Esophageal hypomotility (EH) is characterized by abnormal esophageal peristalsis, either from a reduction or absence of contractions, whereas spastic motor disorders (SMD) are characterized by an increase in the vigor and/or propagation velocity of esophageal body contractions. Their pathophysiology is not clearly known. The reduced excitation of the smooth muscle contraction mediated by cholinergic neurons and the impairment of inhibitory ganglion neuronal function mediated by nitric oxide are likely mechanisms of the peristaltic abnormalities seen in EH and SMD, respectively. Dysphagia and c

OBJECTIVE: In patients suffering from Disorders of Consciousness (DOC) electrophysiological recordings at bedside could serve as a complimentary and economical tool to improve diagnosis. We utilized a motor observation and imagination paradigm to gain new insights on preserved cognitive processing in DOC.
METHODS: EEG brain oscillations were analyzed in 10 VS/UWS (Vegetative State/Unresponsive Wakefulness Syndrome) patients and 7 MCS (Minimally Conscious State) patients and 21 controls during observation and imagination of a grasping movement and group statistics were conducted.
RESULTS: Whi

BACKGROUND: The indeterminate chronic or "asymptomatic" phase of Trypanosoma cruzi (Chagas' disease) infection is characterized by the absence of gastrointestinal symptoms, and has an estimated duration of 20 to 30 years. However, the intramural denervation that induces dysfunction of the gastrointestinal tract is progressive. Recently, epidemiological studies have shown that the seroprevalence for this infection in our area ranges between 2% and 3% of the population.
OBJECTIVE: To detect the presence of esophageal motor disorders in asymptomatic individuals chronically infected with Trypanos

The purpose of the study was to count and characterize the range of stereotypies--repetitive rhythmical, apparently purposeless movements--in developmentally impaired children with and without autism, and to determine whether some types are more prevalent and diagnostically useful in children with autism. We described each motor stereotypy recorded during 15 minutes of archived videos of standardized play sessions in 277 children (209 males, 68 females; mean age 4y 6mo [SD 1y 5mo], range 2y 11mo-8y 1mo), 129 with autistic disorder (DSM-III-R), and 148 cognitively-matched non-autistic developme

Several studies have suggested that there may be an increased prevalence of affective disorders in people with motor neuron disease (MND). However, the literature is inconsistent, possibly because of small sample sizes in the existing studies. The Canadian province of Alberta has a universal health care system in which physician contacts are recorded along with ICD-9-CM diagnostic codes. In this analysis, diagnostic codes indicative of MND and affective disorders were used. Stratified analysis and logistic regression were used in the analysis. There were 336 cases of MND leading to a prevalenc

Five patients affected by thalamic hand and international myoclonus have been evaluated and selected for chronic motor cortex stimulation. A quadripolar electrode strip was placed epidurally under local anesthesia through an MR-image-guided single precentral burr hole placed following the morphologic recognition of the central sulcus. Intra-operative stimulation was used to induce muscle contraction at the affected site to confirm the correct placement of the electrode. A one-week trial period preceded the implant of an internal pace-maker under general anesthesia. A remarkable decrease in pai

Charcot-Marie-Tooth disease (CMT), or inherited peripheral neuropathies, is one of the most frequent genetically inherited neurologic disorders, with a prevalence of approximately one in 2500 people. CMT is usually inherited in an autosomal dominant fashion, although X-linked and recessive forms of CMT also exist. Over the past several years, considerable progress has been made toward understanding the genetic causes of many of the most frequent forms of CMT, particularly those caused by mutations in Schwann cell genes inducing the demyelinating forms of CMT, also known as CMT1. Because the ge

As long as motor disorders are controlled by DOPAtherapy, exercise programs and rehabilitation would not appear to be essential for patients suffering from Parkinson's disease. Such measures do become necessary however when secondary occurrence of motor decline develops. Physical medicine and rehabilitation have not been really involved in Parkinson's disease and few articles have assessed the value of these programs. In fact, controlled randomized studies have faced two kinds of methodological difficulties, those due to rehabilitation practices, and those due to Parkinson's disease specificit

Motor evoked potentials (MEPs) were studied in 63 patients with disorders affecting central motor pathways. These were classified into five subgroups: motor neuron disease (16), multiple sclerosis (13), cerebral infarction (19), spinal cord lesions (10) and hereditary spastic paraplegia (5). Three patients with hysterical paraplegia were also studied. Results were compared to those obtained from 30 normal subjects. In normal subjects the mean central motor conduction time (CMCT) was 6.4 ms (range 3.4-8.1 ms, SD 1.0 ms) for abductor digiti minimi and 13.2 ms (range 9.7-17.0 ms, SD 2.3 ms) for t

Type-1 hereditary motor and sensory neuropathy (HMSN I) is a slowly progressive disease resulting in distal muscle weakness with atrophy, and in sensory disturbance. Restrictive lung disease and respiratory muscle failure, common in many advanced neuromuscular disorders, is not a predominant feature of HMSN-I. Recently, there have been several reports of respiratory dysfunction in patients with HMSN I, complicated by diaphragmatic weakness. In five patients with HMSN I (3 men and 2 women, mean age 55.4 yrs), we measured spirometric variables, maximal inspiratory pressure, and maximal expirator

The Authors report their experience in the study of oesophageal motor abnormalities in patients with gastroesophageal reflux disease. By an electromanometric test in 50 subjects (20 healthy volunteers and 30 patients with oesophagitis), they evaluated the results concluding that: 1) LES low pressure is correlated significantly with oesophagitis; 2) oesophageal peristaltic dysfunction could be the effect of oesophagitis rather than its cause.

The aim of this study was to evaluate, in an acutely brain-damaged population, the occurrence of motor impersistence and its relation to the side of lesion. All the patients hospitalized in a 10-month period for a stroke involving one hemisphere and able to cooperate were given, in the early days of illness, a motor impersistence battery requiring the maintenance of a position of eyelids, gaze, mouth, and limbs for 10 seconds. A control group of 50 patients without brain damage provided the basis for evaluating pathologic performance. Failure to keep eyes closed was the most common manifestati

Motor proficiency, in terms of speed, rhythm and absence of overflow, has previously been shown to distinguish nonlearning disabled hyperactive boys from matched controls. Accepted screening methods for selecting children with dyslexia do not include assessments of hyperactivity or other attentional deficits. Dyslexic children selected in this customary manner were compared with an otherwise matched group that had been screened for attentional disorders, on a series of repetitive and alternating movements of the fingers, hands, and feet. The screened dyslexic group performed more rapidly on fi

This report presents observations in 160 patients undergoing chronic spinal cord stimulation for various disorders of the motor system and compares the results obtained using older conventional two-electrode bipolar stimulation with a newly developed four-electrode multiple level system. Improvement was noted in 84% of the 75 patients with cerebral palsy, 67% of the 42 patients with dystonia, 62% of the 21 patients with torticollis and 73% of the 22 patients with post-traumatic neurologic loss. Significant improvements were noted when comparing the two-electrode system with the new multiple le

AIM: To study sleep-disordered breathing (SDB) in patients with motor neuron disease (MND).
MATERIAL AND METHODS: Ninety-two patients, 50 women and 42 men, were examined Median age was 62 [55; 67.5] years, MND duration 12 [8.9; 27.1] months. The control group consisted of 46 patients (26 men; 20 women). A portable polysomnographic study (PSG) (Polymate YH-1000C (BMC, China)) was conducted immediately after the diagnosis of MND, that is, at a relatively early stage.
RESULTS AND CONCLUSION: Significant changes in PSG in comparison with the control have been revealed. Significant differences we

Obsessive-compulsive disorder (OCD) is a clinically heterogeneous condition. Although its pathophysiology is not completely understood, neurophysiologic and neuroimaging data have disclosed functional abnormalities in the networks linking frontal cortex, supplementary motor and premotor areas, striatum, globus pallidus, and thalamus (CSPT circuits). By means of transcranial magnetic stimulation (TMS) it is possible to test inhibitory and excitatory circuits within motor cortex. Previous studies on OCD patients under medication have demonstrated altered cortical inhibitory circuits as tested by

We investigated motor inhibition abilities in trichotillomania (TTM) and obsessive-compulsive disorder (OCD), two disorders characterized by repetitive, intentionally performed behaviors. Performance in a GoNogo experiment of 25 TTM and 21 OCD participants was compared to the performance of 26 HC participants. In contrast to OCD and HC participants, TTM participants tended to perform either 'fast and inaccurate' (indicating poor motor inhibition) or 'slow and accurate'. TTM participants with poor motor inhibition reported a significantly earlier age of TTM onset than those TTM participants who
