The history of the neurophysiology and neurology of the parietal lobe.

The development and change of knowledge on the function of the parietal lobe from the second half of the 19th century to the early 1970s are reviewed. Motor and somatosensory functions were initially localized in a broad frontoparietal region. At the beginning of the 20th century the motor cortex was restricted to the posterior frontal lobe. The separate attribution of somatosensory functions to the parietal lobe was initially based on anatomic considerations, but mostly on localized bodily sensations elicited by electric stimulation in awake patients. Patients and nonhuman primates with anter

Characterization of corticobulbar pharyngeal neurophysiology in dysphagic patients with Parkinson's disease.

BACKGROUND & AIMS: Dysphagia in patients with Parkinson's disease, persisting despite dopaminergic treatment, affects intake of nutrients and medication, and reduces quality of life (QOL). We investigated the neurophysiologic mechanisms that contribute to dysphagia in these patients, on and off L-3,4-dihydroxyphenylalanine (levodopa), using transcranial magnetic stimulation. METHODS: We studied 26 patients with Parkinson's disease (age, 65 ± 9 y; 10 men). Dysphagia and QOL were first assessed with qualitative questionnaires. Twelve hours after patients were taken off levodopa, they underwent

Hypoxic-ischaemic brain injury: imaging and neurophysiology abnormalities related to outcome.

BACKGROUND: The outcome for patients with hypoxic-ischaemic brain injury (HIBI) is often poor. It is important to establish an accurate prognosis as soon as possible after the insult to guide management. Clinical assessment is not reliable and ancillary investigations, particularly imaging and EEG, are needed to understand the severity of brain injury and the likely outcome. METHODS: We undertook a retrospective study of 39 patients on an intensive therapy unit (ITU) with HIBI who were referred for MRI. The patients were seen consecutively >57 months. HIBI was due to a variety of insults caus

Neuroimaging and clinical neurophysiology in cluster headache and trigeminal autonomic cephalalgias.

Clinical neurophysiology and neuroimaging are two non-invasive approaches used to investigate the pathophysiological basis of primary headaches, including cluster headache (CH) and other trigeminal autonomic cephalalgias (TACs). Modern neuroimaging has revolutionized our understanding of the pathophysiology of primary headaches, and of TACs in particular, focusing on a cerebrovascular dysfunction hypothesis toward a central triggering cause. The introduction of single-photon emission computed tomography (SPECT), positron emission tomography (PET), and voxel-based morphometry has allowed us new

Neuroimaging and neurophysiology of periodic lateralized epileptiform discharges: observations and hypotheses.

PURPOSE: We assessed neuroimaging lesion type and distribution in patients with periodic lateralized epileptiform discharges (PLEDs), with a view to identifying electrographic differences between PLEDs associated with differing lesion locations. Our observations led us to consider a conceptual synthesis between PLEDs and periodic complexes (PCs). METHODS: Retrospective review of acute neuroimaging results (CT/MRI) on patients identified to have EEG PLEDs, for the period 1999-2003 (n=106). Blinded classification of original EEG recordings. RESULTS: Neuroimaging abnormalities were classified a

Intraoperative neurophysiology testing of the recurrent laryngeal nerve: plaudits and pitfalls.

BACKGROUND: Electrode-imbedded endotracheal tubes allow continuous intraoperative assessment of vocal cord function when connected to an electromyographic (EMG) response monitor. Whether this device enhances or hinders the identification and preservation of the recurrent laryngeal nerve (RLN) is unclear. METHODS: The utility of continuous intraoperative neurophysiology testing (INT) of RLNs was evaluated prospectively in 100 patients undergoing 103 thyroid or parathyroid operations, involving 185 RLNs. The initial experience with 93 RLNs was compared with the subsequent 92 RLNs. RESULTS: Ove

Neurophysiological Effects of Bitopertin in Schizophrenia.

PURPOSE/BACKGROUND: Deficits in N-methyl-D-aspartate receptor (NMDAR) function contribute to symptoms and cognitive dysfunction in schizophrenia and are associated with impaired generation of event-related potential measures including auditory mismatch negativity. Parallel studies of the NMDAR agonist D-serine have suggested that sensitivity of these measures to glutamate-based interventions is related to symptomatic and cognitive response. Bitopertin is a selective inhibitor of glycine transport. This study investigates effects of bitopertin on NMDAR-related event-related potential deficits i

Towards a neurophysiological signature for fibromyalgia.

Patients with fibromyalgia (FM) show characteristically enhanced unpleasantness to painful and nonpainful sensations accompanied by altered neural responses. The diagnostic potential of such neural alterations, including their sensitivity and specificity to FM (vs healthy controls) is unknown. We identify a brain signature that characterizes FM central pathophysiology at the neural systems level. We included 37 patients with FM and 35 matched healthy controls, and analyzed functional magnetic resonance imaging responses to (1) painful pressure and (2) nonpainful multisensory (visual-auditory-t

Neurophysiologic intraoperative monitoring in pediatrics.

Neurophysiologic intraoperative monitoring, using somatosensory, brainstem auditory, and visual evoked potentials, transcranial electric motor stimulation, and electromyography, is typically used during complex surgeries involving the motor and sensory cortex, brainstem, cranial nerves, spinal cord, nerve root, peripheral roots, brachial plexus, lumbar plexus, and peripheral nerves. The particular type of surgery and the neurologic structures at risk determine the type of monitoring chosen. Although many methods are the same in adult and pediatric patients, some differences in the pediatric po

[Neurophysiological methods in headache diagnosis].

Neurophysiological methods used in the diagnosis of headache, especially migraine are: electroencephalography (EEG), evoked cortical potentials (VEP, BAER, ERP), reflex responses, autonomic tests and transcranial magnetic stimulation (TMS). Interpretation of EEG can be important for the differential diagnosis of some disorders with headache as a presenting symptom. Noninvasiveness, accessibility and ability to repeat the test due to exposure to harmful ionization are the main advantages of EEG. The role of thorough medical history and clinical assessment in patients with headache should not be

Neurophysiological evidence of methylmercury neurotoxicity.

BACKGROUND: A variety of neurophysiological measures are useful in hospital settings for diagnostic and other clinical purposes. Previously, abnormal changes in various sensory evoked potentials (EPs), and heart rate variability (HRV) were observed in patients with acquired and fetal Minamata disease (MD; methylmercury poisoning). In recent years, some of these methods have been used for the risk assessment of low-level methylmercury exposure in asymptomatic children. The objectives of this article were to present an overview of neurophysiological findings involved in methylmercury neurotoxici

Neurophysiological assessment of craniofacial pain.

This review deals with the diagnostic usefulness of neurophysiological testing in patients with craniofacial pain. Neurophysiological testing of trigeminal nerve function relies on trigeminal reflexes and laser-evoked potentials (LEPs). This review briefly describes the physiology of trigeminal reflexes and LEPs, reports normal values and highlights the neurophysiological abnormalities in the main clinical conditions.

[Neurophysiological studies of the neuromuscular junction].

INTRODUCTION: The role played by neurophysiological studies (NPS) in the diagnosis of diseases affecting neuromuscular transmission (NMT) is based on the study of the failure of muscle fibres to achieve a sufficient degree of depolarisation for the junction potential to reach the appropriate threshold and attain a muscular action potential. This totally or partially blocked impulse will give rise to different types of responses in neurophysiological tests. AIMS: To analyse the different NPS as diagnostic methods in diseases that affect NMT. DEVELOPMENT: The article offers a review of the con

[Neurophysiological investigations of respiratory function].

Neurophysiological assessment of breathing is mandatory for the diagnosis and management of respiratory problems occurring in neuromuscular disorders (neuromuscular complications of critical illness, spinal cord trauma, C4-C5 radiculopathies, phrenic nerve neuropathy, diabetes mellitus, Guillain-Barré syndrome etc...). We review the methods of exploration (principles and normative data) of efferent pathways (phrenic nerve conduction, central motor conduction studies with transcranial magnetic stimulation, diaphragmatic EMG) and afferent pathways (somatosensory evoked potentials of the phrenic

[Neurophysiological testing in myasthenia syndromes].

Myasthenic syndromes are a heterogeneous group of congenital or acquired disorders of neuromuscular junction. Despite major advance in genetics and molecular biology of disorders of neuromuscular junction, clinical diagnosis and choice of treatment largely depends on results of neurophysiological tests. Different protocols of repetitive nerve stimulation and single fibre EMG are indispensable in confirming neuromuscular junction defect, they can also give additional information on the level of abnormality and differentiate myasthenia gravis from Lambert-Eaton syndrome (pre- or postsynaptic def

Neurophysiological changes in Japanese encephalitis.

Japanese encephalitis (JE) is an encephalomyelitis involving cortex, subcortex, brainstem and spinal cord. There is paucity of studies on the neurophysiological evaluation in JE. This study aims at comprehensive evaluation of EEG, sensory and motor evoked potentials, nerve conduction and electromyography; and correlate these with clinical findings. Sixty five patients with JE diagnosed on the basis of clinical, radiological and virological criteria were subjected to a detailed clinical evaluation during the acute stage of illness. Cranial CT scan or MRI was carried out in all the patients. All

Neurophysiological characterization of parkinsonian syndromes.

Electrophysiological examination can provide relevant information on functional abnormalities in patients with parkinsonism. The combined use of various electrodiagnostic techniques can contribute to the diagnosis of the illness, to its correct classification and differentiation from other diseases with a clinically similar presentation, and in particular the identification of the pathophysiological processes underlying some of the signs and symptoms characterizing the movement disorder. Tests which are useful in the differential diagnosis of various parkinsonian syndromes can now be performed
