Complete common mesentery revealed by a cecal bascule: Two case reports.

INTRODUCTION: The complete common mesentery (CCM) is due to a midgut malrotation that occurs during embryonic development. Cecal volvulus (CV) is an uncommon cause of acute intestinal obstruction in this circumstance. The bascule subtype is the rarest form and happens when the cecum folds anteriorly over the ascending colon. CASES PRESENTATION: We present two cases of acute intestinal obstruction caused by Cecal Bascule (CB) due to complete common mesentery. Notably, one patient was initially admitted wrongly for a strangulated umbilical hernia and underwent surgery without prior imaging. Bot

Glytan decreases portal pressure via mesentery vasoconstriction in portal hypertensive rats.

AIM: To investigate the effects of Glytan on splanchnic hemodynamics and its reduction of portal pressure in portal hypertensive rats. METHODS: Glytan (Ganluotong in Chinese), is composed of salvianolic acid B and diammonium glycyrrhizinate. Portal hypertension (PHT) was induced in the rats by common bile duct ligation (BDL). Hemodynamic studies were performed using the colored microsphere method. Radioimmunoassay (RIA) was used to determine endothelin (ET)-1 levels in the mesenteric circulation. Western blotting methods were used to investigate the effect of Glytan on ET A receptor (ETAR), E

Unique presentation of heterotopic pancreatic tissue arising from small-bowel mesentery.

Heterotopic pancreatic tissue in the small-bowel mesentery is an extremely rare anomaly: no such case has been reported in the English literature. We report a 5-year-old boy in whom the aberrant tissue formed a retroperitoneal cyst. The condition was further complicated by a ventral pancreatic duct defect. The unique clinical manifestations and difficulty in making the diagnosis are discussed.

Active oxidants mediate IFN-alpha-induced microvascular alterations in rat mesentery.

The present study was designed to investigate the influences of IFN-alpha on the microcirculatory hemodynamics. The mesenteric microcirculation of male Wistar rats was observed through an intravital fluorescence microscopic system. The leukocyte behavior, RBC velocity, and albumin leakage were monitored simultaneously before and after a continuous infusion of IFN-alpha. In other rats, the oxidant-sensitive fluorescence probe dihydrorhodamine-123 (DHR) fluorescence was observed in the same set up. Administration of IFN-alpha increased the number of adherent and emigrated leukocytes and decrease

Ectopic Pancreatic Adenocarcinoma Arising in the Mesentery of the Jejunum: A Case Report.

INTRODUCTION: Ectopic pancreas is defined as pancreatic tissue lacking anatomical and vascular continuity with the main pancreas. Malignant transformation within ectopic pancreatic tissue is extremely rare, and cases arising in the mesentery are exceptionally uncommon. We report a rare case of adenocarcinoma arising from ectopic pancreatic tissue in the proximal jejunal mesentery. CASE PRESENTATION: An 80-year-old man presented with abdominal pain. Contrast-enhanced CT showed a 3-cm mass adjacent to the proximal jejunum, initially suspected to be a jejunal submucosal tumor, such as gastrointe

Caecal volvulus around the mesentery of ileal neobladder: a case report.

BACKGROUND: Caecal volvulus occurring in the setting of ileal neobladder reconstruction is rare. We report a case of caecal volvulus occurring after urinary reconstruction to highlight the diagnostic challenges and surgical management. CASE PRESENTATION: We report the case of a 61‑year‑old male who presented with clinical features consistent with small bowel obstruction. Cross‑sectional imaging demonstrated a dilated small bowel with a transition point adjacent to the neobladder and mesenteric crowding suggestive of an internal hernia. The patient underwent exploratory laparotomy, which ident

Solitary Fibrous Tumour of the Mesentery: An Uncommon Site for a Rare Tumour.

Solitary fibrous tumour (SFT), a unique spindle-cell neoplasm, was first identified in the pleura and was earlier named as haemangiopericytoma. The origin of SFT is not well established; however, it has recently been described as a mesenchymal neoplasm, probably arising from the ubiquitous dendritic interstitial cells. SFTs are predominantly benign tumours with a low risk of malignant transformation and are commonly seen in the intrathoracic region, seldom involving extrapleural locations. Although SFTs are extensively documented in medical literature, those originating in the mesentery are ex

A clinician's perspective on the new organ mesentery and non-vascular mesenteropathies.

Mesentery was discovered as a new organ in 2017. It is a continuous membranous tissue from the duodenojejunal flexure to the anorectal junction. It has distinct anatomy, physiology, and disease states. Primary mesenteropathies include vascular and non-vascular diseases. Some of them are common, and some of them are rarely seen in clinical practice. Secondary mesenteropathies occur when infection or malignancy in another organ spreads to the mesentery. Each entity has specific diagnostic and treatment protocols. Increased awareness of different mesenteropathies and an understanding of their var

A parasitic leiomyoma of the sigmoid mesentery with schwannoma-like image findings.

BACKGROUND: Parasitic leiomyoma (PL) consists of uterine fibroids separate from the uterus that grow in extrauterine tissues such as the peritoneum and mesenterium. The diagnosis of PL requires a thorough medical history of laparoscopic myomectomies using a morcellator and the identification of typical magnetic resonance imaging (MRI) findings as uterine fibroids. Imaging diagnosis of PL is occasionally difficult when PL degenerates in various ways, owing to atypical findings on computed tomography (CT) and MRI. CASE PRESENTATION: A 29-year-old woman with a history of laparoscopic myomectomy

Caecal volvulus in an adult with an incomplete common mesentery: A case report.

INTRODUCTION AND IMPORTANCE: A common mesentery is defined by the persistence of an embryonic anatomical arrangement secondary to an anomaly of rotation of the primary umbilical loop. Caecal volvulus is a rare cause of intestinal obstruction, which account for 1 to 1.5 % of all intestinal obstructions. A combination of both, intestinal mal rotation and caecal volvulus is rare. CASE PRESENTATION: We report this rare entity in a 50 year old male with no history of abdominal surgery who was admitted for an acute intestinal obstruction. Clinical examination found a non-complicated right inguinal

There is no fusion fascia in the abdomen and extraperitoneal fascia always surrounds the mesentery.

Toldt's fascia has always been described as a fusion fascia formed by two layers of visceral peritoneum when the mesentery attaches to the posterior abdominal wall. However, there is still no consensus about the mesentery and its surrounding fascia based on the current anatomic theories. This study aimed to determine the anatomical structures of the abdomen and provide a correct surgical plane for mesenteric-based surgery. Surgical videos of 121 patients who underwent laparoscopic operations of the digestive tract were reviewed to identify and compare the anatomical structures of the mesentery

Peritonectomy of the colonic mesentery. Case report of a new surgical technology.

INTRODUCTION AND IMPORTANCE: Malignant peritoneal mesothelioma is a local-regional disease process that requires a comprehensive treatment plan including complete cytoreductive surgery and regional chemotherapy. CASE PRESENTATION: Treatments used in our patient began with a complete cytoreductive surgery. This required visceral resections, parietal peritonectomy, peritonectomy of the small bowel and its mesentery, and a peritoneal resection of the colonic mesentery with sparing of the major vasculature of the large bowel. CLINICAL DISCUSSION: Peritoneal resection of the colonic mesentery and

Solitary fibrous tumor of the mesentery: a case report and review of the literature.

Solitary fibrous tumors are rare mesenchymal tumors that typically arise from the pleura and rarely originate from the mesentery. We herein report a case involving a 66-year-old patient who presented with a mass on the left abdomen. This mass had been incidentally noticed 10 years earlier. The patient sometimes experienced abdominal pain. Physical examination revealed an irregular mass, which was resected. A biopsy of the mass revealed that it was a solitary fibrous tumor originating from the mesentery of the small intestine. The patient was discharged 1 week after surgery and had an uneventfu

Castleman's disease mimicked melanoma metastasis in the mesentery - A case report.

INTRODUCTION: Castleman's disease (CD) is a rare and mainly asymptomatic cause of lymph node swelling. Often it is unicentric and located in the mediastinum. Due to rarity of the disease as well as a lack of symptoms, diagnosis proves to be challenging, especially when CD affects another region. PRESENTATION OF CASE: A 51-year old male underwent resection of a malignant melanoma. Further staging revealed an unclear abdominal mass located in the mesentery with close contact to small intestine. Under the assumption of metastasis, complete tumor removal including intestine resection and anastomo

Epithelial-to-mesenchymal transition-based morphogenesis of dorsal mesentery and gonad.

Dorsal mesentery and gonad (ovary and testis) are formed in distinct regions of the body and have different characteristics. Recent studies using chicken embryos showed that progenitors of these two organs are derived from the coelomic lining region, a ventral part of the medial lateral plate mesoderm (M-LPM). Furthermore, both types of progenitors develop in a similar manner, concomitant with morphological changes termed the epithelial-to-mesenchymal transition (EMT). EMT processes in both dorsal mesentery and gonad formation are regulated by BMP signaling. Interestingly, EMT-based morphogene

Navigating the mesentery: part II. Vascular abnormalities and a review of the literature.

AIM: Vascular abnormalities present advantages and/or disadvantages for the patient undergoing surgery. The aims of this study were to define, classify and demonstrate the courses, and to assess the clinical value, of arterial and venous abnormalities in the central mesentery. METHOD: We conducted a review of the anatomy of 340 patients planned for enrolment in the 'Safe Radical D3 Right Hemicolectomy for Cancer through Preoperative Biphasic MDCT Angiography' trial, 312 of whom were submitted to surgery. Vascular abnormalities were analysed in context with surgical notes and images. A meta-an

Navigating the Root of the Mesentery: A Guided Approach to an Artery-First Pancreatoduodenectomy.

In the standard technique of pancreatoduodenectomy (PD), the superior mesenteric artery (SMA) is approached following Kocher maneuver of the duodenum. Alternative approaches include SMA first with no touch approaches. The aim of this article is to report the preoperative planning for and operative techniques of a combined artery first with no touch technique (CTPD) for the performance of PD. The CTPD technique is described with a detailed discussion of the operative anatomy, and of the importance of preoperative mapping using computed tomography to aid dissection of the mesenteric root and ide

Serous cystadenocarcinoma of the mesentery in a man: case report and review of literature.

In February 2007, a 41-year-old Japanese male was admitted to our hospital with increasing upper abdominal pain. A contrast-enhanced computed tomography (CT) scan of the abdomen demonstrated a well-demarcated, hypodense cystic mass with a thickened wall in the mesocolon. The laboratory results were within normal limits, except for increased carcinoembryonic antigen, carbohydrate antigen 19-9, DUPAN-2 and SPAN-1. The patient was diagnosed as having a mesenteric malignant cyst, and during a laparotomy, a right hemicolectomy with mesenteric cystectomy was performed without rupture in March 2007.

Management of perforated essure with migration into small and large bowel mesentery.

The Essure permanent birth control system (Conceptus Inc, San Carlos, CA) is currently the only Food and Drug Administration-approved hysteroscopic sterilization method and has been widely accepted as a safe and effective procedure. We present a rare case of tubal perforation, coil fragmentation, and distal migration into small and large bowel mesentery 8 days after the insertion of the Essure device. We describe the successful management of this complication using laparoscopy and intraoperative fluoroscopy. Providers using Essure must be aware of the possibility of fragmentation of the Essure
