Treatment of obturator neuralgia with laparoscopic neurolysis.

PURPOSE: Obturator neuralgia is a cause of pelvic and perineal pain that is rarely suggested, probably because it is poorly understood. We report the results of a pilot study of laparoscopic treatment for obturator neuralgia. MATERIALS AND METHODS: Seven patients (8 nerves) with obturator neuralgia were treated at our department. The etiology was idiopathic in 3 cases and iatrogenic following inguinal hernia surgery in 3 and TVT(R) repair in 2. The diagnosis was based on allodynic pain of the anterior and internal surface of the thigh, which was associated with limping with a sidestepping gai

The anatomy of Trans-Obturator Posterior Anal Sling (TOPAS) and dynamics of potential mechanism of action.

BACKGROUND: The aim of this study was to investigate the course of the transobturator posterior anal sling and its relationship to anatomical structures. METHODS: The transobturator anal sling procedure was performed in four fresh-frozen pelvises. The pelvises were dissected and the structures adjacent to the sling and the course of the sling were identified and measurements obtained. RESULTS: The transobturator posterior anal sling was inserted 2 ± 0.5 cm posteriorly to the anus, and 2.5 ± 0.5 cm caudal to the coccyx under the levator plate at the level of the puborectalis muscle. The tape

The parasacral sciatic nerve block.

BACKGROUND AND OBJECTIVES: The clinical utility of a new parasacral approach for conduction block of the sciatic nerve was investigated, with critical examination of onset, extent, and success rates when this block was used for surgical procedures below the knee. METHODS: Thirty ASA I-III patients presenting for surgery on the lower limb were enrolled. All received 30 mL of 1.5% lidocaine with 1:200,000 epinephrine following nerve stimulator identification of the sciatic nerve at < or =0.2 mA or less. Trans-sartorial saphenous nerve blocks were performed to provide anesthesia to the medial le

Intrapartum obturator neuropathy diagnosed after cesarean delivery.

Several postpartum neurologic injuries have been described in detail, while obturator nerve injuries are rarely reported. We report a woman who had weakness of the right leg and groin pain after cesarean delivery under general anesthesia. Obturator neuropathy was confirmed by electromyography and no compressive lesion of the nerve was seen on magnetic resonance imaging. The patient was treated conservatively and followed until she recovered fully.

Exostoses in obturator foramen - an osteological study.

BACKGROUND: The obturator foramen is an oval or round opening in the lateral pelvic bone and is normally closed by the obturator membrane, a connective tissue membrane. The foramen is considered to be an important anatomical structure, as it serves as the attachment site for the internal and external obturator muscles and allows passage for the obturator vessels and nerves through the obturator canal. Exostoses, also known as bone spurs, are bony growths that can occur on the surfaces of bones. Since the prevalence of exostoses in the obturator foramen is unknown, the aim of our study was to a

Laparoscopic resection of a dumbbell-shaped lipoma extending through the obturator foramen: A case report.

Lipomas are often lightweight and small in size with few subjective symptoms. Giant lipomas are uncommon. We herein report a case involving a giant, deep-seated dumbbell-shaped intermuscular lipoma in the right thigh that extended into the pelvic region through the obturator foramen and caused obturator neuropathy. A 64-year-old man with numbness while walking was diagnosed with a 17 × 16-cm lipomatous tumor. He underwent radical surgery performed by a multidisciplinary team consisting of orthopedic and colorectal surgeons. High-definition magnified laparoscopic images of the deep pelvis confi

Management of obturator nevre injury during pelvic lymph node dissection.

OBJECTIVE: Obturator nerve injuries may be seen during pelvic lymph node dissection in oncological surgery and although not common it is an important complication. According to the shape and location of the injury, tingling and loss of sensation may develop on the inner surface of the leg, together with loss of motor function of the adductor muscles. In this study an evaluation was made of these complications encountered in our clinic and the management strategies applied to these patients. MATERIAL AND METHODS: The data were retrospectively reviewed of 843 patients who underwent open radical

Safety and Feasibility of the Plantar Portal Technique in the Surgical Management of Plantar Fasciitis: A Cadaveric Study.

INTRODUCTION: This study investigates the proximity of the portals and nearby motor and sensory nerves in endoscopic release of the plantar fascia and the safety and efficacy of the addition of a novel third (plantar) portal to improve access to the calcaneal spur. METHODS: Nine fresh-frozen lower extremity cadavers were examined in this study. In addition to the conventional medial and lateral portals for endoscopic plantar fascia release, a third (plantar) portal was utilized to debride the calcaneal spur area. The fascia was released with the use of an obturator cannula. Each cadaver was c

[The fat body of the obturator canal].

In addition to the obturator nerve and vessels a fat body in the obturator canal is found in the majority of cases investigated. The fat body, small and isolated in newborns, is part of the subperitoneal mass of fat and loose areolar tissue in adults. In the 2 cases of obturator hernia the fat body has no connection with the peritoneum which constitutes the sac of the hernia. A strong fibrous cord running from the urinary bladder to the fat body of the obturator canal is observed in 46 cases. In 5 of these the cord causes a small pouch in the wall of the bladder.

The internal obturator muscle may cause sciatic pain.

Six patients suspected to have piriformis syndrome were operated in the hip region in an attempt to relieve pressure on the sciatic nerve. The piriformis muscle and tendon as well as their relationship to the sciatic nerve were found to be normal. However, the internal obturator muscle was found to be very tense, slightly hyperaemic and pressing the sciatic nerve. During Lasegue's testing on the operating table the internal obturator and not the piriformis muscle impinged on the nerve at an early stage in the hip flexion movement. A sectioning of the tendon to the internal obturator muscle nea

Continuous pericapsular nerve group blockade as analgesia for fracture of the posterior column and wall of the acetabulum; a case report and description of infusion regimen for extending analgesic effect to the distal femoral area.

The use of peripheral nerve blockade for hip surgeries has proved to be beneficial. The PEricapsular Nerve Group block is a new technique described for hip fracture and hip arthroplasty that has shown to provide better analgesia compared to other peripheral blocks commonly performed for this type of surgery. This technique blocks the obturator nerve and the articular branches of the femoral nerve and the accessory obturator nerve. There are few reports describing continuous analgesia using catheters inserted in the pericapsular nerve group area. We describe a case of continuous nerve block for

Modified Urethral Graciloplasty Cross-Innervated by the Pudendal Nerve for Postprostatectomy Urinary Incontinence: Cadaveric Simulation Surgery and a Clinical Case Report.

An artificial sphincter implanted in the bulbous urethra to treat severe postprostatectomy urinary incontinence is effective, but embedding-associated complications can occur. We assessed the feasibility, efficacy, and safety of urethral graciloplasty cross-innervated by the pudendal nerve. A simulation surgery on three male fresh cadavers was performed. Both ends of the gracilis muscle were isolated only on its vascular pedicle with proximal end of the obturator nerve severed and transferred to the perineum. We examined whether the gracilis muscle could be wrapped around the bulbous urethra a

Transobturator surgery for female stress incontinence: a comparative anatomical study of outside-in vs inside-out techniques.

OBJECTIVE To assess the specific risks of injury to neural and vascular structures inherent in two approaches to transobturator surgery for inserting a suburethral sling, i.e. the outside-in (standard technique) and inside-out approaches. MATERIALS AND METHODS The study comprised seven cadavers, providing 14 obturator regions. Five specimens had a tape inserted outside-in on one side, and inside-out on the other; of the remaining two cadavers, one had an inside-out tape and one an outside-in tape, bilaterally. After tape insertion, the cadavers were dissected. Particular attention was paid to

Management of persistent groin pain after transobturator slings.

Prolonged groin pain after transobturator tape is uncommon. Three women reported groin pain that had not improved by 3 months postoperatively. Combined steroid and local anesthetic was effective for pain relief in all patients. The differential diagnosis of persistent groin pain after transobturator tape includes adductor muscle strain, osteitis pubis, obturator/groin abscess, structural adhesions, and inflammation, edema or nerve entrapment of the anterior branch of the obturator nerve. No side effects of treatment were noted. Patients that do not respond to local injection may require mesh d

Outcomes of the Modified Stoppa Approach in Acetabular Fracture Management: Incidence of Nerve Injuries.

OBJECTIVES: Displaced acetabular fractures are complex injuries that necessitate precise surgical intervention. Obturator nerve injuries occur in approximately 2% of cases. The modified Stoppa approach, offering enhanced exposure of the quadrilateral plate, has gained attraction as an alternative technique for anterior acetabular fractures. However, its proximity to the obturator nerve poses a risk of iatrogenic injury. This study aimed to investigate the incidence of nerve injuries and functional outcomes in patients undergoing the modified Stoppa approach for traumatic acetabular fractures.
