All Twenty Nails Affected: Aripiprazole-Induced Photo-Onycholysis in an 11-Year-Old Boy.

Photo-onycholysis is a phototoxic reaction characterized by separation of the nail plate following light exposure and medication use. While commonly associated with antibiotics and psoralens, newer agents have also been implicated. We present the case of an 11-year-old male who developed diffuse brown discoloration, onycholysis, and onychomadesis of all 20 nails one month after starting liquid aripiprazole. The pattern of involvement on sun-exposed digits, preference for open-toed footwear, and exclusion of other triggers supported a diagnosis of drug-induced photo-onycholysis. The patient was

Onycholysis secondary to toluene sulfonamide formaldehyde resin used in a nail hardener mimicking onychomycosis.

Two patients with allergic contact dermatitis to the toluene sulfonamide formaldehyde resin present in nail polish and nail hardener, in whom onycholysis developed, are reported herein. Previously, both patients had been unsuccessfully treated with various antifungal medications. Regrowth of all nails occurred within several months after discontinuance of the offending agents. Results of patch tests to the preparation itself, to the toluene sulfonamide formaldehyde resin, and to formaldehyde confirmed the diagnosis in both patients.

Phototoxicity, Pseudoporphyria, and Photo-onycholysis Due to Voriconazole in a Pediatric Patient With Leukemia and Invasive Aspergillosis.

Voriconazole is a triazole antifungal agent superior to amphotericin B in the treatment of invasive aspergillosis. It is generally well tolerated and has excellent oral bioavailability, providing significant benefit in the treatment of invasive fungal infections. There have been numerous reports of dermatologic reactions to this agent, including erythroderma, cheilitis, Stevens-Johnson syndrome, discoid lupus erythematosus, pseudoporphyria, squamous cell carcinoma, and photosensitivity reactions. Pseudoporphyria, a dermatologic condition mimicking porphyria cutanea tarda, has been described as

Beyond the Plate: Nail Bed Dermoscopy for Differentiation of Psoriatic, Fungal, and Traumatic Fingernail Onycholysis.

BACKGROUND: Onycholysis is a common but etiologically heterogeneous nail sign, most frequently due to psoriasis, onychomycosis, and trauma. Since the diagnosis of the underlying cause can be misleading, we investigated whether dermoscopy (onychoscopy) pre- and post-clipping of the onycholytic nail plate can provide reproducible patterns to distinguish these causes without the need for a biopsy. MATERIALS AND METHODS: In a multicenter retrospective image-based study, 60 onycholytic fingernails were examined (20 patients per diagnosis: psoriasis, onychomycosis, trauma). Onychoscopy of the intac

Small joint involvement in psoriatic arthritis is associated with onycholysis: the Reykjavik Psoriatic Arthritis Study.

OBJECTIVES: Psoriasis and psoriatic arthritis (PsA) are associated with nail changes. Recent reports suggest that nail changes may be a part of the enthesitis of PsA and that they predict the onset of arthritis among patients with psoriasis, but they have not reported on subclasses of nail changes. However, earlier reports suggested that onycholysis is the nail change most strongly associated with PsA. If nail changes in PsA are a sign of enthesitis, they might be associated with small joint disease in general and the objective of this study was to test this hypothesis. METHODS: A total of 15

Multicenter study of a frozen glove to prevent docetaxel-induced onycholysis and cutaneous toxicity of the hand.

PURPOSE: Onycholysis and skin toxicity occur in approximately 30% of patients treated with docetaxel. We investigated the efficacy and safety of an Elasto-Gel (84400 APT Cedex, Akromed, France) frozen glove (FG) for the prevention of docetaxel-induced onycholysis and skin toxicity. PATIENTS AND METHODS: Patients receiving docetaxel 75 mg/m2 alone or in combination chemotherapy were eligible for this case-control study. Each patient wore an FG for a total of 90 minutes on the right hand. The left hand was not protected and acted as the control. Onycholysis and skin toxicity were assessed at ea

Tips to treat the 5 most common nail disorders: brittle nails, onycholysis, paronychia, psoriasis, onychomycosis.

Nail disorders are difficult to treat and often frustrating both for patients and clinicians. Because of the slow growth rate of the nail plate and the difficulty of getting the drug actives to penetrate the nail tissues, it is usually necessary to wait several months before seeing the results of treatments. This delay often leads to discontinuation of therapy by the patients. This article therefore helps clinicians to find the right treatment of the 5 most common nail disorders (brittle nails, onycholysis, paronychia, psoriasis, and onychomycosis) and provides practical tips that might improv

Dermoscopy of Onycholysis Due to Nail Psoriasis, Onychomycosis and Trauma: A Cross Sectional Study in Skin of Color.

BACKGROUND: Clinical differentiation of onycholysis due to various etiologies is difficult task that compels to do invasive investigations to arrive at accurate diagnosis. Wrong diagnosis often leads to treatment failure and physicians and patient's anxiety. Dermoscopic patterns in nail psoriasis, onychomycosis are well established. Here, authors attempted to describe dermoscopic patterns in onycholysis due to psoriasis, onychomycosis and trauma in skin of color. METHODOLOGY: Study was conducted in a tertiary hospital in Southern India. Ethical clearance and informed consent from patients was

[Exudative onycholysis and acute bacterial paronychia related to BIBF-1120 and paclitaxel: response to topical therapy].

A case of a 50 years-old breast cancer patient treated with weekly paclitaxel and BIBF 1120 is reported herein. At the end of the twelfth cycle of chemotherapy, the patient developed distal onycholysis with intense hyponychium serous exudates, pain and malodor in all her fingernails. It was treated with topical fusidic acid and 1% methylprednisolone aceponate two times daily, with an excellent clinical response from the first three days of treatment. Bacterial paronychia with nail plate loss of the fifth left fingernail was observed a week after the topical therapy was started, with positive c

[The blood biochemical indicators as possible markers of destruction of kerato-hyaline tissue of nails in patients with onycholysis].

It is established that the most informative blood biochemical indicators of destructive process in patients with onycholysis are to be considered concentration of oxiproline in blood. The changing of ratio of fractions oxiproline, balance between concentration of enzymes of antioxidant defense and metalloproteinase and concentration of hemoglobin in blood make it possible to diagnose the mixed type of hypoxemia which is a possible cause of destruction of kerato-hyaline tissue in patients with onycholysis.

Application of Topical Lidocaine for Pain Relief in the Setting of Targeted Therapy-Induced Onycholysis: A Case Report.

Selective pan fibroblast growth factor receptor (FGFR) inhibitors have been linked to severe onycholysis, the uncomfortable separation of the nail plate from the nail bed. Recommendations to assist with FGFR inhibitor onycholysis vary based on the severity. We hypothesized that the application of topical lidocaine to mimic a digital nerve block would be beneficial in addition to traditional supportive care interventions and subsequently report its immediate and continued efficacy for targeted therapy-induced onycholysis.
