Cryotherapy in the management of premalignant and malignant conditions of the esophagus.

Endoscopic cryotherapy is a relatively new thermal ablative modality used for the treatment of neoplastic lesions of the esophagus. It relies on cycles of rapid cooling and thawing to induce tissue destruction with a cryogen (liquid nitrogen or carbon dioxide) leading to intra and extra-cellular damage. Surgical treatment was once considered the standard therapeutic intervention for neoplastic diseases of the esophagus and is associated with considerable rates of morbidity and mortality. Several trials that evaluated cryotherapy in Barrett's esophagus (BE) associated neoplasia showed reasonabl

Multiplanar magnetic resonance imaging of the sacral foramina and presacral space.

The sacrum and presacral space present a complex and somewhat troublesome area for radiologic evaluation but are of considerable significance in disease processes of interest to oncology, neurosurgery, and orthopedic surgery. The multiplanar capabilities of magnetic resonance imaging (MRI) can be employed to define fracture pathology and neoplastic destruction alike. Some alternate MRI anatomic planes are presented and explored in normal volunteers and in 30 random clinical cases.

Cardiac tamponade due to group a streptococcus descending necrotizing mediastinitis: case report of an unusual presentation.

BACKGROUND: Cardiac tamponade from necrotizing descending mediastinitis is a rare but life-threatening complication of cervicofacial infections. CASE REPORT: A 49-year-old woman presented in shock with pleuretic chest pain at a small community clinic. She was transferred to our emergency department where cardiac tamponade was diagnosed and drained. Her initial complete blood count and chest radiography suggested a neoplastic process. She, however, was diagnosed with descending necrotizing mediastinitis due to group A Streptococcus. She underwent surgical drainage and recovered uneventfully. W

Monoclonality in normal epithelium and in hyperplastic and neoplastic lesions of the breast.

The clonal nature of neoplastic lesions such as invasive breast cancer and ductal carcinoma in situ (DCIS) has been widely proven by several proliferative, genetic or other malignancy-associated markers. The aim of this study is to clarify whether benign hyperplastic lesions such as ductal hyperplasia of usual type (DH) and papilloma can be distinguished from neoplastic lesions such as DCIS by X-chromosome inactivation analysis. Clonal analysis was performed using a polymerase chain reaction-based assay for non-random X-chromosome inactivation of the human androgen receptor gene (HUMARA). Form

Utility of Magnetic Resonance Spectroscopy and Perfusion Imaging in Differentiating Brain Tumors From Mimics in a Tertiary Hospital in the Philippines.

BACKGROUND: A wide range of non-neoplastic brain lesions can mimic tumors on magnetic resonance imaging (MRI), creating diagnostic challenges. Although MRI is the gold standard for evaluating brain lesions, differentiating between neoplastic and non-neoplastic lesions, as well as high- and low-grade tumors, can be difficult, sometimes leading to unnecessary biopsies. Magnetic resonance spectroscopy (MRS) helps by analyzing biochemical and metabolic processes, especially when conventional MRI falls short. Perfusion MRI (MRP), sensitive to microvasculature, is used to classify tumors, detect str

The role of CD47 in non-neoplastic diseases.

CD47 is a 50 kDa five-spanning membrane receptor that plays a crucial role in multiple cellular processes, including myeloid cell activation, neutrophils transmigration, vascular remodeling, leukocyte adhesion and trans-endothelial migration. Recent studies have revealed that CD47 is a highly expressed anti-phagocytic signal in several types of cancer, and therefore, blocking of CD47 has shown an effective therapeutic potential in cancer immunotherapy. In addition, CD47 has been found to be involved in a complex interplay with microglia and other types of cells, and increasing evidence indicat

Fibrosing/Sclerosing Lesions of the Mediastinum: A Review.

Fibrosing lesions of the mediastinum represent a small but challenging group of lesions that range in etiology from infectious to idiopathic to neoplastic. The diagnosis of such lesions becomes more challenging in the setting of mediastinoscopic biopsies. In addition, over the years, there has been further accumulation of knowledge of the clinical aspects of these lesions that needs to be incorporated into their evaluation. Therefore, it is essential that in the general evaluation of these fibrosing processes, one not only carefully examines the histopathologic features of the lesion, that of

Imaging of the Posterior Skull Base.

The posterior skull base can be involved by a variety of pathologic processes. They can be broadly classified as: traumatic, neoplastic, vascular, and inflammatory. Pathology in the posterior skull base usually involves the lower cranial nerves, either as a source of pathology or a secondary source of symptoms. This review will categorize pathology arising in the posterior skull base and describe how it affects the skull base itself and surrounding structures.

Cutaneous xanthomatous tumours as an expression of chronic myelomonocytic leukaemia?

Nonspecific cutaneous xanthomas have been reported in a variety of lymphocytic neoplastic processes, but to date only three cases of xanthomatous lesions associated with monocytic leukaemias have been described. We now report a patient with a chronic myelomonocytic leukaemia (CMML) associated with these lesions. The clinical and immunohistochemical features do not correspond to any entity previously described and suggest that xanthomas is are a cutaneous expression of the CMML.

[Ultrasonography of cervical cysts and their differential diagnosis].

Among 22 proven branchiogenic or thyreoglossal cysts, 15 presented with a characteristic pattern of homogeneous smooth reflections within a well-demarcated thin and elastic, deformable wall when using a high-resolution transducer. The other cases, showing an inhomogeneous texture and/or irregular walls, cannot be distinguished from inflammatory or neoplastic processes with central necrosis. Complete surgical removal is recommended especially in these latter cases. One case of malignant transformation was found.

[Computerized tomography findings in annular pancreas in an adult].

We present a case of annular pancreas in an adult female studied by computerized tomography. CT demonstrated an enlarged pancreatic head in the center of which was visualized the contrast-filled duodenal lumen. We consider these findings to be sufficiently characteristic of annular pancreas to permit differential diagnosis with neoplastic processes of the pancreatic head without having to resort to more invasive diagnostic methods like endoscopic retrograde cholangiography.

Computed tomographic identification of calcified optic nerve drusen.

Four cases of optic disk drusen were accurately diagnosed with orbital computed tomography (CT). The radiologist should be aware of the characteristic CT finding of discrete calcification within an otherwise normal optic disk. This benign process is easily differentiated from lesions such as calcific neoplastic processes of the posterior globe. CT identification of optic disk drusen is essential in the evaluation of visual field defects, migraine-like headaches, and pseudopapilledema.

Indications for gonioscopy and assessment of gonioscopic signs in optometric practice.

The indications for optometrists to carry out gonioscopic examination are proposed and their significance is discussed. Gonioscopic examination is directed toward recognition of angle closure, or risk of closure, as well as neovascularization, neoplastic activity, inflammation, the results of injury, or degenerative changes at the angle. A schema sets out the decision-making processes which may be used to assist interpretation of gonioscopic signs and their correlation with other clinical data.

Inguinoscrotal Pathology on Computed Tomography: An Alternative Perspective.

Computed tomography (CT) is not the imaging technique of choice to assess inguinoscrotal pathology, as magnetic resonance or ultrasonography have superior soft tissue contrast resolution and do not involve gonadal exposure to ionizing radiation. However, testicular and inguinoscrotal pathology may be found both as an extension of intra-abdominal processes or incidentally on CT scans requested for other reasons. CT also plays a role in the evaluation of testicular injury when associated to pelvic trauma and in perineal infections with scrotal extension. A pictorial review of testicular and ingu

Gorham disease in the maxilla.

Gorham disease is a rare condition that is characterized by the proliferation of thin-walled vascular channels associated with regional osteolysis. The exact etiology of Gorham disease is unknown. The diagnosis of Gorham disease is based on clinical, radiological, and histological features after excluding osteolysis, which is secondary to other pathologic processes. Those pathologic processes include congenital, metabolic, neoplastic, and immunologic etiologies and infections. The appearance of the disease in the craniofacial region often involves the mandible. Malignancies Presenting with Alveolar Infiltrates: Diagnostic Pitfalls, Radiologic Clues, and Clinical Patterns.

Alveolar infiltrates are a common but nonspecific radiologic finding that can obscure the diagnosis of underlying malignancy. While infections and inflammatory processes are typical considerations, a subset of cancers, both primary and secondary, can present with alveolar opacities that mimic these benign conditions. This review synthesizes the spectrum of neoplastic diseases that manifest with an alveolar radiographic pattern, focusing on both primary pulmonary malignancies (such as lepidic-predominant adenocarcinoma, invasive mucinous adenocarcinoma, and pulmonary lymphoma) and select metast

A Systematic Approach to the Histologic Evaluation of Terminal Ileal Biopsies.

BackgroundIleal biopsies represent a critical diagnostic tool in gastroenterological practice, with particular relevance in the evaluation of Crohn's disease (CD). However, the ileal mucosa can harbor numerous nonneoplastic and neoplastic conditions that may clinically and histologically mimic CD. Accurate interpretation requires a nuanced understanding of normal ileal histology and its variants, alongside a structured diagnostic algorithm.ObjectivesTo delineate the spectrum of pathologic processes affecting the ileum and establish a systematic diagnostic approach, while highlighting potential

[Non-neoplastic lesions of the mediastinum].

The mediastinum is a complex body region of limited space but containing numerous organs of different embryonic origins. A variety of lesions that are difficult to distinguish from each other can occur here. Non-neoplastic lesions of the mediastinum represent important differential diagnostic pitfalls to mediastinal tumors, clinically, radiologically and histopathologically. It is important to bear these lesions in mind and to adequately verify or exclude them before starting further differential diagnostic considerations on mediastinal neoplasms. The most common non-neoplastic lesions in this

Diagnostic characteristics of sinonasal organizing hematomas: avoiding misdiagnosis.

BACKGROUND: Organizing hematomas of the paranasal sinuses are diagnostic dilemmas clinically and radiographically, mimicking benign or malignant neoplastic processes and causing patients and clinicians undue worry regarding these diagnoses. Diagnostic criteria for correctly identifying these lesions are not well known. METHODS: A retrospective case series of 7 patients with sinonasal organizing hematoma was studied. Radiographic imaging, clinical characteristics, and pathology were reviewed for new insights. RESULTS: Three patients presented with a primary complaint of epistaxis, 4 had masse
