Intraoperative Monitoring of Facial Nerve Motor-Evoked Potentials in Children.

OBJECTIVE: To determine whether transcranial motor-evoked potential monitoring of the facial nerve (FNMEP) during eloquent tumor resection is feasible in children and can predict both immediate and postoperative facial nerve (FN) function. METHODS: We included 24 consecutive procedures involving 21 patients (median age 5.5 years, range 5 months to 15 years, 8 female) who were operated on with FNMEP monitoring by the first author in 2013 and 2014. During surgery, we maintained a constant response amplitude by increasing the stimulation intensity and aimed to establish a warning criterion based

Intraoperative monitoring of brain tissue oxygenation during arteriovenous malformation resection.

BACKGROUND: In normal perfusion pressure breakthrough (NPPB) it is assumed that following arteriovenous malformation (AVM) resection, vasoparalysis persists in the margins of the lesion and that a sudden increase in cerebral blood flow (CBF) after AVM exclusion leads to brain swelling and postsurgical complications. However, the pathophysiology NPPB remains controversial. The aim of our study was to investigate the oxygenation status in tissue surrounding AVMs and in the distant brain using intraoperative monitoring of cerebral partial pressure of oxygen (PtiO(2)) to achieve a better understand

Intraoperative brain oxygenation monitoring and vasospasm in aneurysmal subarachnoid hemorrhage.

BACKGROUND: Vasospasm is a frequent complication of subarachnoid hemorrhage (SAH), implicated in poor outcome, and prediction of its occurrence might be important on the therapeutic approach of this condition. Brain oxygenation in aneurysmal SAH was prospectively monitored during surgery in 28 patients, using brain tissue oxygen pressure (PbtO(2)), studying its association with the occurrence of post-operative vasospasm, detected by transcranial Doppler. METHODS: PbtO(2) monitoring was performed during surgery of 28 patients with aneurysmal SAH, using a polarographic microcatheter (Licox; GMS

Periacetabular osteotomy: validation of intraoperative fluoroscopic monitoring of acetabular orientation.

During periacetabular osteotomy (PAO) the acetabulum is reorientated and the correction monitored by one or more anteroposterior pelvic radiographs. Obtaining these images is time consuming and requires additional technical and personal resources. Such disadvantages could be overcome with the use of fluoroscopy. However, information obtained from fluoroscopy should have the same quality when compared to standard pelvic radiography. Our purpose was to define a suitable fluoroscopy setup and compare the information obtained from the fluoroscopic images to that obtained from a traditional anterop

Safety of intraoperative transcranial electrical stimulation motor evoked potential monitoring.

This article reviews intraoperative transcranial electrical stimulation (TES) motor evoked potential (MEP) monitoring safety based on comparison with other clinical and experimental brain stimulation methods and clinical experience in more than 15000 cases. Comparative analysis indicates that brain damage and kindling are highly unlikely. There have been remarkably few adverse events. Pulse train TES-induced or coincidental seizures (n = 5) are rare, probably because of very brief (<0.03 second) stimuli, anesthesia, and the general absence of predisposing cerebral conditions. Soft bite blocks

Novel intraoperative cerebral blood flow monitoring by laser-Doppler scanner.

Laser-Doppler (LD) flowmetry was used to measure tissue perfusion non-invasively and continuously during neurosurgical operations using an LD scanner. Scanning was usually completed in 20 seconds. Measurements were processed in software to provide a color-coded image of the tissue perfusion. Moreover, the measurement data, expressed in LD-units, could be used for statistical data analysis. No physical contact was necessary between the scanning device and the exposed brain tissue. The imager provided two-dimensional microvascular flow maps non-invasively and quantitatively during brain operatio

New perspectives in intraoperative facial nerve monitoring with antidromic potentials.

Electromyographic (EMG) activity recorded from the facial muscles in response to electrical and mechanical stimulation is the most popular method for continuous monitoring of the facial nerve during cerebellopontine-angle surgery. EMG recording is, however, extremely sensitive to the administration of neuromuscular blockers. An alternative technique for the continuous monitoring of the facial nerve [i.e., monopolar recording of facial nerve antidromic potentials (FNAPs)], is described. Ten subjects undergoing retrosigmoid vestibular neurectomy for Meniere's disease and 11 subjects operated on

Technical aspects of intraoperative monitoring of lower cranial nerve function.

The efficacy of monitoring facial nerve activity in decreasing long-term morbidity has promoted an interest in monitoring other at-risk cranial nerves during procedures that involve manipulation of the basal cranial nerves. This presentation details practical techniques for monitoring the lower cranial nerves, which have been experientially developed over the past 9 years. Emphasis is placed on the selection of electrodes and procedural changes required for reliable and safe stimulation of the basal cranial nerves. Either paired hook-wire or tethered needle electrodes can be used for monitorin

Intraoperative somatosensory evoked potential monitoring during acute pelvic fracture surgery.

Independent clinical neurological evaluation and intraoperative somatosensory evoked potential (SSEP) monitoring was performed on 30 vertically unstable hemipelvis fractures in 28 patients undergoing acute open reduction and internal fixation. Preoperative ipsilateral neurologic injury of the sciatic/lumbosacral plexus was noted in 15 of 30 fractures (50%). Significant unilateral SSEP changes occurred during manipulative reduction of two displaced sacroiliac joints and one sacral fracture. Because of the expeditious response of the surgical team, with release of traction/retraction, SSEP retur

Intraoperative monitoring of intact PTH during surgery for primary hyperparathyroidism.

The intraoperative differential diagnosis between adenoma and hyperplasia during surgery for primary hyperparathyroidism (pHPT) is sometimes difficult. Several methods have been proposed to aid the surgeon in deciding on the amount of parathyroid tissue to be resected. We examined the use of intraoperative monitoring of intact PTH in 47 patients operated upon for pHPT. The highly sensitive assay for intact PTH was modified to permit a total turn-around time from gland excision to obtained result of about 60 min. The correlation (r) between the results of the modified and the conventional metho

Intraoperative Neuromuscular Monitoring: Electromyography Monitor Versus Peripheral Nerve Stimulator. Does the Face Lie?

During intraoperative anesthetic care, the administration of neuromuscular blocking agents (NMBAs) may be required to facilitate endotracheal intubation or provide ongoing skeletal muscle relaxation during surgical procedures. Assessments of the train-of-four (TOF) responses may be used to judge the adequacy of block and the need for redosing of NMBAs. The TOF evaluation can be performed qualitatively, by manual palpation or visual evaluation of the twitch response, or quantitatively, by using devices that measure the amplitude of the twitches. As an operator-dependent technique, qualitative t
