The most common breast complaints are nipple discharge, breast pain and a breast mass. These problems can be evaluated in a step-wise manner to determine whether the etiology is cancer. A breast mass is the most common symptom of cancer, but breast pain and nipple discharge also must be evaluated carefully. The history and physical examination, needle aspiration, mammography, ultrasound examination and biopsy are useful in determining the underlying cause of the problem.

BACKGROUND: Painless bloody nipple discharge is often classified as pathological due to its association with malignant lesions. However, it can also be a completely harmless condition. Rusty pipe syndrome is a rare cause of benign, self-limiting bloody nipple discharge during late pregnancy and early lactation. Given that rusty pipe syndrome is not described in conventional textbooks, we thought it would be appropriate to bring this benign disease to the notice of readers.
CASE PRESENTATION: A 31-year-old G1P1 female delivered an infant with a birth weight of 3000 g via cesarean section at 39

Nipple discharge is a common symptom and frequently results from benign tumors. However, there is a 5-30% risk of malignancy. A 65-year-old woman presented at the hospital because of bloody nipple discharge in her right breast. She had noticed an abnormal nipple discharge for several months. Mammography showed focal asymmetric densities without calcification in the middle outer quadrant of her right breast. Ultrasonography indicated a 1.5×1.1 cm sized cyst with fluid-fluid level. Breast MRI showed a simple cyst with a benign contrast enhancement pattern. No malignant cells were observed by fin

PURPOSE: Nipple secretions are protein-rich and a potential source of breast cancer biomarkers for breast cancer screening. Previous studies of specific proteins have shown limited correlation with clinicopathological features. Our aim, in this pilot study, was to investigate the intra- and interpatient protein composition of nipple secretions and the implications for their use as liquid biopsies.
EXPERIMENTAL DESIGN: Matched pairs of nipple discharge/nipple aspirate fluid (NAF, n = 15) were characterized for physicochemical properties and SDS-PAGE. Four pairs were selected for semiquantitati

The nipple-areolar complex (NAC), a unique anatomic structure of the breast, encompasses the terminal intramammary ducts and skin appendages. Several benign and malignant diseases can arise within the NAC. As several conditions have overlapping symptoms and imaging findings, understanding the distinctive nipple anatomy, as well as the clinical and imaging features of each NAC disease process, is essential. A multimodality imaging approach is optimal in the presence or absence of clinical symptoms. The authors review the ductal anatomy and anomalies, including congenital abnormalities and nippl

INTRODUCTION: Bloody nipple discharge is an infrequent symptom during childhood. The most common cause in this population is mammary duct ectasia (MDE), which is a benign and self-limiting condition, that is characterized by dilatation of the mammary ducts, fibrosis and periductal inflammation.
OBJECTIVE: Report of a case of MDE in order to improve physicians' diagnosis accuracy and avoid aggressive studies and treatments.
CASE REPORT: Six-months old male healthy infant, exclusively breastfeeded, that visited our clinic with a lump beneath his right nipple and bloody discharge from the same

Nipple adenoma is a rare proliferative lesion that originates from the lactiferous ducts of the nipple. Though it is benign, the typical presentation includes suspicious symptoms-a firm nodule, crusting erosion, and/or discharge from the nipple. These findings can raise concern for malignancy and in particular, Paget's disease. We report two cases of this uncommon entity, highlighting the variable clinical presentation and keys to the diagnostic evaluation and management.

Ductography of the breast is an underused procedure that often helps define the cause of unilateral, single-pore, spontaneous nipple discharge. Since nipple discharge may be caused by benign tumors, such as papillomas, or by carcinoma, such as ductal carcinoma in situ, identification of intraductal abnormalities with ductography is important. Further, diagnostic ductography and preoperative ductography help guide accurate surgical intervention. Without prior ductography, central duct excision may not result in removal of the abnormal ductal tissue or may result in removal of only a portion of

Because of the singular anatomic structure of the nipple, some breast lesions only occur at this site. The overlying skin includes normal Toker cells near the duct orifices. These cells are occasionally so numerous as to be called Toker cell hyperplasia. Ductal carcinoma in situ (DCIS) may involve nipple skin by direct extension from the underlying ducts (Paget disease of the nipple). The numerous skin appendages (eg, sebaceous, apocrine, and eccrine glands(1)) in the nipple and areola are the likely origin of syringomatous adenomas. At the duct orifices, the normal squamous epithelium dips in

Because of the unique anatomic structure of the nipple, a few specific breast lesions occur only at this site. Large lactiferous sinuses may be involved by inflammatory conditions such as squamous metaplasia of lactiferous ducts and ductal ectasia or be the site of uncommon superficial epithelial neoplasms such as nipple adenoma or syringomatous tumor of the nipple. Paget disease of the nipple may be secondary to intraepidermal extension of ductal carcinoma in situ in the underlying lactiferous ducts or develop from malignant transformation of Toker cells. Invasive breast cancer may also arise

Though fibroadenoma is the most common benign tumor of the breast and is more common under the age of 30, giant fibroadenoma is rare representing less than 4% of all fibroadenomas. A 12 years old girl presented with rapidly enlarging well-circumscribed firm, non-tender mass in right breast for 2 months which was painless and with no history of trauma, nipple discharge, fever, anorexia, weight loss or axillary lymphadenopathy. There was no family history of neoplasms. Clinically, the lump was about 12 × 12 cm and not fixed to skin or underlying structures with the absence of nipple retraction o

A palpable mass, mastalgia, and nipple discharge are common breast symptoms for which patients seek medical attention. Patients should be evaluated initially with a detailed clinical history and physical examination. Most women presenting with a breast mass will require imaging and further workup to exclude cancer. Diagnostic mammography is usually the imaging study of choice, but ultrasonography is more sensitive in women younger than 30 years. Any suspicious mass that is detected on physical examination, mammography, or ultrasonography should be biopsied. Biopsy options include fine-needle a

The most common breast problems for which women consult a physician are breast pain, nipple discharge and a palpable mass. Most women with these complaints have benign breast disease. Breast pain alone is rarely a presenting symptom of cancer, and imaging studies should be reserved for use in women who fall within usual screening guidelines. A nipple discharge can be characterized as physiologic or pathologic based on the findings of the history and physical examination. A pathologic discharge is an indication for terminal duct excision. A dominant breast mass requires histologic diagnosis. A

Nipple adenoma is a rare benign tumor arising from the lactiferous ducts of the nipple and may clinically mimic malignant breast conditions such as Paget's disease. Accurate diagnosis is often challenging, and histopathological confirmation is essential. A 37-year-old woman presented with a 10-month history of a red tumor on the right nipple accompanied by erosion and bloody nipple discharge. Imaging studies revealed no definite causative lesion; however, a subtle hypoechoic lesion within the nipple was noted on ultrasonography. A punch biopsy demonstrated benign proliferative glandular struct

Intraductal papillomas (IPs) are tumors arising from the ductal epithelial cells of the breast. Common symptoms associated with IPs include serous or serosanguineous nipple discharge and palpable masses. We present a case involving a 59-year-old female patient who experienced papillary lesions and nipple discharge for 20 years. Diagnostic imaging for this patient included mammography, conventional ultrasound, and contrast-enhanced ultrasound, which revealed an irregular and poorly defined mass at the right nipple. The ultrasound findings raised the possibility of Paget's disease of the breast,

Paget disease of the nipple is a rare presentation of breast cancer. It is sometimes mistaken for a benign skin condition of the nipple and surrounding areola, which can significantly delay the diagnosis and treatment. In over 90% of cases there is an underlying carcinoma, either ductal carcinoma in situ (DCIS) or invasive breast cancer. We present a rare case of Paget disease of the nipple without the typical characteristics, detected by simple nipple scraping technique. A 62-year-old woman presented with a small fissure on the left nipple with scant clear nipple discharge. There were no othe

The authors report their experience in the galactographic study of 95 women with nipple discharge; 65 of them previously underwent both surgical and histological control. The galactographic features are classified according to predominant alterations: ductal ectasia, ductal stenosis, ductal amputation, solitary/multiple filling defects, ductal dystrophy, and irregular distribution of ductal branches. Histopathological findings (65 patients) appear to be related to nipple discharge types: thus, hematic secretion results pathognomonic for intracanalicular neoplasm (24/25), while serous and serou
