Fasciculation frequency changes at different length of relaxed muscle.

We have studied, in chronically denervated muscles, fasciculation potentials (FPs) which were clearly identified when the muscles were either relaxed or passively stretched. The frequency of 10 FPs was determined in both positions in seven patients. In stretched muscles, two FPs were no longer recorded, and the mean FP frequency decreased significantly (P < .01). There was also a decrease of the FP amplitude. As FPs are frequently associated with ordinary muscle cramps, stretching of the muscle may lessen the frequency of FPs by the same mechanism which stops cramps.

Botulinum Toxin Therapy in Writer's Cramp and Musician's Dystonia.

Task-specific focal dystonia is characterized by muscle contraction(s) during a specific task, resulting in abnormal postures or movements. Specifically, writer's cramp involves the upper extremity during the act of writing. Musician's dystonia has a highly variable presentation, and thus makes therapeutic options more limited. Treatments include oral pharmacologic agents, neuromodulation, surgery and, most often, botulinum toxin (BoNT) injection. Selection of target muscles for toxin injection continues to be an area of active research for these task-specific movements. We present a review of

Bronchial involvement in the cramp-fasciculation syndrome.

BACKGROUND/AIMS: Cramp-fasciculation syndrome (CFS) is an acquired, chronic, usually benign and rather heterogeneous condition with isolated fasciculations and muscle cramps generally induced by physical exercise. They commonly involve calf and quadriceps muscles. The pathophysiology of CFS is related to peripheral nerve hyperexcitability, most often located at the motor nerve terminal or intramuscular arborization. METHODS: A 21-year-old man presented with a progressive syndrome of bronchospasm, cramps and muscle twitches related to physical exercise. Spirography showed bronchial hyperrespon

Does pausing ultrafiltration as a treatment for intradialytic hypotension cause backfiltration in hemodialysis?

Ultrafiltration is part of the hemodialysis procedure and can also be used exclusively to remove additional fluid in patients with fluid overload. Ultrafiltration that is too aggressive can result in hypotension, cramping, or other untoward effects. Temporarily pausing the ultrafiltration process is a technique used to help manage the adverse effects of rapid fluid removal. Some nurses prefer not to pause ultrafiltration because of the concern for backfiltration, and they elect to manage hypotension and cramping by normal saline infusions or other interventions. This article reviews the eviden

[Regressive mitochondrial abnormalities in the muscular biopsy of a bicycling champion].

The onset of severe muscle cramps in the course of road racing led a champion to undergo a muscle biopsy. Fuscinophilic deposits were detected around muscle cell fibers together with a few PAS positive deposits. These findings were related to the accumulation of abnormal mitochondria and glycogen granules. The subsequent study of the mitochondrial respiratory chain did not reveal any modification and so ruled out a genuine mitochondrial myopathy. Such mitochondrial modifications are probably related to the onset of muscle cramps, but no metabolic disturbance could be found to account for these

Neuromuscular Electrical Stimulation Reduces Leg Cramps in Patients With Lumbar Degenerative Disorders: A Randomized Placebo-Controlled Trial.

OBJECTIVES: Lumbar spinal stenosis (LSS) and lumbar disc herniation (LDH) are often accompanied by frequently occurring leg cramps severely affecting patients' life and sleep quality. Recent evidence suggests that neuromuscular electric stimulation (NMES) of cramp-prone muscles may prevent cramps in lumbar disorders. MATERIALS AND METHODS: Thirty-two men and women (63 ± 9 years) with LSS and/or LDH suffering from cramps were randomly allocated to four different groups. Unilateral stimulation of the gastrocnemius was applied twice a week over four weeks (3 × 6 × 5 sec stimulation trains at 30

Abnormal plasticity of the sensorimotor cortex to slow repetitive transcranial magnetic stimulation in patients with writer's cramp.

Previous studies demonstrated functional abnormalities in the somatosensory system, including a distorted functional organization of the somatosensory cortex (S1) in patients with writer's cramp. We tested the hypothesis that these functional alterations render S1 of these patients more susceptible to the "inhibitory" effects of subthreshold 1 Hz repetitive transcranial magnetic stimulation (rTMS) given to S1. Seven patients with writer's cramp and eight healthy subjects were studied. Patients also received rTMS to the motor cortex hand area (M1). As an outcome measure, short-latency afferent

Treatment of cervical dystonia hand spasms and laryngeal dystonia with botulinum toxin.

One hundred and twenty-six patients with different forms of focal dystonia (89 with cervical dystonia, 12 with hand cramps and 25 with laryngeal dystonia) were treated with localised injections of botulinum toxin. Mean doses per muscle were 200 mouse units (m.u.) for treating cervical dystonia, 40-120 m.u. for forearm muscles in writers' cramp and 3.7 m.u. for the thyroarytenoid muscle in laryngeal dystonia. Responder rates have been above 80% in all patient groups and beneficial effects could be reproduced over follow-up periods of up to 4 years. The commonest side-effects were dysphagia afte

Abnormal reorganization in focal hand dystonia--sensory and motor training programs to retrain cortical function.

Dystonia is a disabling movement disorder, which is characterized by an abnormal pattern of muscle activity with co-contraction of agonist and antagonist muscles. In the case of focal hand dystonia (FHD), these abnormal movements affect muscles of the forearm and hand while performing a specific task. Patients may initially present with dystonic symptoms occurring with a selective task (simple writer's cramp or musician's cramp), and may progress to develop symptoms with multiple tasks (dystonic writer's cramp). The underlying cause of this disabling condition remains unclear. This review exam

Increased HCN channel driven inward rectification in benign cramp fasciculation syndrome.

Muscle cramps are a common complaint associated with sudden painful involuntary contractions of a muscle. The mechanisms responsible for muscle cramps are still not clear. Axonal excitability and multi-unit electromyography studies were performed in 20 patients suffering from benign cramp fasciculation syndrome, not currently on medication. The measures of axonal excitability suggested greater inward rectification, indicative of an increase in Ih. Mathematical modelling suggested that the data were best explained by depolarization of the voltage dependence of hyperpolarization-activated cyclic

Focal dystonia and repetitive motion disorders.

It commonly is observed that focal hand dystonias, such as writer's cramp or musician's cramp, are associated with repetitive movements, although definitive proof of a causal relationship is lacking. These focal dystonias are often task specific, with involuntary muscle contractions occurring only when patients perform specific acts such as writing or playing a musical instrument. Physiologic studies show deficiencies in spinal reciprocal inhibition and abnormalities of central sensory processing and motor output that may be related to reduced cortical inhibition. Recent studies in primates su

The Role of Electrolytes in Muscle Pain Syndromes: A Systematic Review and Meta-Analysis With Implications for Temporomandibular Disorder.

BACKGROUND: Temporomandibular disorders (TMDs) are a major cause of chronic orofacial pain, with myalgia of the masticatory muscles being central to symptom burden. Electrolyte modulation, particularly magnesium, may influence neuromuscular excitability and nociceptor sensitization, but no systematic review has synthesized the evidence for muscle pain syndromes or its relevance to TMD. OBJECTIVES: To evaluate the efficacy of electrolyte supplementation (magnesium, sodium, calcium, and potassium) in reducing muscle cramps and myalgia, and to explore the biological plausibility and potential ex

Striatal dopaminergic dysfunction at rest and during task performance in writer's cramp.

Writer's cramp is a task-specific focal hand dystonia characterized by involuntary excessive muscle contractions during writing. Although abnormal striatal dopamine receptor binding has been implicated in the pathophysiology of writer's cramp and other primary dystonias, endogenous dopamine release during task performance has not been previously investigated in writer's cramp. Using positron emission tomography imaging with the D2/D3 antagonist 11C-raclopride, we analysed striatal D2/D3 availability at rest and endogenous dopamine release during sequential finger tapping and speech production
