Imaging of infracolic and pelvic compartment by linear EUS.

The peritoneal cavity is subdivided into supracolic and infracolic compartments by transverse mesocolon, which attaches the colon to the posterior abdominal wall. Infracolic compartment is subdivided into right and left compartment by small bowel mesentery. Left infracolic space freely communicates with pelvic compartment. The infracolic compartment contains the coils of small bowel which is separated from paracolic gutter on either side by ascending and descending colon. Pelvic compartment mainly contains bladder, rectum and genital organ (prostate, seminal vesicle in male and uterus in femal

Revisiting Toldt Fascia Through Robotic Top-down and Medial-to-lateral Apporach - Video Vignette.

In this video (Toldt fascia.wmv), we demonstrated anatomic features of Toldt fascia, based on the meticulous dissection and high-resolution images inherent in robotic surgery. Toldt fascia is sandwiched by the overlying mesothelial layer of the mesocolon and underlying mesothelial layer of the retroperitoneum, comprised loose fibrous tissues with minute vessels inside, and is contiguous from the ileocecal mesentery radix to the upper rectum. The medial-to-lateral surgical dissection plane is readily developed within the Toldt fascia; however, any attempt to dissect along the interface between

Single-Port Laparoscopic Duodenojejunostomy Employing Semi-Kocherization for a Young Female with Superior Mesenteric Artery Syndrome.

Single-port laparoscopic duodenojejunostomy employing semi-Kocherization performed for a patient with superior mesenteric artery (SMA) syndrome is presented in this report. A 24-year-old woman missed meals due to work pressure, and her body weight decreased from 42 kg to 27 kg within 6 months. After this severe weight loss, she suffered from postprandial abdominal pain. An enhanced computed tomography revealed that the aortomesenteric angle was 11° (narrow), and the distance was short at 4.5 mm. Duodenography also revealed dilatation of the proximal duodenum. These findings led to a diagnosis

Abnormal length and position of the sigmoid colon and its clinical significance.

Sigmoid colon is one of the most variable parts of the large intestine. Here we report a very rare type of variation of the sigmoid colon. This variation was observed during the routine dissections for undergraduate medical students at Melaka Manipal Medical College (Manipal Campus) India in September 2012. In the current case, the sigmoid colon was about 60 cms long and made an inverted U shaped loop in front of the descending colon and the left kidney. It had a sigmoid mesocolon which covered the left kidney. The sigmoid colon had an ascending and a descending limb. This position of sigmoid

Assessment of AI-driven anatomical recognition in robotic gastrectomy: A multicenter retrospective analysis.

BACKGROUND: Precise recognition of anatomical structures such as the pancreas, transverse mesocolon, and right gastroepiploic vein (RGEV) is critical for the safety and effectiveness of infrapyloric lymph node dissection during robotic distal gastrectomy (RDG). Misrecognizing these structures can increase the risk of complications. However, even expert surgeons may encounter difficulties identifying these structures due to variations in patient anatomy and surgical technique. This study aims to validate the performance of an artificial intelligence (AI) model for automated anatomical recogniti

An unusual case of transverse mesocolic internal hernia with abnormality of both hands and high arched feet.

INTRODUCTION: An internal abdominal herniation is the protrusion of a viscus through a normal or abnormal mesenteric or peritoneal aperture. It is a rare cause of small bowel obstruction with a reported incidence of 0.2-0.9%. It can either be acquired through a trauma or surgical procedure or can be related to congenital peritoneal defects. Herniation through transverse mesocolon is very rare. PRESENTATION OF CASE: A case of acute intestinal obstruction due to internal herniation through a congenital rent in transverse mesocolon with rotation of gut approximately 180° around axis of the band.

Laparoscopic Repair of Transmesosigmoid Hernia Following Robot-Assisted Abdominoperineal Resection: A Case Report.

Sigmoid mesocolon hernia is a rare type of internal hernia that can rapidly progress to vascular compromise, necrosis, and intestinal perforation. Therefore, rapid diagnosis and surgical treatment are important. We present a case of transmesosigmoid hernia following robot-assisted abdominoperineal resection that was successfully treated using a laparoscopic approach. An 80-year-old woman underwent robot-assisted abdominoperineal resection for anal canal cancer. The postoperative course was uneventful. Two months after the surgery, the patient presented with upper abdominal pain. Computed tomog

[Restoration of the continuity of the colon with preservation of an existing colostomy].

A new technique for the restoration of the continuity of the colon with preservation of an existing colostomy is described here. A 75-year-old male with a three-year history of abdominal-perineal resection and a well functioning end colostomy was admitted to our department for a second primary cancer of the descending colon. At operation the colon was divided close to the abdominal wall; the colostomy was left in situ while the descending colon and mesocolon were radically resected. The viability of the colostomy was being continuously inspected. An EEA stapler was then inserted through the co

An unusual variant of a left paraduodenal hernia diagnosed and treated by laparoscopic surgery: report of a case.

An 80-year-old woman who had undergone both a cholecystectomy and an appendectomy presented with intermittent abdominal pain. Computed tomography (CT) revealed an encapsulated circumscribed cluster of jejunal loops in the left upper quadrant. The hernia orifice was adjacent to the left side of the superior mesenteric artery and vein. An upper gastrointestinal series also revealed a cluster of jejunal loops, suggesting the possibility of an internal hernia. Laparoscopic surgery was performed. The hernia orifice was found to be caused by abnormal adhesion between the transverse mesocolon and the

Laparoscopic resection of a giant retroperitoneal T-shaped duplication of descending colon.

The authors encountered an uncommon case of a retroperitoneal T-shaped duplication that presented with a communication with the descending colon, descending downward along the descending colon posterolaterally, passing through the sigmoid mesocolon and across the midline at the level of L5 to S1 vertebrae, ascending behind the ascending mesocolon, and terminating beneath the third portion of the duodenum. Using a 3-port transperitoneal laparoscopic technique, the duplication was completely resected. On reviewing the literature, T-shaped duplication has occurred in only 4 cases; and all arose f

A prospective radiological anatomical study of the variations of the position of the colon in the left pararenal space.

Percutaneous puncture of the kidney allows direct access to the pyelocalicial cavities. The posterior approach of this retroperitoneal organ can be complicated of transcolic punctures due to the postrenal position of the colon. A prospective radiological anatomical study of the relationship between the left kidney and the descending colon was undertaken. One hundred computed tomograms of adult subjects were obtained from which the anatomy of the left perirenal area was determined: the descending colon is more frequently behind the kidney in the young females. Two main factors determinants of t

Easy and secure closure of petersen's defect after laparoscopic distal gastrectomy with Roux-en-Y reconstruction.

BACKGROUND: Previously it has reported that the incidence of internal hernia can be decreased by closing Petersen's defect, but the perfect closure method, in fact, has not been discovered yet. In this study we have developed an easy and reliable method for closing Petersen's defect in the Roux-en-Y reconstruction after a laparoscopic distal gastrectomy. MATERIALS AND METHODS: We performed intracorporeal Roux-en-Y reconstruction after laparoscopic distal gastrectomy with antiperistaltic gastrojejunostomy. The greater omentum is placed on the cranial side of the transverse colon through the de

Feasibility and accuracy in performing complete mesocolic excision during laparoscopic left colectomy.

UNLABELLED: Laparoscopic colorectal surgery for cancer is nowadays performed in several referral centers and has been gaining increasing interest for treatment of colo-rectal cancer. After the introduction of complete mesorectal excision for rectal cancer, complete mesocolic excision has been advocated as an essential surgical step to improve oncologic results for patients with colon cancer. Complete mesocolic excision is a crucial step of hemicolectomy, and consists in the total removal of the mesocolon and its lymph nodes with high ligation of main mesenteric arteries and veins. In laparosco

Intraoperative measurement of colonic anatomy and attachments with relevance to colonoscopy.

This study examined the variations in colonic length and mesenteric attachments in 118 patients undergoing laparotomy. Measurements were taken according to a set protocol with the bowel pulled medially, or towards the pubic symphysis or the xiphisternum, mimicking the possible displacements that may occur during colonoscopy. A free sigmoid loop was not present in 20 patients (17 percent) because of adhesions. A descending mesocolon of 10 cm or more in length was recorded in ten patients (8 percent) and an ascending mesocolon 10 cm or greater in 11 (9 percent). Some 24 patients (20 percent) had
