Nurse Practitioner Roles and Contributions in a Mobile Health Clinic for People Experiencing Homelessness.

AIM: To present a descriptive, practice-based account of a nurse practitioner-led mobile health clinic serving individuals experiencing homelessness, guided by the PRIME-NP framework. BACKGROUND: Homelessness is associated with health inequities and limited access to care. Individuals face barriers including transportation, stigma, and displacement. Although mobile outreach may help address these barriers, little work has examined competencies for nurse practitioners in unstable, resource-limited environments. In this clinic, nurse practitioners provide field-based care, including wound manag

Advancing Clinical Skills for the Nurse Practitioner Student: A Screen-Based Simulation Approach.

BACKGROUND: Effective clinical documentation is essential to safe patient care, making early training critical for nurse practitioner (NP) students. This project implemented an innovative instructional approach using three Screen-Based Simulation (SBS) cases to introduce history-taking and documentation skills prior to students' first clinical rotation. METHOD: A longitudinal single-cohort design was used, and students completed surveys at two points: immediately after completing the SBS cases and again following their initial clinical rotation. RESULTS: Quiz performance showed small improve

Nurse practitioner-led hospital vascular access service: A quality improvement initiative.

INTRODUCTION: Internal quantitative assessments demonstrated that existing vascular access services (VASs) for the insertion of peripherally inserted central catheters were associated with delays in therapy initiation and hospital discharge, as well as heterogeneous complication rates. OBJECTIVES: Evidence suggests that more structured services may improve these outcomes. The aim of this quality improvement initiative was to implement and evaluate a nurse practitioner (NP)-led, hospital-wide VAS to standardise the insertion of peripherally inserted central venous catheters. METHODS: After co

The facilitators and barriers of nurse practitioner primary care practice ownership: A qualitative analysis.

BACKGROUND: The United States faces a growing primary care shortage driven by an aging population, rising chronic disease burden and a physician supply that is lagging behind demand. Nurse practitioners (NPs) are playing an increasingly central role in primary care delivery, and a small but growing number are opening independent practices. Despite their potential to strengthen the primary care safety net, empirical research on NP practice ownership remains limited. PURPOSE: Examine the facilitators and barriers to opening and sustaining NP-owned practices, particularly the structural factors

Innovative Scenario-Based Child Abuse Training for Doctoral Nurse Practitioner Students.

Background: Child maltreatment is a serious public health issue with long-lasting consequences that extend beyond immediate harm to the child. Despite substantial evidence highlighting its profound impact on individuals and society, formal training on recognizing and responding to child abuse remains limited in graduate nursing education, particularly in nurse practitioner (NP) programs. Objective: This study aimed to evaluate the effectiveness of an in-person structured evidence-informed child sexual abuse training session in enhancing NP students' ability to recognize, respond to, and manage

Feasibility of Nurse Practitioner Led Vaginal Dilation Therapy: A Retrospective Brief Report.

STUDY OBJECTIVE: to determine feasibility of nurse practitioner (NP) led vaginal dilator therapy as an alternative to pediatric gynecologist-led therapy and create an evidence-based model of care. METHODS: This is a retrospective, brief report. Electronic medical records were searched using ICD codes for vaginal and uterine agenesis. Patients were included who had participated in vaginal dilator therapy led by a pediatric gynecologist or by an NP in the Access Clinic at BC Women's Hospital. The primary outcomes were patient satisfaction, change in vaginal length, and sexual function throughou

Integration of point-of-care ultrasound in a nurse practitioner-led heart failure clinic.

BACKGROUND: Despite improved survival, one in five patients with heart failure (HF) is readmitted within 30 days of discharge. Assessing fluid status is challenging, with an estimated 50% accuracy when relying on physical examination alone. Pulmonary congestion is a risk factor for readmission and can manifest weeks before symptoms occur. PURPOSE: To conduct a pilot study to assess for pleural effusion with point-of-care ultrasound (POCUS) and to evaluate inferior vena cava (IVC) noncollapsibility as a marker of volume status. METHODS: A convenience sample of adults with HF ( n = 21) were se

Expanding role of the breast advanced nurse practitioner: mainstream pathway for genetic testing.

Genomic panel testing is increasingly being undertaken in the breast specialty to enhance diagnosis, treatment and surveillance recommendations. With increasing numbers of patients becoming eligible for testing, it is useful for advanced nurse practitioners (ANPs) and clinical nurse specialists (CNSs) who possess a relevant skill set to develop their abilities to include genomic mainstream panel testing counselling and to create a service to provide this. Within the breast specialty, ANPs and CNSs who undertake family history assessment as part of their role are perfectly placed to lead on thi

Nurse Practitioner Practice Regulations and Psychotropic Medication Dispensing for Medicaid-Insured Adults.

This study assessed the impact of Maryland's Nurse Practitioner Full Practice Authority Act of 2015 on psychotropic medication dispensing for Medicaid-insured adults treated by nurse practitioners (NPs) and physicians. A piecewise regression analysis of 168,509 Medicaid enrollees found that after this policy implementation, individuals were more likely to receive newly dispensed psychotropic medications from psychiatric-mental health NPs and from other NPs without a psychiatric specialty and were less likely to receive these medications from psychiatrists and from other medical doctors. Indivi

Description of a Nurse Practitioner-Led Orthogeriatric Model of Care: A Health Record Review.

Older adults often present with multiple comorbidities and face significant postoperative complications. This study aimed to describe the role of Nurse Practitioner (NP)-led orthogeriatric services in managing hip fracture patients. We conducted a review of health records of older adults with hip and proximal femoral fractures between July 2017 and June 2018, presenting descriptive statistics on patient characteristics, surgical outcomes, and the involvement of orthogeriatric services. A total of 197 participants were included, with a majority being female (n = 132; 67.0%). Most patients (53.8

Nurse Practitioner Care Compared with Primary Care or Nephrologist Care in Early CKD.

BACKGROUND: Early interventions in CKD have been shown to improve health outcomes; however, gaps in access to nephrology care remain common. Nurse practitioners can improve access to care; however, the quality and outcomes of nurse practitioner care for CKD are uncertain. METHODS: In this propensity score-matched cohort study, patients with CKD meeting criteria for nurse practitioner care were matched 1:1 on their propensity scores for ( 1 ) nurse practitioner care versus primary care alone and ( 2 ) nurse practitioner versus nephrologist care. Processes of care were measured within 1 year af

Barriers and enablers to nurse practitioner implementation of medication abortion in Canada: A qualitative study.

In this study we explored nurse practitioner-provided medication abortion in Canada and identified barriers and enablers to uptake and implementation. Between 2020-2021, we conducted 43 semi-structured interviews with 20 healthcare stakeholders and 23 nurse practitioners who both provided and did not provide medication abortion. Data were analyzed using interpretive description. We identified five overarching themes: 1) Access and use of ultrasound for gestational dating; 2) Advertising and anonymity of services; 3) Abortion as specialized or primary care; 4) Location and proximity to services

Nurse practitioner scope of practice and the prevention of foot complications in rural diabetes patients.

PURPOSE: Diabetic foot ulcers afflict a quarter of type-2 diabetes mellitus patients and are associated with higher mortality rates among people with diabetes. Routine primary and preventive care is essential to both prevent and treat foot ulcers before they can contribute to further adverse outcomes. One approach for expanding this care to people with diabetes in rural communities is increasing the practice authority of nurse practitioners. This study examines whether the presence of nurse practitioner practice authority is associated with fewer foot ulcer complications in rural populations a

Physician and Nurse Practitioner Attitudes on Generic Prescribing of Oral Contraceptive Pills and Antidepressants.

IMPORTANCE: As prescription drug costs rise, it is important to understand attitudes among primary care physicians and nurse practitioners (NPs) towards generic drugs. OBJECTIVE: We aimed to examine the generic skepticism index (GSI) among primary care clinicians, and their willingness to discuss and prescribe generic antidepressants (ADs) and generic oral contraceptives (OCPs). DESIGN: We used a factorial vignette design survey to test 4 factors: message source, message, brand preference, and drug class. Participants were randomized to different combinations of factors. SETTING: This was a

Abortion-care education in Japanese nurse practitioner and midwifery programs: a national survey.

BACKGROUND: While various reports have been published concerning ethical dilemmas in nursing and midwifery, and while many nurses and midwives struggle with the conflict between personal feelings raised by abortion and the duties of their position, few studies investigate the extent and conditions of abortion-care education for registered nurses (RNs) and certified nurse-midwives (CNMs) in Japan. OBJECTIVES: To describe Japanese abortion-care education programs and to investigate program directors' or other relevant persons' perceptions of abortion-care education. DESIGN: Descriptive study w

A national survey of nurse practitioner chlamydia knowledge and treatment practices of female patients.

Chlamydia is the most prevalent sexually transmitted disease in the United States, with over 4 million new infections presenting each year. The disease presents disproportionate problems for women who are generally asymptomatic; chlamydia is more difficult to diagnose in women than men. Untreated infections in women evolve into serious reproductive tract sequelae, including pelvic inflammatory disease, chronic pain, infertility, ectopic pregnancy, and neonatal complications. The bright side to this epidemic is that these sequelae are preventable. Chlamydia is both treatable and easily cured wh

The role of the nurse practitioner in the diagnosis and management of bulimia nervosa, Part 1: Diagnosis.

Bulimia nervosa is a complex syndrome that is usually difficult to diagnose and treat. Its secretive nature and minimal physical symptoms impede detection, while its chronicity impedes management. As a primary care provider, the nurse practitioner must be able to assess and intervene with these challenging clients. This series of articles will allow the nurse practitioner to develop a foundation for caring for clients with bulimia nervosa.

Finding the Sweet Spot: Exploring Nurse Practitioner Job Satisfaction, Burnout and Resilience.

Nurse practitioners (NPs) report concerning levels of burnout that can negatively impact patient outcomes and job turnover. Little is known about how NPs effectively navigate their practice and mitigate worklife stressors. Using a constructivist grounded theory approach, this study explored this gap by investigating burnout, job satisfaction and resilience among 10 hospital-based NPs. Data analysis revealed three key themes: (1) making a difference … keeps me going; (2) finding the sweet spot; and (3) being the safety net. Promoting a healthy work environment requires a targeted plan that invo
