Intraoperative Femur Fracture Risk During Primary Direct Anterior Approach Cementless Total Hip Arthroplasty With and Without a Fracture Table.

BACKGROUND: There is no study to date comparing intraoperative femur fractures (IFFs) in the direct anterior approach (DAA) with and without a fracture table. We hypothesize that there is no significant difference in the IFF with and without a fracture table when performed by experienced DAA hip surgeons. METHODS: This study is a 1-year retrospective review of patients who underwent DAA total hip arthroplasty by 2 surgeons: one surgeon uses a flat table and manually elevates the femur with a large bone hook, while the other surgeon uses a fracture table and a mechanical femoral elevator. Primary mediastinal hydatid cyst.

An unusual case of primary mediastinal echinococcosis in a young female presented with non-specific symptoms. On examination, she had signs of Horner's syndrome and mild superior vena cava compression. She was operated successfully. The final diagnosis could only be made on the operating table and confirmed by histopathology. Hydatid cyst in mediastinum is uncommon but because of surrounding vital structures and potential of its complete cure should be explored without delay.

Hiding scrambled text messages in speech signals using a lightweight hyperchaotic map and conditional LSB mechanism.

This study presents a lightweight, secure audio steganography system for hiding text messages for transmission over the Internet, with the aim of addressing the current problems of high computational cost and insufficient security identified in earlier studies. We propose a two-phase functioning mechanism. Text characters are first transformed into ASCII code and stored in a vector, which is then divided into three sub-vectors. These sub-vectors are scrambled using two low-complexity operations, namely a forward-backward reading technique and an odd-even index. Two scrambling loops are perform

A novel approach to frontal sinus surgery: treatment algorithm revisited.

BACKGROUND: Access to the frontal sinus remains a challenging problem for the craniofacial surgeon. A wide array of techniques including minimally invasive endoscopic approaches have been described. Here we present our technique using medical modeling to gain fast and safe access for multiple indications. METHODS: Computer-aided surgery involves several distinct phases: planning, modeling, surgery, and evaluation. Computer-aided, precise cutting guides are designed preoperatively and allowed to perfectly outline and then cut the anterior table of the frontal sinus at its junction to the surro

New technique of imaging and treatment of pediatric supracondylar humeral fracture without moving the injured limb.

The management of closed, displaced supracondylar fracture of humerus in children can be divided into 3 stages. In stage 1 closed reduction is achieved whose acceptability is confirmed, on table, in stage 2 using an image intensifier. This reduction is maintained in stage 3 by immobilization in slab/cast or by percutaneous Kirschner wire fixation. Although enough literature for proper technique in stage 1 and stage 3 are available, there are certain practical problems encountered in the equally important stage 2, which needs a closer look. Most of the C-arm compatible tables have their metalli

[Direct cholecystography after laparoscopic cholecystostomy in acute cholecystitis].

Direct cholegraphy was performed after laparoscopic cholecystostomy in 82 patients. It should be conducted on the operating table if proper fixation of the draining tube is doubtful, but, mainly, on the fourth or fifth postoperative day. Double contrasting of the biliary tract with air and a contrast medium produces the best result.

Electrocution during anaesthesia.

Cardiac arrest due to electrocution occurred in a young obstetric patient undergoing laparotomy. The patient subsequently died despite initial resuscitation. The cause was traced to an inappropriate and faulty switch in the base of the operating table; the circuit was completed by the ECG monitor which was of the direct earth type.

Challenges during bilateral total temporomandibular joint replacement for ankylosis in ankylosing spondylitis patient-a case report.

Alloplastic replacement of temporomandibular joint is the preferred treatment for temporomandibular joint ankylosis (TMJA) in ankylosing spondylitis (AS) patients. These patients exhibit neck rigidity due to fixed flexion deformity or fusion of vertebrae that make the intubation and surgical positioning of patients difficult and challenging. Chin on the chest makes tracheostomy almost impossible. Fiberoptic-assisted intubation is recommended. It is mandatory that no neck flexion or rotation is performed during intubation or surgical positioning. The use of an operating table that permits later

Mini-open intramedullary nailing of acute femoral shaft fracture: reduction through a small incision without a fracture table.

BACKGROUND: Femoral shaft fractures are usually treated with intramedullary nailing. In this study, we report on a modified surgical technique with reduction through a small incision for this type of fracture. METHODS: From 1994 to 1997, this technique was used on 74 patients with 82 femoral shaft fractures. Seventy patients (76 fractures) with at least 3 years of postoperative follow-up were included for clinical evaluation. The surgical technique involves a mini-wound at the fracture site, and fracture reduction is performed with 1 finger or a bone hook without the use of a fracture table.

Management of frontal sinus fractures. Changing concepts.

Since the turn of the century, surgeons have handled frontal sinus fractures with a variety of different procedures. The optimal management procedure remains controversial. We have presented a graduated anatomic algorithm for treatment of frontal sinus fractures based on the degree of fracture displacement and nasofrontal duct involvement and presence of CSF leak. Nondisplaced fractures are best handled conservatively, without operative intervention. However, the majority of frontal sinus fractures require operative correction. Uncomplicated anterior table displacement with an aesthetic deform

Evidence-based ergonomic parameters for safer surgical practice: a systematic review.

INTRODUCTION: Work-related musculoskeletal disorders are highly prevalent among surgeons and are primarily associated with sustained non-neutral postures, inadequate visualization, improper table height, and insufficient adaptation to individual anthropometry; therefore, this systematic review aimed to synthesize evidence on ergonomic risk factors and define evidence-based parameters for safer surgical practice. METHODS: A comprehensive systematic search of multiple databases was conducted using predefined eligibility criteria, including studies evaluating surgeon anthropometry, visualization

Reamed intramedullary nailing of the femur: 551 cases.

BACKGROUND: The care of the patient with the fractured femur entails a multiple surgical team approach. Healing of the fracture and expediency in the operating room are both important. We sought to determine the results of the treatment of fractures of the femoral shaft with interlocking femoral nails inserted with closed techniques, and to compare the outcomes of fractures nailed by using a fracture table with those stabilized with the leg draped free on a radiolucent table. METHODS: Eight hundred eighty-two consecutive patients with fractures of the femoral shaft treated with a first-genera

Comparison of closed percutaneous screw reduction versus open reduction and internal fixation in the treatment of frontal sinus fractures: A retrospective study.

The aim of this study was to compare closed percutaneous screw reduction to traditional open reduction-internal fixation (OR-IF) for the treatment of anterior table fractures. Both groups were evaluated in terms of operative variables, complications, and treatment success. Of 32 patients included, 19 patients underwent OR-IF, while 13 underwent percutaneous screw reduction. The median operative time, length of hospital stays, and treatment cost of the OR-IF group were 100 min (range 60-130), 4 days (range 3-9), and $727 (range $642-$1291), respectively. The same variables for the closed reduct

Post-operative subcutaneous fat necrosis in a newborn: a case report.

Subcutaneous fat necrosis of the newborn (ScFN) is an uncommon condition of neonates and infants. The disorder is caused by generalized and local tissue hypoperfusion. The ScFN tends to improve spontaneously with or without some severe complications such as hypercalcemia. The ScFN may occur as iatrogenic after hypothermic surgical interventions. We present iatrogenic ScFN in a newborn with uncomplicated hypercalcemia due to cold exposure on operating table during at an umbilical cord hernia operation. To our knowledge, this is the first report of a patient in whom ScFN occurred during a common

Thoracoscopic surgery of the oesophagus in rats: a training concept for the treatment of tracheo-oesophageal malformations in preterm infants.

In extremely preterm babies with type Vogt III b oesophageal atresia, the primary operative management can be restricted to a gastrostomy and ligation of the fistula. Some of these patients, however, may not even tolerate a thoracotomy or placement on the operating table. We developed a minimally invasive procedure to prepare and ligate the oesophagus thoracoscopically and perform laparoscopic gastrostomy in a rat model. In 15 operations we observed only one complication. This study implies that after adequate practice, thoracoscopic dissection of the oesophagus with ligation of a tracheo-oeso

"On table" positioning for optimal access for cancer excision in the lower rectum.

Poor visualization and restricted access often make tumor lesions in the lower rectum difficult to excise, particularly in a narrow male pelvis. The aim of this study was therefore to study whether (and if so to what extent) different positions of the patient on the operating table might improve accessibility. Twenty consecutive patients (men and women) undergoing laparotomy with surgery of the lower rectum were studied. The geometric configuration of the pelvis was studied and compared on lateral radiographs obtained at the operating table in each of four positions. Compared with the conventi

Intraoperative colonic lavage and primary anastomosis in peritonitis and obstruction.

BACKGROUND: The surgical management of left colonic emergencies has evolved in the past few decades. Recently, there has been increasing interest in resection with primary anastomosis in selected cases. The aim of this study was to evaluate the differences in outcome in patients with peritonitis or obstruction treated by resection, on-table lavage and primary anastomosis of the left colon. METHODS: Between January 1992 and August 1995, 212 patients underwent emergency operation for a distal colonic lesion: 97 presented with peritonitis, 113 with obstruction and two with other indications. Congenitally corrected transposition of the great arteries plus dextrocardia operated with an unusual operative technique.

We describe a case of congenitally corrected transposition of the great arteries plus dextrocardia and normal anatomical abdominal viscera. Systemic (tricuspid) valve replacement was performed due to moderately severe valve regurgitation. An unusual operative technique was used because of the patient's rare morphology. On cardiopulmonary bypass, the systemic valve was approached via a left atriotomy anterior to the left pulmonary veins, whereby the surgeon was positioned on the patient's left side. As the surgeon had excellent exposure from the opposite side of the table, he was able to perfor

Intraoperative colonic lavage in emergency surgical treatment of left-sided large bowel lesions.

There has been a trend towards resection and primary anastomosis after on-table lavage in patients with left sided colonic emergencies. This study will analyse the outcome in our centre. Thirty-two patients underwent the operation for various lesions of the left colon and the rectum between December 1998 and December 2002. The mean age of the patients was 37.7+/-11.1 years. Mean extra time for the procedure was 37.4+/-5.2 min. The mean hospital stay was 11.2+/-2.9 days. Postoperative complications included four cases (12.5%) of wound infection, and one (3.1%) right sub-phrenic abscess. No clin
