Extracorporeal circulation increases nitric oxide-induced methemoglobinemia in vivo and in vitro.

OBJECTIVES: Methemoglobinemia is a well-known side effect of nitric oxide inhalation. We tested the hypothesis whether cardiopulmonary bypass increases methemoglobin formation by nitric oxide. DESIGN: A two-phase study: a) a controlled, prospective in vivo study of inhaled nitric oxide treatment followed by b) a second, prospective and controlled in vitro study. SETTING: Pediatric intensive care unit and research laboratory in a university hospital. PATIENTS: The in vivo study consisted of 25 patients following open-heart surgery and 19 children with acute respiratory distress syndrome (ARD

The Beef Jerky Blues: Methemoglobinemia From Home Cured Meat.

Methemoglobinemia can result from ingestion of nitrite- or nitrate-containing foods. Here, we report a case where an adolescent girl and her father developed clinically significant methemoglobinemia after ingestion of "homemade" beef jerky prepared with sodium nitrate salt purchased at a local grocery store. Both had palpitations, dyspnea, and visible mucosal cyanosis. The daughter had a methemoglobin level of 44.2% and the father's methemoglobin level was 34.2%. Prompt recognition of methemoglobinemia is important for initiating antidotal therapy with methylene blue.

Methemoglobinemia: pathogenesis, diagnosis, and management.

The diagnosis of methemoglobinemia should be considered in patients presenting with cyanosis and hypoxia. A variety of frequently used medications are capable of inducing methemoglobinemia, with dapsone and benzocaine being common culprits. Unique features, such as a saturation gap and chocolate-brown-colored blood, can raise suspicion for methemoglobinemia. Typically, symptoms correlate with the methemoglobin level, and treatment with methylene blue is reserved for patients with significantly elevated methemoglobin levels. In the presence of comorbid conditions that impair oxygen transport, h

Two cases of fatal methemoglobinemia caused by self-poisoning with sodium nitrite: A case report.

RATIONALE: Sodium nitrite intoxication reportedly causes severe methemoglobinemia. Recent studies reported that most clinically significant cases resulted from intentional exposure in suicidal attempts. We describe 2 cases of severe methemoglobinemia secondary to intentional sodium nitrite intoxication in suicidal attempts. PATIENTS CONCERNS: A 26-year-old man and 20-year-old woman attempted suicide by taking sodium nitrite, and were brought to the emergency department. DIAGNOSIS: The male patient collapsed at the scene. He ingested approximately 18 g of sodium nitrate, and his methemoglobin

Fatal Methemoglobinemia Due to Acute Inhalation of Methyl Nitrite in an Industrial Accident.

Methyl nitrite is suggested to cause methemoglobinemia by generating methemoglobin, which may be lethal when the methemoglobin concentration exceeds 70%. However, intoxication with methyl nitrite is seldom reported compared with that with other nitrites. Here, we present an industrial accident involving methyl nitrite inhalation during its synthesis process that resulted in three fatalities and one survivor. The autopsy revealed conspicuous blue-gray discoloration in various parts of the body, including the skin, airway mucosa, vessels, brain, heart, and among other areas. The toxicological te

Incidence of Methemoglobinemia in Cardiothoracic Surgery ICU Patients on Inhaled Nitric Oxide.

BACKGROUND: Inhaled nitric oxide (iNO) is a selective pulmonary vasodilator utilized for the treatment of right ventricular dysfunction in the cardiac surgery patient population, which carries a risk of developing methemoglobinemia. The actual frequency of methemoglobinemia is not well defined. OBJECTIVE: The purpose of this study was to evaluate the incidence of methemoglobinemia in a cardiothoracic surgery intensive care unit (ICU) patient population. METHODS: This was a single-center, retrospective cohort study of 208 cardiothoracic surgery ICU patients who received at least 24 hours of i

Methemoglobinemia induced by an over-the-counter medication.

OBJECTIVE: To report a case of methemoglobinemia induced by benzocaine and resorcinol (Vagisil) cream, an over-the-counter medication that was used to treat diaper rash in a neonate. CASE REPORT: A 6-day-old, 3350-g white boy was found to be cyanotic with blue mucous membranes on presentation. He had a diaper rash that was red and irritated, which his mother had treated with Vagisil cream. Methemoglobinemia secondary to absorption of benzocaine and resorcinol (1,3-benzenediol) from the Vagisil was diagnosed, with a methemoglobin level of 35% on admission (normal 0.4-1.5). Ingestion of Isobutyl Nitrite Leading to Methemoglobinemia: A Case Report.

This case report highlights an occurrence of methemoglobinemia caused by the ingestion of liquid amyl nitrite, a substance often misused as a recreational drug. The patient in this case presented with cyanosis and elevated methemoglobin levels. Prompt administration of methylene blue successfully reversed the methemoglobinemia by reducing methemoglobin back to hemoglobin. This case emphasizes the importance of rapid diagnosis and treatment in nitrite-induced methemoglobinemia, demonstrating methylene blue as a highly effective antidote. It also highlights the risks associated with amyl nitrite

Hb Chile [beta28(B10)Leu-->Met]: an unstable hemoglobin associated with chronic methemoglobinemia and sulfonamide or methylene blue-induced hemolytic anemia.

Among the causes of life-long cyanosis are congenital methemoglobinemia due to M hemoglobins, congenital methemoglobinemia due to methemoglobin reductase deficiency, a small number of low oxygen affinity hemoglobins, and a small number of unstable hemoglobins that spontaneously form methemoglobin in vivo at an accelerated rate. We report an unstable hemoglobin with these characteristics that was observed in a family of indigenous (native American) origin living near Santiago, Chile. This variant has the substitution beta28(B10)Leu-->Met, unambiguously corresponding to the DNA mutation of CTG--

Methemoglobinemia caused by a low dose of prilocaine during general anesthesia.

Methemoglobinemia is a blood disorder in which an abnormal amount of methemoglobin is produced, and prilocaine is one of the drugs that can cause this disorder. The maximum recommended dose of prilocaine is 8 mg/kg. We report a case of methemoglobinemia caused by the administration of 4.2 mg/kg of prilocaine without other methemoglobinemia-inducing drugs during general anesthesia. A 17-year-old girl with hyperthyroidism and anemia was scheduled to undergo maxillary sinus floor elevation and tooth extraction. The patient's peripheral oxygen saturation (SpO2) decreased from 100% at arrival to 95

Methemoglobinemia associated with massive acetaminophen ingestion: a case series.

Background: Acetaminophen is a common pharmaceutical ingestion reported to US poison centers. In overdose, toxic metabolites are known to cause hepato- and nephrotoxicity. While G6PD deficiency may be a risk factor for methemoglobin production in the setting of acetaminophen overdose, it is rarely reported in patients who do not have this condition. Methods: We present two cases of methemoglobinemia following massive acetaminophen ingestion with no known history of G6PD deficiency or other substances known to induce methemoglobinemia. The two cases had peak methemoglobin measurements of 32% and

Potential for an external vaginal antiitch cream containing benzocaine to cause methemoglobinemia in healthy women.

OBJECTIVES: Our purpose was to assess the potential for an external vaginal antiitch cream (20% benzocaine, 3% resorcinol) to significantly increase levels of methemoglobin above normal in healthy women. STUDY DESIGN: Fifty-five women reporting external vaginal itch were recruited for the study. Each patient was used as her own control with methemoglobin levels being measured before and after use of the cream. Women were instructed to apply a 1-inch strip of cream by fingertip to the external genital area three or four times a day for 7 consecutive days. RESULTS: There were no significant di

High-pressure proton nuclear magnetic resonance studies of hemoproteins. Pressure-induced structural change in heme environments of myoglobin, hemoglobin, and horseradish peroxidase.

Hyperfine shifted proton NMR spectra of metmyoglobin, methemoglobin, and their complexes with azide, imidazole, and cyanide as well as the spectrum of native horseradish peroxidase were obtained at high pressures up to 2000 atm with a specially designed high-pressure cell for 220-MHz superconducting NMR spectrometer. For the azide complexes of metmyoglobin, in all of which the iron atoms are in thermal spin equilibrium between high- and low-spin states, the increased pressure shifted their heme methyl proton signals to the upfield side. For the cyanide complexes of metmyoglobin and methemoglob

[Congenital methemoglobinemia].

This is a report on a case of inborn methemoglobinemia. The disease has been diagnosed in the first week of infant's life. At first it was assumed it was a case of inborn heart defect because of expressed generalized cyanosis, but very soon it was excluded. Values of methemoglobin were extremely high, so three days after birth hereditary methemoglobinemia was diagnosed. Our diagnosis was confirmed by disappearing of cyanosis after ascorbic acid was applied, by values of methemoglobin reductase of the infant and its father, anamnestic data revealing that mother had had temporary cyanosis during

Case Report of Remifentanil Labor Analgesia for a Pregnant Patient With Congenital Methemoglobinemia Type 1.

Congenital methemoglobinemia is a rare disease characterized by cyanosis and a left shifting of the oxyhemoglobin dissociation curve. The disease necessitates avoidance of certain medications commonly used in obstetrics, making labor analgesia and anesthesia challenging. We present a case report of peripartum anesthetic management of a pregnant patient with congenital methemoglobinemia type 1 who received remifentanil labor analgesia and continuous methemoglobin monitoring. Continuous real-time monitoring of methemoglobin concentrations may prove to be a useful monitor in future care settings.

Anesthetic experience: congenital methemoglobinemia due to hemoglobin M.

Methemoglobinemia is rare. It is classified into two types: congenital methemoglobinemia and acquired methemoglobinemia. Methemoglobin is incapable of binding oxygen, leading to complications such as cyanosis, dyspnea, headache, and heart failure. In the present case, a 35-year-old man with congenital methemoglobinemia underwent general anesthesia for thyroidectomy. The patient was diagnosed with hemoglobin M at 7 years of age. Ventilation was performed with FiO2 1.0. Arterial blood gas analysis showed that the pH was 7.4, PaO2 439 mmHg, PaCO2 40.5 mmHg, oxyhemoglobin level of 83.2%, and methe

Two cases of methemoglobinemia following zopiclone ingestion.

INTRODUCTION: Most cases of methemoglobinemia result from exposure to certain medications and chemicals such as nitrates, nitrites, aniline, dapsone, phenazopyridine, benzocaine, and chlorates which oxidize the iron from the ferrous state. Intoxication with zopiclone is expected to produce drowsiness, confusion and coma but not methemoglobinemia. We report two cases of zopiclone overdose with methemoglobinemia. CASE REPORTS: Case one: A 43-year-old woman presented to the emergency department two hours after ingesting 100 tablets of 7.5 mg zopiclone. Her initial vital signs, physical examinati

A Case of Suspected Cocaine Adulterant-Associated Methemoglobinemia in the United States.

A 30-year-old male with a past history of polysubstance use presents to a drug rehabilitation facility after cocaine and heroin use just prior to arrival. While drinking at a water fountain at the facility, he became unresponsive. He was discovered to have an oxygen saturation of 50% on room air with an improvement to 87% on non-rebreather masks. Arterial blood gas revealed a methemoglobin level of 45.8%. The patient was given methylene blue with a repeat methemoglobin level of 0.00% within six hours. We attribute this presentation to local anesthetic-adulterated cocaine, a well-documented cau

A Case Report of Cyanosis With Refractory Hypoxemia: Is It Methemoglobinemia?

Dapsone is used in the treatment of a variety of dermatological conditions and prophylaxis of opportunistic infections. However, if consumed at a dose of more than 200 mg/day, it can cause methemoglobinemia, a condition characterized by elevated methemoglobin levels in the blood; methemoglobin is an abnormal form of hemoglobin, containing iron in the ferric state (Fe3 +) rather than the reduced ferrous form (Fe2 +) found in hemoglobin. A small amount of it is produced in the body due to oxidant damage to the red blood cells. Methemoglobinemia can cause varied clinical manifestations involving

Methemoglobinemia Hemotoxicity of Some Antimalarial 8-Aminoquinoline Analogues and Their Hydroxylated Derivatives: Density Functional Theory Computation of Ionization Potentials.

The administration of primaquine (PQ), an essential drug for the treatment and radical cure of malaria, can lead to methemoglobin formation and life-threatening hemolysis for glucose-6-phosphate dehydrogenase deficient patients. The ionization potential (IP, a quantitative measure of the ability to lose an electron) of the metabolites generated by antimalarial 8-aminoquinoline (8-AQ) drugs like PQ has been believed to be correlated in part to this methemoglobinemia hemotoxicity: the lower the IP of an 8-AQ derivative, the higher the concentration of methemoglobin generated.
