Risk factors for nephrolithiasis in patients with familial idiopathic hypercalciuria.

PURPOSE: About 40% of patients with nephrolithiasis have idiopathic hypercalciuria, sometimes associated with a family history of kidney stones. In these families, little is known about the frequency of, and risk factors for, stone formation among hypercalciuric patients. We therefore conducted a prospective study of 216 subjects from 33 families with idiopathic hypercalciuria. MATERIALS AND METHODS: We recorded the age, weight, and history of calcium stones in all subjects, and measured 24-hour urine volume and excretion of calcium, uric acid, sodium, magnesium, urea, citrate, phosphate, and

The incidence of nephrolithiasis in patients with spinal neural tube defects.

PURPOSE: Bladder stones are common in patients with spinal neural tube defects but there are little data on the incidence of renal calculi in this population. We examined the incidence, nature and risk factors of nephrolithiasis in our clinic population of patients with neural tube defects. MATERIALS AND METHODS: We retrospectively reviewed the charts and radiological studies of 327 patients followed at our neural tube defects clinic with routine radiological imaging of the urinary tract. Additional confirmatory studies were performed when stones were noted. RESULTS: Renal calculi were ident

Nephrolithiasis as a presenting feature of chronic sarcoidosis: a prospective study.

BACKGROUND AND AIM OF THE WORK: The presentation of sarcoidosis with renal stones has never been evaluated in a prospective study. DESIGN: We have studied 110 consecutive patients with histologically proven sarcoidosis, first seen in the years 1993-1994 in the Milan Sarcoid Clinic. Those who had renal stones preceding or suggesting the diagnosis of sarcoidosis, and those who had evidence of renal calculi at presentation, were studied up to 31 December 1995 with serial evaluation of calcaemia, calciuria and serum 1,25 (OH)2D3 and are the basis of this report. RESULTS: Four patients had had ca

[Ultrasonic study of the parathyroid glands in patients with coralliform nephrolithiasis].

Functional activity of the parathyroid glands (PTG) is important for the choice of proper therapeutic policy in patients with dendritic nephrolithiasis. The location of the hyperfunctioning gland (usually adenomatously or hyperplastically changed) is a complicated problem as the dimensions of the gland are extremely small. Ultrasonic investigation of the parathyroid gland area for detection of the enlarged gland is used at the Research Institute of Urology, the RSFSR Ministry of Health. Linear (7.5 mHz) and sector (5 mHz) transducers have been used through the water medium and by the immediate

Urinary proteomics reveals association between pediatric nephrolithiasis and cardiovascular disease.

PURPOSE: To study (1) the differences in the relative abundance of urinary proteins between children with kidney stones (RS) and hypercalciuria, hypocitraturia, normal metabolic work-up, and healthy controls (HC); (2) the association of these proteins with various diseases. METHODS: Quantitative proteomic comparison of pooled urine from RS (N = 30, 24 females, mean age 12.95 ± 4.03 years) versus age- and gender-matched HC, using mass spectrometry. Relative protein abundance was estimated using spectral counting. Proteins of interest were selected using the following criteria: (1) ≥ 5 spectral

The calcium-sensing receptor promotes urinary acidification to prevent nephrolithiasis.

Hypercalciuria increases the risk for urolithiasis, but renal adaptive mechanisms reduce this risk. For example, transient receptor potential vanilloid 5 knockout (TPRV5(-/-)) mice lack kidney stones despite urinary calcium (Ca(2+)) wasting and hyperphosphaturia, perhaps as a result of their significant polyuria and urinary acidification. Here, we investigated the mechanisms linking hypercalciuria with these adaptive mechanisms. Exposure of dissected mouse outer medullary collecting ducts to high (5.0 mM) extracellular Ca(2+) stimulated H(+)-ATPase activity. In TRPV5(-/-) mice, activation of t

Evaluation of nephrolithiasis in autosomal dominant polycystic kidney disease patients.

BACKGROUND AND OBJECTIVES: Nephrolithiasis (LIT) is more prevalent in patients with autosomal dominant polycystic kidney disease (ADPKD) than in the general population. Renal ultrasonography may underdetect renal stones because of difficulties imposed by parenchymal and/or cyst wall calcifications. DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: A total of 125 patients with ADPKD underwent ultrasonography and unenhanced computed tomography (CT) scan, routine blood chemistry, and spot and 24-h urine collections. RESULTS: CT scan detected calculi in 32 patients, including 20 whose previous ultr

NPT2a gene variation in calcium nephrolithiasis with renal phosphate leak.

A decrease in renal phosphate reabsorption with mild hypophosphatemia (phosphate leak) is found in some hypercalciuric stone-formers. The NPT2a gene encodes a sodium-phosphate cotransporter, located in the proximal tubule, responsible for reclaiming most of the filtered phosphate load in a rate-limiting manner. To determine whether genetic variation of the NPT2a gene is associated with phosphate leak and hypercalciuria in a cohort of 98 pedigrees with multiple hypercalciuric stone-formers, we sequenced the entire cDNA coding region of 28 probands, whose tubular reabsorption of phosphate normal

[Dent's disease: hereditary nephrolithiasis related to defective tubular endocytosis processes].

Dent's disease, a X-linked hypercalciuric nephrolithiasis, is caused by mutations of the CLCN5 gene. The disease is characterised by low molecular weight proteinuria with variable presence of hypercalciuria, hyperphosphaturia, nephrocalcinosis, and kidney stones. CLCN5 encodes a chloride channel belonging to the voltage-gated chloride channel family, which is predominantly expressed in the endosomes of proximal tubular cells. By shunting the current of electrogenic H+-ATPase, ClC-5 is crucial for efficient acidification of renal endosomes. As shown in knock-out mouse models, the ClC-5 loss of

Nephrolithiasis by Corynebacterium urealyticum infection: literature review and case report.

INTRODUCTION: Alkaline-encrusted cystitis or pyelonephritis is a chronic inflammatory condition due to deposition of crystals usually caused by urea-splitting bacteria. Corynebacterium urealyticum (CU) is a gram-positive, urea-splitting and multi-antibiotic resistant bacillus with special tropism for the urinary tract and it is often associated with encrusted pyelocystitis. METHODS AND RESULTS: Here we report the case of a 75-year-old Caucasian man with a prolonged history of renal stones who was admitted for gross hematuria associated with renal failure. A diagnosis of encrusted pyelocystiti

Recurrent Pyelonephritis in Quadriplegia: Rethinking Oral Cefdinir in the Presence of Nephrolithiasis.

Patients with cerebral palsy who develop functional quadriplegia are at an increased risk for recurrent urinary tract infections and kidney stones. Impaired bladder emptying, chronic constipation, and reduced physical mobility collectively promote urinary stasis and bacterial growth. Despite the frequency of these complications, there is little guidance in the currently available literature on how to select appropriate empiric oral antibiotics for pyelonephritis in this patient population, especially when renal stones are also present. This case highlights the importance of careful antibiotic

Chest radiographic findings suggestive of possible sarcoidosis in patients with nephrolithiasis.

Renal involvement in sarcoidosis may manifest as hypercalcemia, hypercalciuria, nephrolithiasis, nephrocalcinosis, and other abnormalities, but is often underrecognized. This study investigated chest radiographic findings suggestive of possible sarcoidosis in patients with nephrolithiasis and compared available laboratory parameters between patients with and without such findings. This retrospective analytical cross-sectional study included patients undergoing PCNL or TUL for urinary stones at Razi Hospital (Guilan, Iran) from 2016 to 2021. Routine preoperative chest X-rays were reviewed; 1297

Imaging trends and surgical outcomes for nephrolithiasis in the pediatric emergency department.

OBJECTIVES: There has been a marked rise in the incidence of pediatric nephrolithiasis over the past two decades. Although computed tomography (CT) is the gold standard for the diagnosis of nephrolithiasis, efforts to reduce minimize childhood exposure to ionizing radiation remain important. This study aims to assess current imaging trends and rates of surgical intervention for nephrolithiasis at the hospital level. METHODS: A multicenter, cross-sectional analysis was conducted using the Pediatric Health Information System, which included children treated in the emergency department (ED) and

Giant pyonephrosis related to nephrolithiasis in diabetes woman: A case report.

Pyelonephritis is one of the main systemic bacterial infections encountered in emergency departments. We present a case of diabetes woman aged 30 years referred to our urology department of El-Idrissi Hospital, Kenitra (Morocco) for recurrent episodes of urinary tract infection, multiple urolithiasis, chills, unilateral lower back pain, chills and severe hydroureteronephrosis. Abdominal CT showed a non-functioning obstructed kidney with pyelic and ureteral stones. Nephroureterectomy was performed by extraperitoneal nephrectomy for avoiding any more extended nephrectomy incision or second iliac

Nephrolithiasis in two patients on iron chelation therapy: A case report.

Drug-induced nephrolithiasis can arise from insoluble components within medications or crystallization of metabolites due to changes in metabolism and urinary pH. The connection between drugs utilized for iron chelation therapy (ICT) and nephrolithiasis is not well understood. In this report, we describe two pediatric patients diagnosed with nephrolithiasis while undergoing treatment with the chelating agents deferasirox, deferiprone, and deferoxamine for iron overload secondary to repeat blood transfusion.

Incidence of nephrolithiasis after bladder augmentation in people with spina bifida.

INTRODUCTION: Risks of nephrolithiasis after bladder augmentation in people with spina bifida (SB) remain unclear. Annual incidence of nephrolithiasis in the general population is 0.01% for 10-14 years old, 0.07% for 15-19 years old and 0.2% for 20-24 years old. Our aim was to assess the incidence and risk factors of nephrolithiasis in SB patients after augmentation. METHODS: Patients with SB and augmentation followed at our institution were retrospectively reviewed (born ≥1972, surgery 1979-2019). Patients were screened annually with renal bladder ultrasound and abdominal radiograph. Main ou

Hypercalcemia, nephrolithiasis, and hypervitaminosis D precipitated by supplementation in a susceptible individual.

Vitamin D supplementation is common among the general public. Although generally considered safe, vitamin D supplement-induced toxicity has been reported, often in association with manufacturing or labeling errors. Additionally, selected patient populations may have a hypersensitivity to vitamin D supplementation, leading to consequences due to supraphysiologic serum 25-hydroxyvitamin D levels. A 58-y-old woman developed hypercalcemia and its sequelae while on vitamin D supplementation. Despite being vitamin D replete, a functional medicine practitioner prescribed vitamin D starting at 8000 IU
