Tuberculosis in Cape Town: An age-structured transmission model.

BACKGROUND: Tuberculosis (TB) is the leading cause of death in South Africa. The burden of disease varies by age, with peaks in TB notification rates in the HIV-negative population at ages 0-5, 20-24, and 45-49 years. There is little variation between age groups in the rates in the HIV-positive population. The drivers of this age pattern remain unknown. METHODS: We developed an age-structured simulation model of Mycobacterium tuberculosis (Mtb) transmission in Cape Town, South Africa. We considered five states of TB progression: susceptible, infected (latent TB), active TB, treated TB, and tr

Finite element modeling of multilayered structures of fish scales.

The interlinked fish scales of Atractosteus spatula (alligator gar) and Polypterus senegalus (gray and albino bichir) are effective multilayered armor systems for protecting fish from threats such as aggressive conspecific interactions or predation. Both types of fish scales have multi-layered structures with a harder and stiffer outer layer, and softer and more compliant inner layers. However, there are differences in relative layer thickness, property mismatch between layers, the property gradations and nanostructures in each layer. The fracture paths and patterns of both scales under microi

A structural model of perfusion and oxygenation in low-flow states.

BACKGROUND: Recent investigations underscore the critical importance of ventilation strategies on resuscitation outcomes. In low perfusion states, such as cardiac arrest and traumatic shock, the rise in intrathoracic pressure that accompanies positive-pressure ventilation can significantly impede venous return and lead to a decrease in cardiac output. The optimal ventilation strategy in these "low-flow" states remains unclear. OBJECTIVE: To create a mathematical model of perfusion and oxygenation to predict the effects of PPV with both normotension and hypotension. METHODS: The lung pressure

A structured threshold model for mountain pine beetle outbreak.

A vigor-structured model for mountain pine beetle outbreak dynamics within a forest stand is proposed and analyzed. This model explicitly tracks the changing vigor structure in the stand. All model parameters, other than beetle vigor preference, were determined by fitting model components to empirical data. An abrupt threshold for tree mortality to beetle densities allows for model simplification. Based on initial beetle density, model outcomes vary from decimation of the entire stand in a single year, to inability of the beetles to infect any trees. An intermediate outcome involves an initial

Fine structure histopathology of labyrinthitis ossificans in the gerbil model.

Labyrinthitis ossificans (LO) is the pathological deposition of new bone within the lumen of the cochlea and labyrinth. This process occurs most commonly as a result of infection or inflammation affecting the otic capsule. Trauma and vascular compromise can also lead to neo-ossification within the otic capsule. The mechanism that regulates this process remains unestablished. This study details the end-stage histopathology in high-resolution plastic thin sections. Twenty Mongolian gerbils were infected by intrathecal injection of Streptococcus pneumoniae type 3 followed by subcutaneous penicill

NMR structure of a gemcitabine-substituted model Okazaki fragment.

Gemcitabine (2'-deoxy-2',2'-difluorodeoxycytidine; dFdC) is a potent anticancer drug that exerts cytotoxic activity, in part, through incorporation of the nucleoside triphosphate dFdCTP into DNA and perturbations to DNA-mediated processes. The structure of a model Okazaki fragment containing a single dFdC substitution, [GEM], was determined using NMR spectroscopy and restrained molecular dynamics to understand structural distortions that may be induced in replicating DNA resulting from dFdC substitution. The electrostatic surface of [GEM] was also computed to determine how the geminal difluoro

Metal-catalyzed oxidation of a structured lipid model emulsion.

The effects of temperature, time, metal, citric acid, and tocopherol contents on the oxidation stability of a model oil-in-water emulsion prepared with enzymatically synthesized menhaden oil-caprylic acid structured lipid were evaluated by response surface methodology. The emulsions were stabilized by whey protein isolate. Oxidation was monitored by measuring lipid hydroperoxides and thiobarbituric acid reactive substances (TBARS). Cupric sulfate and ferrous sulfate were used to study the effect of metal concentration and type. A statistical model was developed to determine the relationships b

Functional and structural measurements in a multifactorial glaucoma risk model.

PURPOSE: To evaluate the relationship between a multifactorial probability risk model of developing glaucomatous visual field defects and the assessment of the retinal nerve fiber layer (RNFL) and short-wavelength automated perimetry (SWAP) in glaucoma suspects. METHODS: 157 eyes of 157 glaucoma suspects were included in the study. The risk of developing glaucomatous defects was assessed by applying a multifactorial model that included intraocular pressure, vertical cup-to-disk ratio, age and family history. Photographs of the RNFL and SWAP were performed. RESULTS: The SWAP was abnormal in 5

Modeling otoacoustic emission and hearing threshold fine structures.

A class of cochlear models which account for much of the characteristic variation with frequency of human otoacoustic emissions and hearing threshold microstructure is presented. The models are based upon wave reflections via distributed spatial cochlear inhomogeneities and tall and broad cochlear activity patterns, as suggested by Zweig and Shera [J. Acoust. Soc. Am. 98, 2018-2047 (1995)]. They successfully describe in particular the following features: (1) the characteristic quasiperiodic frequency variations (fine structures) of the hearing threshold, synchronous and click-evoked emissions,

Family structure and depressive symptoms among older adults in China: A marginal structural model analysis.

BACKGROUND: Mental well-being in the aging population is inevitably linked to families due to the reliance of older adults on family members. This study investigates the causal relationship between family structure and depressive symptoms among Chinese older adults in general and between gender and residential areas. METHODS: We used China Health and Retirement Longitudinal Study (CHARLS) panel data, covering four data collection rounds over seven years. Family structure was classified into single-member, couple, nuclear family, and extended family. Taking into account time-varying confoundin

Mapping Global and Regional Risks: Structural Insights From Systems Thinking and Interpretive Structural Modeling.

This study contributes to global risk analysis by providing a structural and exploratory enhancement to the analytical framework used to interpret the World Economic Forum's Global Risks Report (GRR), particularly the 2024 edition. It explores the complex interactions among critical global and regional risks using interpretive structural modeling (ISM) and systems thinking (ST)-based causal loop diagrams (CLDs). Rather than offering validated intervention guidance or predictive modeling, the study focuses on qualitative and structural mapping of risk interdependencies. Instead of combining the

Behavioral and physiological pain structures of PIPP-R and parental stress: structural equation modeling approach.

BACKGROUND: Although most non-acute pain assessment tools are multi-dimensional (behavioral and physiological measures) in their approach, the outputs of such tools are considered unidimensional. This study aimed to explore and determine the behavioral and physiological pain structures of Premature Infant Pain Profile-Revised (PIPP-R) for neonates and its association with parental stress. METHODS: This cross-sectional study was conducted in 2022-2023 in Isfahan, Iran. We recruited 400 pre-term infants, i.e. with gestational age (GA) of less than 37weeks who were admitted to the NICU of educat

Symmetrical structure design of PLGA Biodegradable sinus stents and structure optimization based on surrogate models.

This study aims to enhance the degradation uniformity of PLGA sinus stents to minimize fracture risk caused by stress corrosion. Symmetric stent structures were introduced and compared to sinusoidal structure in terms of stress and degradation uniformity during implantation and degradation processes. Three surrogate models were employed to optimize the honeycomb-like structure. Results showed honeycomb-like structures exhibited the superior stress distribution and highest degradation uniformity. The kriging model achieved the smallest error and degradation uniformity of 83.24%. In conclusion,

Predictive model for congenital heart disease in children of Pakistan by using structural equation modeling.

BACKGROUND: The structural abnormality of the heart and its blood vessels at the time of birth is known as congenital heart disease. Every year in Pakistan, sixty thousand children are born with CHD, and 44 in 1000 die before they are a month old. Various studies used different techniques to estimate the risk factors of congenital heart disease, but these techniques suffer from a deficiency of capacity to present human understanding and a deficiency of adequate data. The current study provided an innovative approach by defining the latent variables to handle this issue and building a reasonabl

The trait model of the DSM-5 alternative model of personality disorder (AMPD): A structural review.

Publication of the Diagnostic and Statistical Manual for Mental Disorders, Fifth Edition (American Psychiatric Association, 2013), which included an alternative model of personality disorders (AMPD), was followed by an explosion of publications on the new model. The large majority of these publications focused on the AMPD's Criterion B, which consists of 25 trait facets arranged in 5 broad domains that bear strong similarity to the established five-factor model (FFM) of personality. The operationalization of AMPD traits that has received the most research attention is its self-report measure,

Using phantom variables in structural equation modeling to assess model sensitivity to external misspecification.

External misspecification, the omission of key variables from a structural model, can fundamentally alter the inferences one makes without such variables present. This article presents 2 strategies for dealing with omitted variables, the first a fixed parameter approach incorporating the omitted variable into the model as a phantom variable where all associated parameter values are fixed, and the other a random parameter approach specifying prior distributions for all of the phantom variable's associated parameter values under a Bayesian framework. The logic and implementation of these methods

Scalar correlation functions for model structure selection in high-dimensional time-series modelling.

Model structure selection is an important step in high-dimensional time-series modelling. Traditionally AIC and BIC have been used for this purpose, however, only post model estimation. On the other hand, modern approaches use penalized regression methods, but the optimization is in a user-specified model class. In this work, we propose a pre-estimation approach based on two novel correlation functions, namely, the scalar autocorrelation function (SACF) and the scalar inverse autocorrelation function (SIACF) for identifying the appropriate model class among the vector autoregressive (VAR), vec
