance: This case presents a rare variation in mesenteric and pelvic vasculature that holds relevance for endovascular procedures. Limited published cases of the testicular artery arising off the inferior mesenteric artery exist in the literature and play an important role in clinical outcomes. Case Presentation: An 89-year-old male presented with gastrointestinal bleeding from diverticulosis. During an arteriogram to locate and assess sigmoid arteries for embolization, an unusual anatomical variant of the left testicular artery was discovered. Typically, the left

Novel indication for chimney graft placement in the inferior mesenteric artery in AAA patients with coexistent bilateral internal iliac artery occlusion.

PURPOSE: To report a novel indication for the use of chimney grafts to preserve flow to the inferior mesenteric artery (IMA) in patients undergoing endovascular aneurysm repair (EVAR) for aortobi-iliac aneurysms with coexistent bilateral occlusion of the internal iliac arteries (IIA). TECHNIQUE: Via a cutdown over the left axillary artery, a 5-F vertebral catheter was delivered over a conventional 0.035-inch hydrophilic guidewire to selectively catheterize the IMA via a 7-F shuttle sheath, which was then advanced at least 2 cm into the target vessel. The first chimney graft was advanced into

Chyle leak following root of mesentery dissection in pancreaticoduodenectomy with inferior infracolic superior mesenteric artery first approach.

BACKGROUND: The root of mesentery dissection is one of the critical maneuvers, especially in borderline resectable pancreatic head cancer. Intra-abdominal chyle leak (CL) including chylous ascites may ensue in up to 10% of patients after pancreatic resections. Globally recognized superior mesenteric artery (SMA) first approaches are invariably performed. The mesenteric dissection through the inferior infracolic approach has been discussed in this study emphasizing its post-operative impact on CL which is the cornerstone of this study. AIM: To assess incidence, risk factors, clinical impact of

Metastatic prostate cancer in the inferior mesenteric artery territory lymph nodes following anterior resection for colon cancer.

Prostate cancer metastasises via haematogenous or lymphatic routes and may be found at atypical sites. This is the first reported case of metastatic prostate cancer in the lymph nodes of the inferior mesenteric artery distribution following anterior resection for colon cancer in the South-East Asian population. Immunohistochemical staining was crucial to determine that the early staged colonic carcinoma was adequately resected and thus directs the focus of subsequent oncological treatment on the residual metastatic prostate carcinoma. This highlights the importance of communication between the

Covered endovascular reconstruction of the aortic bifurcation associated to a snorkel technique for inferior mesenteric artery preservation.

Aortic distal occlusion has been usually treated by open surgery. A persisting patent and overdeveloped inferior mesenteric artery (IMA) suggests a significant participation in the bowel circulation. Coverage of the inferior mesenteric artery (IMA) origin could compromise the bowel circulation. Covered endovascular reconstruction of the aortic bifurcation (CERAB) is a proven and guideline-suggested alternative for revascularization of the distal aortic occlusion. CERAB associated to a snorkel to the IMA aims to reduce the risk of bowel ischemia keeping and percutaneous and minimal invasive app

High Ligation of the Inferior Mesenteric Artery Induces Hypoperfusion of the Sigmoid Colon Stump During Anterior Resection.

Background: Anastomotic leakage (AL) after colorectal surgery is associated with insufficient vascular perfusion of the anastomotic ends. This study aimed to evaluate the effect of high vs. low ligation of the ileocolic artery and inferior mesenteric artery, respectively, on the vascular perfusion of the bowel stumps during ileocecal resection (ICR) and anterior rectal resection (AR). Methods: We retrospectively evaluated patients who underwent ICR or AR between 2016 and 2020. Real-time indocyanine green fluorescence angiography was performed to measure the fluorescence time (FT) as a marker o

Innovative Procedure for Inferior Mesenteric Artery Embolization Using the Amplatzer Vascular Plug 4 during Endovascular Aneurysm Repair.

PURPOSE: The aim of this study is to present an easy and quick technique for embolization of the inferior mesenteric artery (IMA) during endovascular aneurysm repair (EVAR). TECHNIQUE: We performed IMA embolization using a conventional EVAR device alone with the femoral artery approach during EVAR, which has not been reported previously. First, a 16F or 18F DrySeal Sheath is inserted from the femoral artery into the site around the IMA. Second, cannulation is performed in the IMA with an angiography catheter having a 0.038-inch inner lumen that is used for gate cannulation during EVAR. Third,
