[Olfactory dysfunctions. Epidemiology and therapy in Germany, Austria and Switzerland].

INTRODUCTION: Olfactory dysfunction has been reported to affect more than 200,000 patients a year in the USA. The aim of this survey was to obtain comparable epidemiological data and treatment information on olfactory dysfunction in German speaking countries. METHODS: Questionnaires were sent to all otorhinolaryngology departments in Germany, Austria and Switzerland; 52% of hospitals completed the survey. RESULTS: An average of 46 patients with olfactory dysfunction were treated per hospital every month. Hyp- and anosmia were most commonly caused by inflammatory diseases of the nose/paranasa

COVID-19 related olfactory dysfunction.

PURPOSE OF REVIEW: This article reviews the literature on COVID-19 related anosmia, focusing on the epidemiology, pathophysiology recovery rates, current available treatment options, and research regarding novel treatments. RECENT FINDINGS: Loss of sense of smell is one of the most prevalent symptoms reported by patients after COVID-19 infection. Even though there is a high self-reported recovery rate, recent studies have demonstrated that up to 7% of the patients remain anosmic more than 12 months after onset, leaving millions worldwide with severe olfactory dysfunction. Olfactory training r

COVID-19 and anosmia: The story so far.

OBJECTIVES: The pandemic has affected over 182 million coronavirus disease 2019 (COVID-19) cases worldwide. Accumulated evidence indicates that anosmia is one of the significant characteristics of COVID-19 with a high prevalence. However, many aspects of COVID-19-induced anosmia are still far from being fully understood. The purpose of this review is to summarize recent developments in COVID-19-induced anosmia to increase awareness of the condition. METHODS: A literature search was carried out using the PubMed, Embase, Web of Science, and Scopus. We reviewed the latest literature on COVID-19-

Failure to track a stable AMOC state under rapid climate change.

Persistent olfactory dysfunction (OD) is the second most common symptom of post coronavirus disease-19 (COVID-19) now being termed long-COVID. Its prevalence after recovery from COVID-19 is estimated to be 12% after nearly 6 months of follow-up. It thus becomes imperative for the treating clinicians to update themselves with the pathophysiology of this post COVID disability as well as the tools for diagnosis and the available treatment options. A systematic literature search was performed as per PRISMA guidelines in MEDLINE, Cochrane Library, LILACS, Google Scholar, ClinicalTrials.gov, and med

Long-term self-reported symptoms and psychophysical tests in COVID-19 subjects experiencing persistent olfactory dysfunction: a 4-year follow-up study.

BACKGROUND: Since the onset of the COVID-19 pandemic, chemosensory dysfunction (CD), including olfactory and taste quantitative dysfunction (OD/TD), has emerged as a prevalent and early symptom in SARS-CoV-2-infected subjects. This study explores the prevalence, duration, and recovery trajectory of COVID-19-related olfactory dysfunction (C19OD), with a specific focus on the four-year follow-up. METHODS: Using a combination of psychophysical tests (Sniffin' sticks) and patient-reported outcome measures (sVAS and tVAS), 83 participants were prospectively evaluated for OD and parosmia. Factors i

A tool for the psychophysical assessment of olfactory dysfunction in patients with COVID-19.

Numerous studies have reported that patients with coronavirus disease 2019 (COVID-19) experience alterations in the sense of smell. Therefore, there is an increased need for a psychophysical evaluation tool for olfactory dysfunction in patients with COVID-19 without increasing the risk of virus spread. We applied a single-use sniffing-bead system, which consisted of small beads and a disposable plastic handpiece, to patients with COVID-19 presenting with olfactory dysfunction. The bead sets contained eight concentrations of 2-phenylethyl alcohol (PEA), which has been used in olfactory function

Regenerating smell in neurodegenerative disease -translating theory into therapy.

PURPOSE: Quantitative and qualitative olfactory dysfunction are one of the earliest and most prevalent symptoms across neurodegenerative diseases, notably Parkinson's and Alzheimer's disease. These pathologies may spread bidirectionally between the peripheral olfactory system and central brain regions, supporting a model in which the olfactory system represents both an early marker and a conduit for disease propagation. This short communication aims to investigate the potential of Platelet Rich Plasma (PRP) to treat smell alteration in the neurodegenerative diseases. METHOD: We conducted a re

The use of platelet-rich plasma in treatment of olfactory dysfunction: A pilot study.

BACKGROUND: Olfactory dysfunction is a prevalent problem with a significant impact on quality of life and increased mortality. Limited effective therapies exist. Platelet-rich plasma (PRP) is an autologous biologic product with anti-inflammatory and neuroprotective effects. This novel pilot study evaluated the role of PRP on olfactory neuroregeneration in patients with hyposmia. METHODS: Seven patients who had olfactory loss greater than 6 months in duration, no evidence of sinonasal inflammatory disease, and no improvement with olfactory training and budesonide topical rinses were enrolled i

In vivo visualization of olfactory pathophysiology induced by intranasal cadmium instillation in mice.

Intranasal exposure to cadmium has been related to olfactory dysfunction in humans and to nasal epithelial damage and altered odorant-guided behavior in rodent models. The pathophysiology underlying these deficits has not been fully elucidated. Here we use optical imaging techniques to visualize odorant-evoked neurotransmitter release from the olfactory nerve into the brain's olfactory bulbs in vivo in mice. Intranasal cadmium chloride instillations reduced this sensory activity by up to 91% in a dose-dependent manner. In the olfactory bulbs, afferents from the olfactory epithelium could be qu

Orthodromic synaptic activation of rat olfactory bulb mitral cells in isolated slices.

Axons of olfactory receptor neurons terminate in the glomerular layer of the olfactory bulb, where they synapse with the apical dendrites of mitral cells. Although the mitral cell and its excitation by the olfactory nerve have been the subject of numerous experimental investigations, in vitro studies of these neurons have primarily used nonmammalian preparations. We have recorded the responses of rat olfactory bulb mitral cells to stimulation of the olfactory nerve layer in vitro using extracellular and whole cell patch techniques. Olfactory bulbs were cut into 400-microns thick slices in appr

Evaluation of intranasal zinc sulfate treatment on olfactory discrimination in rats.

The effects of intranasal syringing with 5% zinc sulfate were assessed in rats trained in a wind-tunnel olfactometer to detect the presence of an odor or to discriminate odors of graded intensities. The syringing produced a severe but brief interference in olfactory performance. Full recovery of detection performance occurred within 72 hr after treatment. Initial recovery of odor intensity discrimination occurred in 2-8 days and full recovery within 8-10 days. These results suggest that intranasal syringing with ZnSO4 is not an adequate control for the effects of anosmia produced by olfactory

Sudden sensorineural olfactory loss: a structured narrative review and proposal for a standardised terminological framework.

BACKGROUND: Sudden loss of smell - particularly when sensorineural in origin - is a clinically disabling condition that the medical community has never adequately named. Patients presenting with acute olfactory loss encounter a field without shared definitions, dedicated ICD codes, or guidelines built around their specific presentation. The consequence is diagnostic delay, terminological inconsistency in the literature, and a research base that cannot be meaningfully synthesised. OBJECTIVE: This paper reviews the evidence on sudden sensorineural olfactory loss, demonstrates that existing term

Interventions for the prevention of persistent post-COVID-19 olfactory dysfunction.

BACKGROUND: Loss of olfactory function is well recognised as a cardinal symptom of COVID-19 infection, and the ongoing pandemic has resulted in a large number of affected individuals with abnormalities in their sense of smell. For many, the condition is temporary and resolves within two to four weeks. However, in a significant minority the symptoms persist. At present, it is not known whether early intervention with any form of treatment (such as medication or olfactory training) can promote recovery and prevent persisting olfactory disturbance.  OBJECTIVES: To assess the effects (benefits and

Management of Post-Infectious Anosmia and Hyposmia: A Systematic Review.

BACKGROUND: Anosmia and hyposmia significantly affect patients' quality of life and have many etiologies, including trauma, inflammatory conditions including chronic rhinosinusitis, neoplasm, and viral infections, such as rhinovirus and SARS-CoV-2. OBJECTIVE: Our purpose was to establish whether a consensus exists regarding optimal management of olfactory dysfunction and to provide insight into the treatment of anosmia in the current climate of increased prevalence secondary to COVID-19. Thus, we aimed to systematically review the literature on the management of non-Chronic-rhinosinusitis- re

Congruency of multisensory olfactory stimuli.

We perceive our environment via different sensory channels in a multisensory fashion. During multisensory integration, these channels can enhance or hinder each other depending on congruency. Here, we aimed to investigate how the congruency of gustatory and visual costimulation alter the perception of olfactory stimuli. We hypothesized that congruent costimulation enhances the perception of likeness, i.e. how well stimuli match a label, compared with no and to incongruent costimulation. We also aimed to quantify the effect of gustatory and visual costimulation. We tested 48 healthy young parti

Clinical Olfactory Working Group consensus statement on the treatment of postinfectious olfactory dysfunction.

BACKGROUND: Respiratory tract viruses are the second most common cause of olfactory dysfunction. As we learn more about the effects of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), with the recognition that olfactory dysfunction is a key symptom of this disease process, there is a greater need than ever for evidence-based management of postinfectious olfactory dysfunction (PIOD). OBJECTIVE: Our aim was to provide an evidence-based practical guide to the management of PIOD (including post-coronavirus 2019 cases) for both primary care practitioners and hospital specialists.
