Sodium retention in nephrotic syndrome is due to an intrarenal defect: evidence from steroid-induced remission.

There is increasing evidence that the sodium retention of nephrotic syndrome is directly due to an intrarenal mechanism and not to a low blood volume stimulating the renin-angiotensin-aldosterone system. However the mechanism of the natriuresis that occurs during remission is not known. Patients with nephrotic syndrome were therefore studied during steroid-induced remission. At the onset of natriuresis, blood volume and plasma albumin were low and did not change. Plasma renin activity and plasma aldosterone were initially high and both fell during the natriuresis. At the end of the natriuresis

Clinical relevance of dietary salt intake on aldosterone and the aldosterone-to-renin ratio as screening parameters for primary aldosteronism.

AIMS: The recommendations for screening for primary aldosteronism (PA) are determination and interpretation of both plasma aldosterone and the aldosterone-renin ratio (ARR). Although it is known that oral sodium chloride intake has an important impact on plasma aldosterone and ARR, more detailed data of this impact are sparse. We evaluated the relevance of natriuresis as a parameter of oral sodium intake, as well as patient age and antihypertensive medication on the PA screening parameters in our hypertensive patient population. METHOD: Our cross-sectional, single-center study investigated th

Effects of Nifedipine on Renal and Cardiovascular Responses to Neuropeptide Y in Anesthetized Rats.

Neuropeptide Y (NPY) acts via multiple receptor subtypes termed Y1, Y2 and Y5. While Y1 receptor-mediated effects, e.g., in the vasculature, are often sensitive to inhibitors of L-type Ca2+ channels such as nifedipine, little is known about the role of such channels in Y5-mediated effects such as diuresis and natriuresis. Therefore, we explored whether nifedipine affects NPY-induced diuresis and natriuresis. After pre-treatment with nifedipine or vehicle, anesthetized rats received infusions or bolus injections of NPY. Infusion NPY (1 µg/kg/min) increased diuresis and natriuresis, and this was

The significance of the furosemide test for predicting ascites control by diuretics in cirrhotics: a comparison with volume expansion and octreotide infusion.

To verify prospectively the usefulness of the furosemide-induced natriuresis test in predicting ascites control by medical treatment, 15 stable cirrhotics (9 male) with ascites were studied. Sodium excretion was measured after this test and after volume expansion with saline associated with intravenous infusion of octreotide; 6 months later, response to medical treatment was rated as good (N=9) or poor (N=6). Patients with poor ascites control had lower sodium excretion with the furosemide-induced natriuresis test (median, 88 vs 201 mmol; P < 0.01). Poor control was observed in four of four pa

Diuretic response and effects of diuretic omission in ambulatory heart failure patients on chronic low-dose loop diuretic therapy.

AIMS: To study loop diuretic response and effect of loop diuretic omission in ambulatory heart failure (HF) patients on chronic low-dose loop diuretics. METHODS AND RESULTS: Urine collections were performed on two consecutive days in 40 ambulatory HF patients with 40-80 mg furosemide (day 1 with loop diuretic; day 2 without loop diuretic). Three phases were collected each day: (i) first 6 h; (ii) rest of the day; and (iii) night. On the day of loop diuretic intake, the total natriuresis was 125.9 (86.9-155.0) mmol/24 h and urine output was 1650 (1380-2025) mL/24 h. There was a clear loop diur

Kidney dopamine D1-like receptors and angiotensin 1-7 interaction inhibits renal Na+ transporters.

The role of dopamine D1-like receptors (DR) in the regulation of renal Na+ transporters, natriuresis, and blood pressure is well established. However, the involvement of the angiotensin 1-7 (ANG 1-7)-Mas receptor in the regulation of Na+ balance and blood pressure is not clear. The present study aimed to investigate the hypothesis that ANG 1-7 can regulate Na+ homeostasis by modulating the renal dopamine system. Sprague-Dawley rats were infused with saline alone (vehicle) or saline with ANG 1-7, ANG 1-7 antagonist A-779, DR agonist SKF38393, and antagonist SCH23390. Infusion of ANG 1-7 caused

Atrial natriuretic factor and salt wasting after aneurysmal subarachnoid hemorrhage.

BACKGROUND AND PURPOSE: The causes of volume depletion and hyponatremia after subarachnoid hemorrhage are not fully understood but may be in part due to natriuresis or "cerebral salt wasting." Because previous studies using infrequent hormone sampling have given inconsistent results, we determined if elevations in atrial natriuretic factor concentrations preceded negative sodium and fluid balances. METHODS: We measured diurnal atrial natriuretic factor and vasopressin concentrations and sodium balance for 5 days in 14 consecutive patients after aneurysmal subarachnoid hemorrhage. RESULTS: Pl

Renal antigen-presenting cells from ANG II hypertensive donors transfer blood pressure and promote sodium retention.

Antigen-presenting cells (APCs) are present in the renal interstitium and may modulate tubular function. We hypothesize that angiotensin II (Ang II) induces a prohypertensive phenotype in renal APCs, contributing to decreased natriuresis and hypertension. We evaluated the role of renal APCs as modulators of blood pressure (BP) in CD11c.DOG mice injected with diphtheria toxin (DT). Elimination of 70% of renal APCs by DT prevented the increase in BP, cardiac hypertrophy, decreased natriuresis, and sodium-potassium-chloride cotransporter type II (NKCC2) activation. Second, we compared the effect

Renal sodium handling in cirrhosis with ascites: mechanisms of impaired natriuretic response to reclining.

We recently showed that patients with compensated cirrhosis can dispose of their fluid overload while reclining. In contrast, patients with ascites fail to develop supine-induced natriuresis. To assess the effect of reclining on renal sodium handling in patients with advanced cirrhosis and the mechanisms blunting natriuresis in this situation, renal function and plasma concentrations of atrial natriuretic factor, aldosterone and norepinephrine were evaluated in 10 nonazotemic patients with cirrhosis and ascites and 10 healthy controls standing for 2 h and reclining for 2 h. While standing, all

[Comparison of the diuretic and natriuretic effects of dopamine in control and diabetes insipidus dogs. Effect of propranolol].

The diuretic effect of dopamine is unchanged in the dog with diabetes insipidus but the natriuretic effect of dopamine is shorter. With the normal dog propanolol delays the dopamine-induced diuresis and natriuresis but the magnitude of this double effect is unchanged. In the dog with diabetes insipidus, propanolol suppresses the dopaminergic diuresis and reduces the natriuresis.

Diuretics in cardiovascular therapy: the new clinicopharmacological bases that matter.

Diuretics in current use include early distal tubular (i.e., thiazide-type), loop (i.e., furosemide-type), and potassium-and-hydrogen-retaining substances. Available oral formulations of diuretics differ in terms of their renal excretory potency in man, as formally assessed through the effect of a single dose on 24-hour natriuresis in healthy subjects. The 2.5 mg formulation of the loop diuretic torasemide does not increase mean 24-hour natriuresis, and it is therefore considered a very-low-dose formulation. Amiloride 5 mg and torasemide 5 mg and 10 mg, which increase mean 24-hour natriuresis

Mechanism of renal effects of intracerebroventricular histamine in rabbits.

Histamine, when given intracerebroventricularly (i.c.v.), has been reported to produce antidiuresis in the rabbit. In this study it was attempted to elucidate the mechanism involved in the effect. Histamine (H), 100 micrograms/kg i.c.v., produced antidiuresis with decreases in renal plasma flow and glomerular filtration rate in urethane-anesthetized rabbits. With larger doses, a tendency towards increased electrolyte excretion was noted in spite of decreased filtration. In the denervated kidney, marked diuresis and natriuresis were observed following i.c.v. H, whereas the contralateral innerva

Night-to-Day Ratio of Excreted Urinary Sodium Concentration Using Spot Urine in Older Adults With Nocturnal Polyuria.

BACKGROUND: The diurnal rhythm in water and solute diuresis is known to be disturbed in older adults with nocturia due to nocturnal polyuria. We estimated the prevalence of increased natriuresis in nocturia due to nocturnal polyuria by comparing individuals with and without nocturnal polyuria. METHODS: We calculated the night-to-day ratio of excreted urinary sodium and potassium concentrations adjusted for urinary creatinine concentration and urinary osmolality using spot urine samples collected at 2 PM (daytime) and 6 AM (nighttime) and compared results among controls with nocturia and paite

Loop Diuretic Dose Intensification versus Adjuvant Thiazide for Diuretic Resistance in Acute Heart Failure: Mechanistic Randomized Controlled Trial.

KEY POINTS: Adjuvant thiazide improved diuretic response more than loop dose increase in patients with acute heart failure and diuretic resistance. The underlying diuretic resistance mechanism did not identify preferential response to either diuretic escalation strategy. BACKGROUND: The optimal therapy for loop diuretic resistance may differ if caused by resistance at the site of action in the loop of Henle (diuretic resistance at the loop of Henle [DR-Loop]) versus compensatory distal reabsorption (diuretic resistance due to distal sodium reabsorption [DR-Distal]). This study primarily aimed

Natriuretic response to an acute oral potassium load in healthy individuals and patients with chronic kidney disease: a randomized controlled trial.

BACKGROUND: The health benefits of dietary potassium have been linked to potassium-induced natriuresis. However, it is unknown whether potassium loading in patients with chronic kidney disease (CKD) induces natriuresis and to what degree it increases plasma potassium. We hypothesize that potassium-induced natriuresis remains intact and that potassium loading is safe in patients with CKD. METHODS: In this placebo-controlled crossover study, 18 healthy individuals and 9 patients with non-diabetic CKD stage G3b-G4 received an acute oral load of potassium chloride (KCl), potassium citrate (K-cit)

Diuretic effect of acute hypoxia in humans: relationship to hypoxic ventilatory responsiveness and renal hormones.

Acute hypoxia causes increased sodium and water excretion. Animal studies suggest that this renal response is largely driven by direct peripheral arterial chemoreceptor stimulation, independent of accompanying changes in ventilation and acid-base status. Whether the diuresis and natriuresis observed in humans made acutely hypoxic are caused by peripheral chemoreceptor stimulation is not known, but, if so, we hypothesized that people with a high ventilatory response to hypoxia (high peripheral chemosensitivity) should have greater diuresis and natriuresis than those with a low ventilatory respo

Urinary sodium and right ventricular function in acute heart failure.

AIMS: Urinary sodium (UNa) is a powerful predictor of adverse events in acute heart failure (AHF), although its etiopathogenic mechanism remains poorly understood. Previous studies highlight the importance of venous congestion on renal function in AHF. Our objective was to evaluate the relationship between right ventricular (RV) function and UNa, as well as their prognostic interaction in AHF. METHODS: We prospectively analyzed 101 patients hospitalized for AHF, divided into high or low natriuresis groups based on the median natriuretic response to a furosemide stress test. We performed a com

Circulating Dipeptidyl Peptidase 3 levels, an Antagonist of Angiotensin-(1-7), in Acute Heart Failure: Insights from the PUSH-AHF trial.

BACKGROUND: Angiotensin-(1-7) promotes vasodilation and counteracts angiotensin II in the vasculature and kidneys, while dipeptidyl peptidase 3 (DPP3) inactivates Ang-(1-7). Although circulating DPP3 (cDPP3) has been studied in chronic heart failure (HF), data in acute HF are limited. We evaluated cDPP3 levels and their association with clinical characteristics, decongestive response, and outcomes in acute HF. METHOD: We analyzed patients enrolled in the randomized PUSH-AHF trial comparing natriuresis-guided diuretic therapy with standard of care (SOC) in acute HF. cDPP3 levels were measured

Chronic ACTH Infusion Alters the Diurnal Rhythm of Sodium Excretion, Inducing Nondipping Blood Pressure and Salt-Sensitivity in Male Mice.

BACKGROUND: Cushing syndrome causes hypertension and increased cardiovascular risk. The hypertensive mechanisms are not clearly defined. We hypothesized that glucocorticoid excess would induce salt-sensitivity, reflecting an impaired pressure-natriuresis response and abnormal salt handling by the kidney. METHODS: We modeled Cushing syndrome in male C57BL/6J mice with prolonged adrenocorticotropic hormone (ACTH) infusion and measured blood pressure on a control diet and following high-salt intake. Sodium artesunate-induced diuresis in a patient with malaria.

Sodium artesunate is used in the treatment of malaria. We report a case of sodium artesunate induced diuresis and natriuresis in a patient with malaria. Following artesunate administration there was polyuria accompanied by natriuresis that was reversed after discontinuation of artesunate treatment. The diuretic effect of artesunate can modify the course of renal failure in severe malaria. Prescribers should be aware of this effect of artesunate, so that it can be used judiciously and to the advantage of patients with severe malaria.
