Mental Health Outcomes of Premature Discharge from United States Air Force Basic Military Training.

INTRODUCTION: Each year, a substantial proportion of trainees who enter the U.S. Air Force (USAF) Basic Military Training (BMT) are unable to complete training. Reentry to civilian life poses significant challenges, including finding employment, establishing a new career path, and paying for education. To our knowledge, no study has examined the association of discharge from USAF BMT and mental health symptoms. Utilizing a sample of 85 individuals discharged from USAF BMT for reasons other than mental health concerns, the present study aimed to: (1) characterize the severity of current depress

Is cohesion within military units associated with post-deployment behavioral and mental health outcomes?

PURPOSE: Prior studies suggest that cohesion among members of military units has a positive impact on behavioral and mental health sequelae of combat deployment. However, these studies have not distinguished variation in cohesion across units from variation in perception of cohesion across individuals within units. METHODS: A sample of U.S. Marines was assessed before and after deployment to Iraq or Afghanistan in 2010 or 2011. Within-group centering was used to distinguish unit-level from individual-level associations of cohesion with four behavioral and mental health outcomes assessed after

Sexual harassment and assault experienced by reservists during military service: prevalence and health correlates.

The current investigation identified the gender-specific prevalence of sexual harassment and assault experienced during U.S. military service and the negative mental and physical health correlates of these experiences in a sample of former reservists. We surveyed a stratified random sample of 3,946 former reservists about their experiences during military service and their current health, including depression, posttraumatic stress disorder, somatic symptoms, and medical conditions. Prevalence estimates and confidence intervals of sexual harassment and assault were calculated. A series of logis

Mental health evaluations of U.S. Air Force basic military training and technical training students.

A 2000 report by Staal, Cigrang, and Fiedler and a 1998 report by Cigrang, Carbone, Todd, and Fiedler described the attrition of U. S. Air Force basic military trainees due to mental health disorders for the year 1997. This article looks at the population of Air Force basic military trainees and technical training school students located at the same base during the year 2001. In addition, we look at the effect of allowing basic trainees and those in technical school to refer themselves for mental health evaluations as opposed to only evaluating those referred by secondary sources. Health assessment of U.S. military personnel deployed to Bosnia-Herzegovina for operation joint endeavor.

In anticipation of U.S. forces deploying to Bosnia-Herzegovina, plans were established to conduct medical surveillance of all military service members. This surveillance would provide the Department of Defense with an overview of the hospitalization and outpatient morbidity experience of U.S. forces. Standardized collection of medical data from all U.S. camps using 14 diagnostic categories based on International Classification of Diseases, 9th Revision, codes began in March 1996. Special assessments for hantavirus and tick-borne encephalitis (TBE) infection risk were also conducted. The averag

Treatment of PTSD in the military health system: Which patients receive evidence based psychotherapy?

The goal of this study was to identify patient-level factors associated with receiving evidence-based psychotherapy (EBP) for posttraumatic stress disorder (PTSD) in the military health system (MHS). We used machine learning models (LASSO, RFE, Elastic Net) to examine predictors of initiation (at least one session) and minimally adequate dosage (three sessions in 90 days or eight sessions in 14 weeks) of EBP defined as prolonged exposure, cognitive processing therapy, or eye movement desensitization and reprocessing therapy. Participants (N = 3,459) were diagnosed with PTSD and receiving behav

Burden, Epidemiology, and Clinical Characteristics of Leptospirosis within the United States Military Health System.

Leptospirosis is a global zoonosis with varying severity that remains underreported due to diagnostic limitations. We analyzed the burden, epidemiology, and clinical characteristics of leptospirosis diagnoses in the Military Health System, which includes care to more than 1.3 million active-duty service members and 6.5 million retirees and dependents worldwide. Potential diagnoses were identified by leptospirosis-specific International Classification of Diseases (ICD)-9 and ICD-10 codes or laboratory tests between January 1, 2013 and December 31, 2021. Upon chart review they were classified pe

Disordered eating among military adolescents presenting for annual health visits at a naval clinic.

INTRODUCTION: The purpose of this study was to estimate disordered eating behavior (DEB) prevalence in military dependent adolescents, compare it to previous reports in the general adolescent population, and investigate whether obesity, sex and gender, or key military-specific demographics (parent in the military, parental active duty [AD] status, military branch, number of lifetime moves, number of parental deployments) are associated with higher DEB risk. METHODS: Retrospective cross-sectional study of military dependents aged 11-19 years seen in an adolescent clinic at a naval medical cent

Valor and vulnerability: Honor endorsement and mental health stigma across military and civilian contexts.

This study investigates the intersection of honor culture and mental health stigma across military and civilian contexts. We examined the impact of honor endorsement on public and self-stigma surrounding mental health care (MHC) in samples of both military service members/veterans and civilians. Results reveal differences in how military and civilian participants conceptualize and endorse honor, as well as how honor related to MHC stigma and intentions to seek MHC. The implications of these findings for both researchers and clinicians are discussed.

Well-being domains in U.S. military veterans: identifying modifiable factors to promote whole health.

The U.S. Department of Veterans Affairs is actively transitioning away from a disease-centric model of healthcare to one that prioritizes disease prevention and the promotion of overall health and well-being. Described as Whole Health, this initiative aims to provide personalized, values-centered care that optimizes physical, behavioral, spiritual, and socioeconomic well-being. To inform this initiative, we analyzed cross-sectional data from a nationally representative sample of primarily older U.S. military veterans to estimate levels of well-being across these domains, and identify sociodemo

Micronutrient Status During Military Training and Associations With Musculoskeletal Health, Injury, and Readiness Outcomes.

OBJECTIVE: Micronutrient status, specifically vitamin D and iron, represent modifiable factors for optimizing military readiness. The primary purpose of this investigation was to determine associations between micronutrient deficiency (i.e., iron status and 25-hydroxy-vitamin D [25(OH)D]) and operationally relevant outcomes (i.e., skeletal health, musculoskeletal injury) at baseline and post-10 weeks of arduous military training. METHODS: A total of 227 (177 men, 50 women) Marine Officer Candidates School (OCS) candidates who completed OCS training with complete data sets were included in thi

Sex Differences in Postinjury Health Profiles Among U.S. Military Personnel Following Deployment-Related Concussion.

Background: Women in the U.S. military are now authorized to serve in direct combat occupations. This may increase their risk of combat injuries, such as concussion, in future conflicts. Knowledge of sex differences in health profiles after concussion is paramount for military medical planning efforts. The purpose of this study was to assess sex-related differences in health profiles among U.S. military personnel following deployment-related concussion. Materials and Methods: We conducted a retrospective study of service members who sustained a concussion during combat deployment between 2004

Trauma, resilience and significant relationships: Sex differences in protective factors for military mental health.

BACKGROUND: Military service is historically a male-dominated occupation, as such, the majority of research examining the development of mental disorder in Australian Defence Force members has had primarily male samples. While there have been mixed findings internationally regarding sex differences in rates of mental disorder and subthreshold symptoms among military personnel, across studies, the evidence tends to suggest that female military members are at least as likely as males to experience subthreshold mental health symptoms and have similar or higher rates of posttraumatic stress disord

Toxicant Exposures and Health Symptoms in Military Pesticide Applicators From the 1991 Gulf War.

OBJECTIVE: The chronic impact of acetylcholinesterase inhibitors and other toxicants on Gulf War (GW) veterans' health symptoms is unclear. METHODS: Building on reports of adverse neuropsychological outcomes in GW pesticide applicators exposed to pesticides and pyridostigmine bromide, we now report on health symptoms in this group. RESULTS: In adjusted analyses, applicators with high exposures/impact to pesticides reported significantly more symptoms (18/34 symptoms) than applicators with lower exposures/impact and were more likely to meet modified Kansas and CDC Gulf War Illness criteria. Characteristics of mpox cases diagnosed in Military Health System beneficiaries, May 2022-April 2024.

In May 2022, cases of mpox emerged beyond its historically endemic parts of Central and West Africa, primarily affecting men who have sex with men (MSM), and spreading via sexual networks. During this global outbreak the novel clinical and epidemiological characteristics of mpox disease were thoroughly documented in civilian populations but comparable data have not been reported for Military Health System (MHS) populations including beneficiaries. MHS cases were identified through a variety of data sources, including the Disease Reporting System internet (DRSi) and a customized query of ICD-10
