Giant cell arteritis (GCA) is a systemic vasculitis of medium and large arteries often with ophthalmic involvement, including ischemic optic neuropathy, retinal artery occlusion, and ocular motor cranial nerve palsies. This last complication occurs in 2%-15% of patients, but typically involves only 1 cranial nerve. We present 2 patients with biopsy-proven GCA associated with multiple cranial nerve palsies.

BACKGROUND: Ophthalmic complications following inferior alveolar nerve anaesthesia are rare. They include transient blindness (amaurosis), ophthalmoplegia, ptosis, mydriasis and diplopia. These events may occur following the intravascular administration of anaesthetic solution and are distressing to both patient and operator alike.
CASE REPORT: We report the unusual case of a young patient who experienced amaurosis, total ophthalmoplegia, complete upper eyelid ptosis, mydriasis and periorbital blanching following inferior alveolar nerve anaesthesia. Similar but less profound signs were observ

PURPOSE: To evaluate the association of COVID-19 infection and vaccination with neuro-ophthalmic adverse events.
METHODS: In this nationwide population-based retrospective cohort study, 8,498,353 patients were classified into three groups: control, COVID-19 infection, and COVID-19 vaccination. We conducted separate analyses for the early phase (within 60 days) and late phases (61-180 days) to estimate the incidence rates and hazard ratio (HR) for each neuro-ophthalmic adverse event. The adverse events included in this analysis were optic neuritis, papilledema, ischemic optic neuropathy, third

We report a case with progressive visual loss whose ophthalmic examination revealed bilateral optic atrophy and choroidal folds in the left eye. Neuroimaging demonstrated a sellar mass compressing the optic chiasm and the left optic nerve was to be stretched. We assume the traction created on the optic nerve by the mass stretched the optic nerve and caused the choroidal folds.

AimCorneal confocal microscopy (CCM) is a non-invasive ophthalmic imaging technique for evaluating the corneal microstructure, particularly the subepithelial nerve plexus. This dense, dynamic plexus contains small nerve fibers from the ophthalmic branch of the trigeminal nerve. This study aimed to evaluate corneal nerve alterations in individuals with migraine, including its subtypes and visual hypersensitivity (measured with the Leiden visual sensitivity scale (L-VISS), a validated nine-item questionnaire).MethodsIn total, 55 migraine participants were included, including 26 with episodic mig

PURPOSE OF REVIEW: Optical coherence tomography (OCT) affords clinicians the ability to quantify the thickness of the retinal nerve fiber layer (RNFL), which is useful in managing diseases of the optic nerve. The purpose of this review is to coalesce the current literature on the use of OCT in neuro-ophthalmology to enhance its use in clinical practice.
RECENT FINDINGS: OCT's advancement into spectral domain refined its ability to measure the RNFL by increasing scanner speed. Although OCT was shown to be superior to other instruments in measuring the RNFL in certain conditions, it lacks laser

PURPOSE OF REVIEW: This review aims to provide an overview of powered endoscopic sinus surgery and raise awareness of the possible neuro-ophthalmic complications associated with the procedure.
RECENT FINDINGS: The neuro-ophthalmic complications of endoscopic sinus surgery can be categorized into five anatomic sites: eye, orbit, optic nerve, extraocular muscle, and lacrimal drainage system. Double vision may be caused by various ocular motility abnormalities. Visual loss may be the result of orbital hemorrhage or direct optic nerve trauma.
SUMMARY: A wide range of neuro-ophthalmic complicatio

Neurotrophic keratopathy (NK) results from loss of trigeminal innervation to the cornea, disrupting epithelial integrity, tear film regulation, and ocular surface homeostasis. Conventional treatments address the surface consequences of denervation but do not restore sensory input. Corneal neurotization (CN) directly addresses this biological deficit by transferring healthy donor axons to the anesthetic cornea, re-establishing afferent innervation, and creating the conditions for epithelial maintenance and visual rehabilitation. Since Terzis first described direct surgical CN in 2009, the field

Herpes zoster Ophthalmicus (HZO) affecting the ophthalmic division (V1) of the trigeminal nerve. HZO may cause extraocular muscle palsies, with the third nerve being the most commonly affected and the fourth nerve the least. The combined involvement of the optic nerve and isolated paralysis of the eye muscle is very rare, with only limited case reports documenting this complication of ocular herpes zoster. This case report also describes a case of optic nerve involvement with 6th nerve palsy, which was treated with corticosteroid and valacyclovir.

Sensory innervation to the eye and periocular area arises from the ophthalmic branch of the trigeminal nerve. Thus, ocular, orbital, and systemic disorders may produce head pain with ocular signs and symptoms. Whereas some of these entities have characteristic diagnostic features, others mimic primary headache disorders such as migraine and cluster headache. This article reviews common ocular and neuro-ophthalmic conditions that are accompanied by pain in or near the eye.

BACKGROUND: Ophthalmic eye blocks, such as retrobulbar, peribulbar and sub-Tenon's, are traditionally conducted "blind". Complications are rare but potentially devastating. Life-threatening complications include brain stem anesthesia and local anesthetic toxicity, whereas sight-threatening complications include globe perforation, optic nerve damage and ocular muscle damage. Ultrasound permits a view of orbital structures and can be used to guide needle placement. The ultrasound appearances of unintended local anesthetic injection into vital orbital structures have not been documented. This stu

BACKGROUND: Power drilling commonly used for anterior clinoidectomy and optic canal unroofing can result in thermal injury to the optic nerve.
METHODS: The authors describe an intradural "limited drill" technique of anterior clinoidectomy and optic canal unroofing for microneurosurgical management of ophthalmic segment aneurysms, where optic canal unroofing is done with 1 mm Kerrison punch and the use of the power drill is restricted to anterior clinoidectomy to avoid thermal injury to the optic nerve. The optic nerve, internal carotid artery (ICA), and aneurysm are covered with wet gelfoam p

BACKGROUND: There is a vast discrepancy between the incidence of skull base metastases reported in vivo and at autopsy. Asymptomatic character or unspecific symptoms make the diagnosis difficult, particularly in patients with no history of cancer. Our case illustrates a skull base metastasis from breast cancer, detected in a diagnostic process initiated by ophthalmologic examination.
CASE REPORT: We report the case of a 53-year-old woman complaining of ptosis and diplopia, with concomitant loss of skin sensation within the right half of the forehead, and without any other worrisome symptoms o

Fifty-eight of 570 patients with an aneurysm and/or subarachnoid hemorrhage displayed palsy of one or several cranial nerves. In 48 patients, this lesion occurred in the context of SAH (total n = 534). The oculomotor nerve was most frequently affected (38 cases), followed by the abducens nerve with the second-highest incidence (18 cases). The highest rate of cranial nerve symptoms occurred in cases of ophthalmic carotid aneurysms (83.3%). Damage to the abducens nerve regressed most rapidly.
