Minimally invasive endocrine surgery: laparoscopic resection of insulinomas.

Pancreatic insulinomas are mostly benign and solitary tumors. Successful management of patients with insulinoma relies on accurate localization of the tumors and the use of appropriate surgical techniques. However, preoperative radiological imaging studies often fail to localize the insulinomas because of the small tumor sizes. Conventional intraoperative ultrasonography combined with palpation has been widely used as the best localization tool. Since contact ultrasonography, a new technique for localizing pancreatic lesions, became available as a laparoscopic study, several surgeons have util

Minimally invasive surgery. Bedside tracheostomy and gastrostomy.

Minimally invasive surgical techniques have gathered tremendous momentum. Most patient benefit is realized in the ambulatory setting. Smaller incisions result in less pain and earlier return to activities. Critically ill patients typically do not benefit from minimally invasive techniques in this manner; however, they do benefit from other aspects of minimally invasive tracheostomy and gastrostomy. Small tracheostomy wounds are associated with reduced wound problems (infection and breakdown). The small stab wounds of minimally invasive gastrostomy are associated with less pain and with an abse

[Robot-assisted surgery in the head and neck region].

Robot-assisted surgery has been established in the head and neck region as a promising minimally invasive method, particularly for oropharyngeal tumors. It enables precise tissue-preserving procedures with good oncological control and functional preservation. Modern systems offer multi- and single-port technologies, each providing distinct advantages depending on anatomy and indication. Transoral approaches allow scar-free resection with shortened recovery times. Despite growing evidence, robot-assisted surgery remains cost-intensive and technically demanding. Training requirements, case numbe

[The GeRMIQ-Curriculum: a Blueprint for a National Training Concept in Minimally Invasive and Robotic Surgery].

PURPOSE: Minimally invasive (MIS) and robot-assisted surgery (RAS) have revolutionised surgical practice and place high demands on knowledge and technical skills as well as structured training concepts. Currently, there is no comprehensive curriculum in Germany, which leads to insufficient quality of training, with corresponding consequences. MATERIALS AND METHODS: The GeRMIQ curriculum (German Robotic and Minimally Invasive Surgery Qualification) was developed to close this gap and create a national, standardised, and forward-looking program for basic surgical training. RESULTS: The12-month

[Advances in minimally invasive surgical techniques for esophageal cancer].

Over the past two decades, minimally invasive esophageal surgery has evolved from initial technical exploration into an era of diversification and precision. This article systematically reviews the current mainstream minimally invasive esophagectomy techniques and their evolutionary logic. Currently, minimally invasive McKeown esophagectomy, with its advantage of thorough lymph node dissection, remains the most widely used standard procedure, with multiple RCTs confirming its non-inferior long-term survival compared to open surgery. The non-transthoracic approach, represented by inflatable med

Robotic and endoscopic minimally invasive breast surgery: a narrative synthesis on divergent global adoption and emerging trends.

Minimally invasive breast surgery (MIBS), encompassing robotic-assisted and endoscopic approaches, has gained increasing attention as an alternative to conventional breast surgery, aiming to optimize cosmetic outcomes while maintaining oncologic safety. To date systematic reviews and meta-analyses have evaluated perioperative and oncologic outcomes, but with minimal understanding and emphasis on how these techniques are adopted globally and how health-system factors influence their dissemination. This narrative review synthesizes evidence from published clinical series, comparative studies, sy

[Postoperative Pneumothorax Induced by Temporary Epicardial Pacing Wire Placed in Minimally Invasive Cardiac Surgery:Report of a Case].

We report a case of pneumothorax induced by the temporary epicardial pacing wire placement during minimally invasive atrium septum defect closure via right mini-thoracotomy. Temporary epicardial pacing wires are commonly employed in cardiac surgery with complications being rare. In this case, the wire sutured on the right surface of the right atrium directly contacted the right lung, resulting in pneumothorax, as the pericardium, harvested for atrium septum defect closure, was not sutured. Since conservative treatment was not effective, the patient underwent video assisted lung repair surgery.

Outpatient Versus Inpatient Minimally Invasive Adrenalectomy: A Systematic Review and Meta-Analysis.

OBJECTIVE: Advances in surgical care have allowed the implementation of outpatient protocols across many traditionally inpatient procedures, including adrenal surgery. However, there is an ongoing debate regarding the safety of outpatient minimally invasive adrenalectomy. This study aims to perform a systematic review and meta-analysis comparing outpatient to inpatient minimally invasive adrenalectomy. METHODS: A systematic search for studies comparing outpatient versus inpatient minimally invasive adrenalectomy in September 2023 on Cochrane, EMBASE, Web of Science, and PubMed was performed.

Robot-Assisted Minimally Invasive Esophagectomy: Current Best Practice.

BACKGROUND: Esophagectomy, the cornerstone in the multimodal treatment of esophageal cancer, has evolved from open surgery to minimally invasive esophagectomy (MIE) in recent decades. MIE reduces complications, facilitates faster recovery, and provides comparable or superior oncologic outcomes and survival rates compared to open surgery. SUMMARY: Since the early 2000s, robot-assisted minimally invasive esophagectomy (RAMIE) has emerged, offering enhanced precision over MIE through features such as three-dimensional visualization, improved instrument dexterity, tremor filtration, and motion sc

Robotic-assisted surgery in high-risk surgical patients with endometrial cancer.

Many patients diagnosed with an endometrial cancer are at high-risk for surgery due to factors such as advanced age, raised body mass index or frailty. Minimally-invasive surgery, in particular robotic-assisted, is increasingly used in the surgical management of endometrial cancer however, there are a lack of clinical trials investigating outcomes in high-risk patient populations. This article will review the current evidence and identify areas of uncertainty where future research is needed.

Robotic Ear Surgery.

Robots under research and development for otology can be classified as collaborative (intervention is constrained by the robot but surgeon directly actuates the end-effector), teleoperated (surgeon remotely controls a tool with modification [eg, tremor reduction] by the robot), or autonomous (surgeon monitors the robot performing a task). Current clinical trials focus on more accurate stapes surgery, minimally invasive access to the cochlea, and less traumatic insertion of cochlear implant electrode arrays. Autonomous approaches to major aspects of surgical interventions (eg, mastoidectomy) wi

Xanthogranulomatous pyelonephritis: a comparison of open and minimally-invasive surgical approaches.

To compare perioperative outcomes between patients undergoing minimally-invasive (MIS) and open surgical approaches for the treatment of Xanthogranulomatous Pyelonephritis (XGP). Between 2007 and 2017 we retrospectively identified 40 patients undergoing nephrectomy at our institution for pathologically confirmed XGP. Patients whose operations were ultimately completed with open technique were analyzed with the open cohort, whereas patients whose operations were completed in entirety using any laparoscopic approach were analyzed with the MIS group. Twenty-three patients were analyzed in the ope

Texture differentiation using audio signal analysis with robotic interventional instruments.

Robotic minimally invasive surgery (RMIS) has played an important role in the last decades. In traditional surgery, surgeons rely on palpation using their hands. However, during RMIS, surgeons use the visual-haptics technique to compensate the missing sense of touch. Various sensors have been widely used to retrieve this natural sense, but there are still issues like integration, costs, sterilization and the small sensing area that prevent such approaches from being applied. A new method based on acoustic emission has been recently proposed for acquiring audio information from tool-tissue inte

Use of minimally invasive spine surgical instruments for the treatment of bone tumors.

Orthopedic oncologists often encounter patients with minor bony lesions that are difficult to access surgically and therefore require large exposures out of proportion to the severity of disease that confer significant patient morbidity. Minimally invasive surgical techniques offer the advantage of smaller incisions, shorter operative times, decreased tissue damage, and decreased costs. A variety of surgical procedures have emerged using minimally invasive technologies, particularly in the field of spine surgery. Areas covered: In this article, we describe the Minimal Exposure Tubular Retracto

[MINIMALLY INVASIVE CARDIAC SURGERY IN JAPAN: INTRODUCTION].

Minimally invasive cardiac surgery (MICS), first introduced in the 1990s, was originally performed via partial sternotomy, the subxyphoid approach, and right anterolateral thoracotomy. Over the past 20 years, MICS procedures have progressed from mere alternatives to standard full sternotomy to endoscope-assisted and then to totally endoscopic open-heart procedures. MICS has gained popularity among surgeons and patients; without sacrificing the safety level and durability of cardiac procedures equivalent to median sternotomy, refinement of MICS technologies has resulted in a decrease in length

[Current operative techniques and strategies in endocrine surgery].

Technical advances and focusing on subsets modified endocrine surgery in the last ten years tremendously. There is on one side a clear trend towards minimal invasive approaches, first of all in the surgery of the adrenal glands, where the transperitoneal or retroperitoneal laparoscopic adrenalectomy has become the gold standard for tumors up to a size of 10 cm in diameter. But also in pancreatic endocrine surgery for small tumors localized in the pancreas tail and up to a certain extend in thyroid and parathyroid surgery, laparoscopic or video assisted techniques are used. On the other side mo

Minimally invasive treatment of fecal incontinence and constipation in children.

The capacity for voluntary bowel movements and thus fecal incontinence may be limited in children born with anorectal anomalies and Hirschsprung's disease, as well as for many patients with spinal problems. Some of these patients have severe constipation which if not properly managed causes overflow pseudoincontinence and some have true fecal incontinence. For patients with anorectal malformations and those with idiopathic constipation with overflow pseudoincontinence, disimpaction, followed by an aggressive laxative regimen, often makes them continent, i.e. capable of having voluntary bowel m

Repair of secundum atrial septal defect: limiting the incision without sacrificing exposure.

A simple and effective technique for repair of secundum atrial septal defect is described. The heart is exposed through a limited midline skin incision and partial sternotomy, and the atrial septal defect is closed through a right atriotomy with ascending aortic and dual venous cannulation. This approach achieves a cosmetically superior result with standard instrumentation and cardiopulmonary bypass techniques, without compromising exposure or using peripheral incisions.
