Nurse-Led Mobility Program: Driving a Culture of Early Mobilization in Medical-Surgical Nursing.

BACKGROUND: Hospitalized medical-surgical patients are at risk for adverse health outcomes due to immobility. Despite well-documented consequences, low mobility is prevalent. LOCAL PROBLEM: In a 547-bed hospital, medical-surgical patients were mobilized less frequently than expected. Physical therapists were inappropriately consulted 22% of the time for routine mobility of patients. A preimplementation survey of registered nurses indicated a lack of knowledge and confidence to safely mobilize patients. METHODS: This quality improvement project implemented a nurse-led mobility program in an e

Selective primary health care: an interim strategy for disease control in developing countries.

Priorities among the infectious diseases affecting the three billion people in the less developed world have been based on prevalence, morbidity, mortality and feasibility of control. With these priorities in mind a program of selective primary health care is compared with other approaches and suggested as the most cost-effective form of medical intervention in the least developed countries. A flexible program delivered by either fixed or mobile units might include measles and diphtheria-pertussis-tetanus vaccination, treatment for febrile malaria and oral rehydration for diarrhea in children,

Residential Relocation of Community-Dwelling Older Adults: The Role of Physical Function and the Housing Environment.

ObjectiveMost older adults in the United States (U.S.) prefer to remain in their current housing as they age, but difficulty with mobility (e.g., walking) may make this more challenging. We estimated the association between difficulty with mobility (mobility) and residential relocation in the following year.MethodsWe included adults aged ≥65 participating in ≥2 rounds (years 2011-2019) of the National Health and Aging Trends Study. Mobility was self-reported and change in permanent address was captured by study staff. We used a prospective cohort design and logistic regression with a clusterin

Implementing the Age-Friendly 4Ms+ Framework in a Vulnerable Population at a Safety-Net Hospital.

On account of the growing proportion of older adults in the United States, thousands of health systems have implemented the Age-Friendly Health Systems' (AFHS) 4Ms framework - What Matters, Medication, Mentation, Mobility - to optimize older adult care. Safety-net hospitals serve a vulnerable older adult population characterized by adverse social determinants of health and often operate with limited financial resources. The objective of this article is to describe the process of implementing the 4Ms framework, plus two additional Ms - Malnutrition and Mal-Care Transitions - into the inpatient

Does a Mobile Phone Depression-Screening App Motivate Mobile Phone Users With High Depressive Symptoms to Seek a Health Care Professional's Help?

BACKGROUND: The objective of disease screening is to encourage high-risk subjects to seek health care diagnosis and treatment. Mobile phone apps can effectively screen mental health conditions, including depression. However, it is not known how effective such screening methods are in motivating users to discuss the obtained results of such apps with health care professionals. Does a mobile phone depression-screening app motivate users with high depressive symptoms to seek health care professional advice? This study aimed to address this question. METHOD: This was a single-cohort, prospective,

Surgical and anesthetic activity in a french emergency field hospital responding to devastating floods.

PURPOSE: Floods are the most frequent natural disasters, causing widespread mortality and devastation. In 2023, Storm Daniel triggered catastrophic flooding in Derna, Libya, leading to over 11,300 deaths and leaving more than 10,000 people missing. In response, the French Emergency Medical Team (EMT-FRA) ESCRIM, deployed via the European Union Civil Protection Mechanism, arrived to provide critical medical aid. METHODS: The team set up a field hospital equipped with adult and pediatric care, ICU, radiology, and laboratory services, adhering to strict aseptic conditions. Data collection was per

[Softly-softly].

The missions of the mobile psychiatric team in Lyon involve reaching out to vulnerable people to guide them towards the most appropriate care. Establishing a bond and finding the right words to engage with them constitute a balancing act which the caregiver must undertake with the homeless. It is important to try and understand the situation, the person's life story, invent alternatives, while refraining from pushing them too much, in order to maintain their trust.

Establishing Ebola Virus Disease (EVD) diagnostics using GeneXpert technology at a mobile laboratory in Liberia: Impact on outbreak response, case management and laboratory systems strengthening.

The 2014-16 Ebola Virus Disease (EVD) outbreak in West Africa highlighted the necessity for readily available, accurate and rapid diagnostics. The magnitude of the outbreak and the re-emergence of clusters of EVD cases following the declaration of interrupted transmission in Liberia, reinforced the need for sustained diagnostics to support surveillance and emergency preparedness. We describe implementation of the Xpert Ebola Assay, a rapid molecular diagnostic test run on the GeneXpert platform, at a mobile laboratory in Liberia and the subsequent impact on EVD outbreak response, case manageme

Development and preliminary clinical validation of a mobile health application for pressure injury staging in ICU patients.

BACKGROUND: Pressure injury is a common adverse event among critically ill patients and is associated with poor clinical outcomes, increased nursing workload, and substantial healthcare costs. Accurate staging of pressure injuries is essential for guiding nursing interventions; however, conventional visual assessment is highly dependent on individual experience and often shows limited consistency. METHODS: A prospective cross-sectional study was conducted in the intensive care units (ICUs) of tertiary hospitals. A smartphone-based application integrating wound image acquisition and intelligen

An Innovative Model of Stroke Care for Rapid Assessment and Discharge of Patients With Transient Ischemic Attack and Stroke in Northeastern Ontario: Protocol for the Implementation and Evaluation of MOTIVE (Mobile Transient Ischemic Attack and Stroke With Adaptive Workflow) Team.

BACKGROUND: Patients presenting to the emergency department (ED) with transient ischemic attack (TIA) or stroke, as well as admitted patients who develop stroke symptoms in acute nonstroke units, are commonly transferred to stroke units, where trained interdisciplinary teams provide comprehensive assessments and discharge planning. However, the lack of integrated interdisciplinary stroke assessments in the ED and acute nonstroke unit care settings has contributed to inefficient patient flow and capacity pressure, prolonged hospital length of stay (LOS), and delayed discharge. Prior models have

Implementation of mobile medical units for naloxone distribution and MOUD linkage in Ohio's HEALing Communities Study: a community case study.

UNLABELLED: Naloxone, an opioid overdose reversal drug, and medication for opioid use disorder have been found to effectively combat opioid-related overdose and death. However, cost and access remain considerable barriers for many individuals at the highest risk of opioid-related overdoses. Similarly, many struggle to access medications for opioid use disorder because of the limited availability of treatment facilities and inadequate transportation options. Within the broader HEALing Communities Study, some Ohio communities elected to use mobile medical units to support delivery of naloxone an

The framework and best practices of emergency medical teams: lessons learned from Israel's international field hospital response.

In 2010, a devastating earthquake struck Haiti, killing over 200,000 and injuring over 250,000. Dozens of countries sent medical teams, with Israel’s being one of the first to arrive. These teams saved lives and salvaged limbs. However, other responses were not as professional, resulting in criticism in the medical community of “Disaster Tourism.” For example, certain physicians attempted to perform procedures or surgeries that they were not certified to perform. There was a call to develop a framework for disaster response. The World Health Organization spearheaded this project by initiating

Fear of progression in patients with mild or common type COVID-19.

AIM: To investigate the current condition and degree of fear of disease progression and associated factors in patients with mild or common type COVID-19. BACKGROUND: At the end of 2019, COVID-19 spread from Wuhan in Hubei Province throughout China. Confirmed cases and deaths have since been reported in many countries around the world. However, fear of progression in these patients has been poorly explored. METHODS: During February 2020, we recruited 114 patients with mild or common type COVID-19 admitted to a Fangcang shelter hospital. We assessed patients' degree of fear using the simplifie

Taking therapeutic apheresis services to patients in South Africa: An eight year review of SANBS mobile therapeutic apheresis service, 2013-2020.

INTRODUCTION: A 20 year review of health and health care presents the multiple challenges faced by South Africans. Health and poverty is highlighted with 45% of population living on approximately US$ 2 per day and 10 million living on less than US$ 1 per day. Widening disparities in health care provision between public and private sector hospital services exist. The South African population includes the largest number of people living with HIV infection/AIDS of any country in the world, with a 70% estimate of 7.5 million people living with HIV on antiretroviral therapy. The South African Natio

Composition or Context: Using Transportability to Understand Drivers of Site Differences in a Large-scale Housing Experiment.

BACKGROUND: The Moving To Opportunity (MTO) experiment manipulated neighborhood context by randomly assigning housing vouchers to volunteers living in public housing to use to move to lower poverty neighborhoods in five US cities. This random assignment overcomes confounding limitations that challenge other neighborhood studies. However, differences in MTO's effects across the five cities have been largely ignored. Such differences could be due to population composition (e.g., differences in the racial/ethnic distribution) or to context (e.g., differences in the economy). METHODS: Using a non
