Effectiveness of pharmacological treatments for severe agitation in real-world emergency settings: protocol of individual-participant-data network meta-analysis.

BACKGROUND: Severe psychomotor agitation and aggression often require immediate pharmacological intervention, but clear evidence-based recommendations for choosing among the multiple options are lacking. To address this gap, we plan a systematic review and individual-participant-data network meta-analysis to investigate their comparative effectiveness in real-world emergency settings with increased precision. METHODS: We will include randomized controlled trials investigating intramuscular or intravenous pharmacological interventions, as monotherapy or in combination, in adults with severe ps

Comparative effectiveness of physical activity interventions and anti-hypertensive pharmacological interventions in reducing blood pressure in people with hypertension: protocol for a systematic review and network meta-analysis.

BACKGROUND: The prevalence of hypertension is a major public health challenge. Despite it being highly preventable, hypertension is responsible for a significant proportion of global morbidity and mortality. Common methods for controlling hypertension include prescribing anti-hypertensive medication, a pharmacological approach, and increasing physical activity, a behavioural approach. In general, little is known about the comparative effectiveness of pharmacological and behavioural approaches for reducing blood pressure in hypertension. A previous network meta-analysis suggested that physical

Interventions for Menière's disease: protocol for an umbrella systematic review and a network meta-analysis.

INTRODUCTION: The large number of treatment modalities for patients diagnosed with Menière's disease (MD) complicates the selection of the best available treatment as the comparative efficacy of these interventions is not clear. We aim to identify the treatment or treatments with the highest efficacy of current pharmacological and non-pharmacological treatments for MD. METHODS AND ANALYSIS: We will identify all available systematic reviews on the treatment of MD. An online database search will be conducted in association with the UK Cochrane Centre, particularly the Ear, Nose and Throat Group

Network meta-analysis with dose-response relationships.

BACKGROUND: Network meta-analysis (NMA) is a widely used method for synthesizing evidence from multiple interventions for a medical condition. However, NMA applications typically ignore the crucial role of drug dosage on intervention effects. Traditional NMAs either consider each intervention dose as an independent node or ignore the intervention dose, which may impact heterogeneity, inconsistency, or sparsity. METHODS: This paper introduces a novel frequentist approach, termed dose-response network meta-analysis (DR-NMA), which explicitly models the dose-response relationships across multiple

Influence Analyses of "Designs" for Evaluating Inconsistency in Network Meta-Analysis.

Network meta-analysis is an evidence synthesis method for comparing the effectiveness of multiple available treatments. To justify evidence synthesis, consistency is an important assumption; however, existing methods founded on statistical testing can be substantially limited in statistical power or have several drawbacks when handling multi-arm studies. Moreover, inconsistency can be theoretically explained as design-by-treatment interactions, and the primary purpose of such analyses is to prioritize the further investigation of specific "designs" to explore sources of bias and other issues t

Editorial: A Meta-Analysis of the Treatment of Acute Mania in Youth: Why Do Atypical Antipsychotics Work Better Than Mood Stabilizers?

Randomized controlled trials (RCTs) of youth with mania are very challenging to conduct, given the low base rate of bipolar disorder (BD) and the relative rarity of mania (vs bipolar depression, which tends to be much more common). Thus, many of the RCTs are relatively small, and it may be difficult to clinically interpret results. At the same time, findings about which anti-manic medications are most effective in youth are of critical importance, both because (1) poorly treated mania can lead to substantial negative psychosocial consequences, and (2) these medications can have significant adv

Efficacy and safety of different doses of azilsartan medoxomil in patients with hypertension: A protocol of a network meta-analysis.

BACKGROUND: Hypertension is one of the most common chronic diseases and an increasingly public-health challenge worldwide. Previous meta-analyses evaluated the effects of azilsartan medoxomil compared to placebo or other antihypertensive drugs in patients with hypertension. However, it is still unclear which dose of azilsartan is optimal. This study will perform a network meta-analysis to assess the efficacy and safety of different doses of azilsartan medoxomil in patients with hypertension. METHODS: PubMed, EMBASE.com, the Cochrane library, Scopus, and Web of Science were searched from incep

Evaluating search approaches for a network meta-analysis: a case study of Epistemonikos Database of Trials, CENTRAL, and other sources.

BACKGROUND: MEDLINE, Embase, the Cochrane Central Register of Controlled Trials (CENTRAL), and reference lists are commonly used when searching randomized controlled trials (RCTs) for inclusion in a systematic review. Recently, a new database for RCTs, Epistemonikos Database of Trials (EDT), was launched in a pilot stage. This case study aimed to compare EDT, MEDLINE, Embase, CENTRAL, reference list checking, and database combinations for the identification of RCTs about pharmacological migraine treatments and to assess the impact of missed trials on network meta-analyses (NMA). METHODS: We a

Methods of cervical ripening in induction of labour: an individual participant data network meta-analysis of randomised controlled trials (CIRCLE-NMA) study protocol.

INTRODUCTION: Induction of labour (IOL) is a commonly performed obstetric intervention, particularly when delivery is deemed more beneficial than continuing the pregnancy due to maternal or fetal indications. When the cervix is unfavourable for delivery, cervical ripening is performed prior to IOL. A wide variety of mechanical, pharmacological and combination methods are used, but the optimal approach balancing efficacy, safety and patient experience remains uncertain. Conventional aggregate data (AD) meta-analyses lack individual-level data, limiting exploration of patient-level factors for p

Comparative effectiveness and safety of biliary drainage strategies for malignant biliary obstruction: a protocol for systematic review and network meta-analysis.

INTRODUCTION: Malignant biliary obstruction (MBO) is a common complication of pancreaticobiliary and periampullary malignancies and is associated with substantial morbidity and reduced survival. Multiple biliary drainage strategies are available, including endoscopic, percutaneous and surgical approaches, as well as adjunctive therapies. However, direct head-to-head comparisons remain limited, and the relative effectiveness and safety of these interventions are not well established. This study aims to compare the effectiveness and safety of different biliary drainage strategies for MBO using a

Direct oral anticoagulants for stroke prevention in patients with atrial fibrillation: A network meta-analysis of randomized trials.

BACKGROUND: Direct oral anticoagulants (DOACs) are increasingly preferred over vitamin K antagonists (VKAs) for stroke prevention in atrial fibrillation (AF), including valvular (VAF) and non-valvular (NVAF). This network meta-analysis aimed to evaluate and compare the efficacy and safety of different DOACs versus VKAs in patients with AF. METHODS: A Bayesian network meta-analysis was conducted to estimate odds ratios (ORs) with 95 % credible intervals (CrIs). RESULTS: 43 RCTs were included (30 VAF, 13 NVAF). Dabigatran (OR 0.77, 95 % CrI 0.68-0.87) demonstrated the strongest reduction in is
