The Swedish health registries were used to investigate a possible effect on the incidence of neural tube defects (NTDs) of maternal smoking and maternal body mass index (BMI) (kg/m2). Among 1,199,701 infants born in 1983-1993 with known smoking exposure in early pregnancy, 621 infants with NTDs were selected. The maternal vitamin status, especially of folate, is involved in the pathogenesis of neural-tube defects (NTDs). Maternal folate administration can prevent these malformations. During December 1987 to December 1990 the incidence of neural tube defects in the Asir region located in southwestern Saudi Arabia was 0.82/1000. These results were drawn from a survey conducted of all cases of neural tube defects referred to the main hospital at Abha City (Asir Central Hospital) (ACH). Anencephaly was seen in 0.43/1000 births, spina bifida cystica 0.33/1000, and encephalocele 0.08/1000 births. Forty-two babies were found to have neural tube defects during a 3-year prospective study in a large cosmopolitan West African city. An incidence of 7/1000 deliveries was noted. The commonest defect was meningomyelocele, in 45 per cent of cases. Anencephaly patients are of renewed interest because they are regarded as a potential source of organ donation. Twenty-six newborns with spina bifida had complete spinal ultrasound (US) prior to initial surgical repair of the neural tube defects. Associated anomalies were found on screening US in seven infants and included diastematomyelia (two cases), hydromyelia (two cases), and dural fat deposits (three cases). Initial surgical therapy was altered in one infant with diastematomyelia. Four closed simple meningoceles showed no internal neural elements on US and none were found at surgery.

Genetic counselling is recommended for parents of children with neural tube defects (NTDs) to inform them of the recurrence risks and the option of prenatal diagnosis. We report a clinical analysis of 327 cases of Spina Bifida and 102 cases of Anencephaly in Brittany. Maternal age, parity, birth weight are not different from random population. Male rate is 0.40 for Anencephaly and 0.50 for Spina Bifida. The study of pregnancies shows an increased frequency of hydramnios during anencephaly pregnancies (0.34) and high rate of abnormalities during the pregnancy of Spina Bifida (0.34) or Anencephaly (0.86). Four mothers of Spina Bifida were taking valproic acid. Thirty children (0.07) had other malformations.

The prevalence and distribution of 991 cases of neural tube defects who were born in Utah in 1940-1979 are analyzed. The average prevalence at birth is 1.00 per 1000 births (live and stillbirths), which is similar to other states in the western United States. A decline in prevalence is observed during the past 10 years, although there is not a long-term secular trend for the whole time period. Examination of hospital records shows that 17.9% of neural tube defects were not reported on birth or fetal death certificates. The male/female sex ratio is 0.67. Copper-bearing intrauterine devices are being increasingly used for contraception. A growing number of pregnancies, which may subsequently carry to term, are therefore being exposed to high levels of copper at the critical period of organogenesis, with risk of possible teratogenesis.
