Effect of foot orthoses on the biomechanics of stair ascent and descent in chronic metatarsalgia.

In individuals with musculoskeletal disorders such as chronic metatarsalgia, foot orthoses (FOs) are frequently prescribed to reduce plantar pressure, improve foot function and alleviate pain. During walking, medially wedged FOs (MWFO + MPs) with a metatarsal pad are more effective than standard FOs (SFOs) in reducing plantar pressure under the metatarsal heads. As stair ambulation involves higher physical demands than walking and modifies plantar loading, we questioned whether FOs remain effective during more challenging tasks. This study aimed to determine whether MWFO + MPs reduce peak plan

Impact of different foot orthoses on gait biomechanics in individuals with chronic metatarsalgia.

BACKGROUND: Foot orthoses (FOs) are commonly prescribed to reduce pain and improve function in individuals with musculoskeletal disorders, including those with chronic metatarsalgia (CM). Reducing the mechanical overload under the metatarsal heads during locomotion is the central point of the treatment for CM. Medially wedged FOs (MWFOs) with a metatarsal pad could further reduce pressure loading under the metatarsal heads and modify foot and ankle biomechanics compared to standard FOs (SFOs). RESEARCH QUESTION: Do MWFOs further decrease the peak plantar pressure under the metatarsal heads in

Efficacy of Ultrasound-Guided Pulsed Radiofrequency for Recalcitrant Metatarsalgia A Case Report.

Metatarsalgia is characterized by pain in the forefoot, which is associated with increased stress over the metatarsal head region. Despite the availability of a variety of conservative or surgical treatments for this condition, a few cases have demonstrated relapse or poor response to treatment. Pulsed radiofrequency (PRF) can provide pain relief in patients with diverse chronic conditions without causing neural injury. Recently, studies have shown that ultrasound-guided PRF may be beneficial for adhesive capsulitis, carpal tunnel syndrome, tarsal tunnel syndrome, and recalcitrant plantar fasc

Percutaneous dorsal closing wedge osteotomy of the metatarsal neck in management of metatarsalgia.

INTRODUCTION: Metatarsalgia can be caused by plantarflexion of a central metatarsal or discrepancies in the metatarsals' length. Nonsurgical management is usually sufficient to achieve satisfactory results. For those recalcitrant cases, metatarsal osteotomy is needed to relieve the pain. We describe a technique of percutaneous dorsal closing wedge osteotomy of the metatarsal to manage the recalcitrant metatarsalgia. A case series was reviewed retrospectively. MATERIALS AND METHODS: From March 2010 to March 2013, percutaneous dorsal closing wedge osteotomy of the metatarsal neck has been perfo

The influence of the length of the first metatarsal on transfer metatarsalgia after Wu's osteotomy.

BACKGROUND: The aim of this study was to evaluate how changes in the length of the first metatarsal, hallux valgus angle (HVA), intermetatarsal 1-2 angle (IMA), plantar angulation and sesamoid position influence the severity of the postoperative 2-5 metatarsalgia and to determine patient satisfaction with the cosmetic outcome after Wu's osteotomy. MATERIALS AND METHODS: A retrospective analysis of the clinical data and radiographs of 87 cases was performed at a mean followup time of 4.2 years after Wu's subcapital cross osteotomies. RESULTS: The mean HVA decreased from 42 to 13 degrees, the

Morton's metatarsalgia due to intermetatarsophalangeal bursitis as an early manifestation of rheumatoid arthritis.

Of 50 patients in whom Morton's metatarsalgia was diagnosed during an 18-year period, 12 (24%) had rheumatoid arthritis at the time of presentation. After conservative management had failed, 20 patients had subtotal excision of the intermetatarsophalangeal bursa and associated digital nerve; in two patients, only the nerve was excised. During the follow-up period ranging from two months to 15 years, an additional eight patients developed sero-positive rheumatoid arthritis. Thus, a total of 20 patients (40%) presenting with Morton's metatarsalgia had rheumatoid disease at initial presentation o

Association of Metatarsalgia After Hallux Valgus Correction With Relative First Metatarsal Length.

BACKGROUND: Metatarsalgia is frequently associated with hallux valgus. The aim of this study was to evaluate how the relative length and position of the first metatarsal head influenced metatarsalgia and plantar callosities beneath the lesser metatarsal heads. METHODS: A retrospective analysis of the clinical data and radiographs of 102 cases was performed at a mean follow-up of 16 months after biplane interlocking osteotomies. Clinical evaluation was made using the Japanese Society for Surgery of the Foot (JSSF) hallux scale. Radiologic evaluation was made with standard weight-bearing antero

Shortening effect influence of Distal Minimally Invasive Metatarsal Osteotomy in primary metatarsalgia.

PURPOSE: In primary metatarsalgia, Distal Minimally invasive Metatarsal Osteotomy (DMMO) achieves a correct load distribution which is a factor in pain relief, but contrary to the elevation of the metatarsal head, shortening the metatarsal length has no influence on plantar-loading parameters, while the increased metatarsal length is a factor in the development of metatarsalgia. Thus, we hypothesized that metatarsalgia could be partly related to a functional imbalance between bone structure and soft tissues and pain relief after DMMO results from soft tissue relaxation. METHODS: Many authors

Effect of toe exercises and toe grip strength on the treatment of primary metatarsalgia.

BACKGROUND: The relationship of metatarsalgia and toe function is poorly understood. We investigated the efficacy of toe exercises for the treatment of metatarsalgia. METHODS: Forty-one (56 feet) metatarsalgia patients (mean age ± SD: 63.4 ± 10.6) underwent toe strength measurement. We recorded pre- and post-treatment VAS score, AOFAS score, marble pickup, single-leg standing time (SLST), and compared in two subgroups to evaluate impact of disease duration on treatment outcome. RESULTS: Post treatment, toe plantarflexion strength improved (all p < 0.01); VAS scores decreased (p < 0.01); AOFA

Effect of First Ray Insufficiency and Metatarsal Index on Metatarsalgia in Hallux Valgus.

BACKGROUND: Two concepts have been proposed to explain the etiology of metatarsalgia in hallux valgus patients: First, as the magnitude of hallux valgus increases, there is a mechanical overload of the lesser metatarsals. Second, increased relative lesser metatarsal length is a factor in the development of metatarsalgia. However, there is no current evidence that these structural factors lead to primary metatarsalgia. The purpose of the study was to evaluate the factors associated with metatarsalgia in hallux valgus patients. METHODS: A cross-sectional study of 121 consecutive adult patients

Primary metatarsalgia: the influence of a custom moulded insole and a rockerbar on plantar pressure.

The effects of a custom moulded insole and a rockerbar on peak pressure and force impulse as well as on pain scores in subjects with a history of metatarsalgia were studied. In addition the subjects' preference for the type of intervention was determined. Forty-two subjects with a history of primary metatarsalgia were selected. They were all provided with the same brand of extra depth shoes with a ready made insole. The effect of custom moulded insoles, a rockerbar and the interaction between the two interventions were studied by testing the four possible combinations: ready made insole withou

Morton's metatarsalgia. Light and electron microscopic observations and their relation to entrapment neuropathies.

The dissected plantar nerves of 105 patients with clinical symptoms of Morton's disease were examined under light and electron microscopy. Of the 105 cases, 75 showed characteristic neuronal lesions; in the others different pathologic substrates responsible for the clinical symptoms could be demonstrated. The nervous lesions are characterized by: 1. Thickening of the walls of the endoneurial vessels produced by multiple layers of basement membranes. 2. Edema and sclerosis of the endoneurium. In the electron microscope the endoneurium was filled by deposits of fibrils with a tubular substructur

Outcome of Minimally Invasive Distal Metatarsal Metaphyseal Osteotomy (DMMO) for Lesser Toe Metatarsalgia.

BACKGROUND: As in all fields of surgery, advances in orthopaedic surgery develop toward less invasive surgical techniques. The advantages of smaller incisions include minimal soft tissue dissection allowing procedures to be performed as outpatient surgery. There is the assumption that this leads to a quicker recovery time permitting an earlier return to work. As with any new surgical technique, there is an associated learning curve. This study looked into the outcome of minimally invasive distal metatarsal metaphyseal osteotomy (DMMO) performed at a University Hospital. METHODS: Thirty patien
