Clinical evaluation of 3D printed splint in the treatment of temporomandibular disorders.

OBJECTIVE: To use a T-scan (Tekscan, Boston, USA) to evaluate the grinding effect of a three-dimensional (3D) printed occlusal splint and provide reference guidelines for adjusting occlusion and alleviating temporomandibular joint symptoms before orthodontic treatment. METHODS: Eighteen patients with temporomandibular disorder (TMD) were treated with 3D-printed stable occlusal plates for three months. The occlusal time (OT) of the dentition and the difference percentage of occlusal force between the left and right sides (Asymmetry index of occlusal force, AOF) were observed one week, one, two

A pilot study on spatial changes in the maxilla caused by osteopathic therapy.

OBJECTIVES: A variety of theories on the pathogenesis of temporomandibular disorders (TMD) exists resulting in treatment approaches ranging from the fabrication of occlusal splints to alternative treatment modalities such as osteopathy. The goal of this pilot study was to investigate whether osteopathic treatment causes spatial changes in the maxilla. METHOD AND MATERIALS: Following ethics commission approval and informed patient consent, three patients diagnosed with TMD participated in this investigation. In addition to regular treatment, an individualized mandibular occlusal splint was fab

Changes in cortical activation in craniomandibular disorders during splint therapy - a single subject fMRI study.

There is some controversial discussion within the therapy of craniomandibular disorders (CMDs) about the mode of action of occlusal splints. Here we present a case report on one CMD-patient measuring cerebral activation changes with functional magnetic resonance imaging (fMRI) before and after therapy with a stabilization splint. Wearing the Michigan splint for 11 nights and partially days resulted in substantial pain relief and changes in occlusal movement performance. Cerebral activation during occlusion was decreased after therapy (PRE-POST) in bilateral sensorimotor regions but also additi

An observational study of the effects of using an high oral splint on pain control.

Increasing occlusal vertical dimension (OVD) by means of oral splints is a practice widely used in the treatment of temporomandibular disorders (TMD), specifically myofascial pain, although the results are still uncertain. Oral splints with a vertical height that significantly exceeds the clinical rest position are considered by some researchers to be a better therapeutic solution in alleviating TMD symptoms than are “low” splints. In our observational study, 21 patients suffering from myofascial pain were examined for the effects of wearing a “high” oral splint while sleeping for 3 months. To

A computer model of the periodontal ligament space in man.

An engineering model of the periodontal ligament fibres was used to calculate length changes in the fibres with: direct occlusal forces on the tooth, occlusal loads on other teeth in the arch and splinting. The fibre elongations appear to influence tooth eruption, the clinical behaviour of splinted and non-splinted teeth, and the establishment of vertical dimension of occlusion and occlusal plane height. The model also provides insight into the controversy surrounding the intermediate plexus of the ligament.

Surgical management of oral submucous fibrosis in an edentulous patient: A procedural challenge.

Prosthetic rehabilitation of patients with oral submucous fibrosis (OSMF) offers a formidable challenge due to restricted mouth opening. Physiotherapy via interocclusal force application is the mainstay for surgical treatment. Herein, we report a case of a man with OSMF requiring construction of dentures. The main objective was to deliver intra-operative and post operative inter occlusal forces without fracturing the mandible and thus providing rehabilitation to the patient prosthetically. An edentulous male patient reported with grade IVA OSMF to our department to improve his mouth opening fo

The Use of a CAD/CAM Thermoformed Splints System in Closed Reduction of Condylar Fractures.

(1) Background: Mandibular fractures are very common. Common indications of closed treatment for mandibular fractures are non-displaced or minimally displaced simple fractures in adult compliant patients with good dentition, the absence of occlusal disruption, and fractures in growing children. In closed treatment, the mandible is maintained in centric occlusion with a maxillomandibular fixation (MMF) with orthodontic elastics. Many methods of MMF have been described, often using orthodontic appliances. In recent years, CAD-CAM technology has improved many procedures used in maxillofacial surg

The effect of specially designed and managed occlusal devices on patient symptoms and pain: a cohort study.

There is limited data regarding the benefit of using an occlusal device to help patients resolve a variety of symptoms involving temporomandibular disorder, as well as head, neck, and shoulder pain. The purpose of this study was to evaluate the effect of a carefully adjusted occlusal device on 12 symptoms to determine if there was enough evidence to justify a randomized control trial of this methodology. Splints were designed to ensure a stable, reproducible, mandibular position in a cohort of 157 dental patients with mixed histories of the following 12 symptoms: temporomandibular joint "pop,"

Technological Advances and Adjuncts to Optimize Outcomes in Craniomaxillofacial Trauma.

Complex maxillofacial trauma is a challenging problem for even the most experienced reconstructive surgeon. However, advances in presurgical planning, utilization of intraoperative technology, occlusal splints, three dimensional (3D) printing, and creation of custom plates can aid in simplifying challenging cases, decreasing operative time, and optimizing patient outcomes. However, there are limitations to the use of these innovations that surgeons must be aware of. This article will discuss technological advances in maxillofacial trauma.

Splintless orthognathic surgery: a novel technique using patient-specific implants (PSI).

In the past few years, advances in three-dimensional imaging have conducted to breakthrough in the diagnosis, treatment planning and result assessment in orthognathic surgery. Hereby error-prone and time-consuming planning steps, like model surgery and transfer of the face bow, can be eluded. Numerous positioning devices, in order to transfer the three-dimensional treatment plan to the intraoperative site, have been described. Nevertheless the use of positioning devices and intraoperative splints are failure-prone and time-consuming steps, which have to be performed during the operation and du

Randomized clinical trial of treatment for TMJ disc displacement.

Of the various conservative treatment modalities available for temporomandibular disorders, we believe that therapeutic exercise has a good prognosis, especially for anterior disc displacement without reduction. Since its effectiveness has not been extensively evaluated, we conducted a comparative study to verify the hypothesis that treatment efficacy would not differ for exercise and occlusal splints. Fifty-two individuals with anterior disc displacement without reduction were randomly assigned to a splint or a joint mobilization self-exercise treatment group. Four outcome variables were eval

Pain Catastrophizing and Functional Activation During Occlusion in TMD Patients-An Interventional Study.

In temporomandibular disorder (TMD), the effects of standard interventions such as using an occlusal splint and its impact on pain relief and pain catastrophizing are poorly understood. Earlier work pointed to a crucial role of insula activation with changes in pain relief by occlusal splint treatment. We performed a functional imaging study using specially developed splint systems to allow for a placebo-controlled longitudinal design. Using functional MRI we examined 20 TMD patients during repetitive occlusal movements at baseline and over the course of splint therapy and also collected self-
