Keratoconus Management in the Community: A Survey of Optometrists.

OBJECTIVES: Defining the patterns of practice and referral criteria of optometrists within New Zealand to investigate the diagnosis and management of keratoconus. METHODS: Optometrists recruited through the New Zealand Association of Optometrists, Cornea and Contact Lens Society of New Zealand, and private practices were invited to complete an anonymous survey. RESULTS: Responses were received from 168 optometrists (representing 20.0% of the optometrist population). Half (48%) of optometrists had ≥15 years of experience, and 22% prescribed soft contact lenses daily, whereas only 6.4% prescri

Optimising subjective anterior eye grading precision.

PURPOSE: To establish the optimum grading increment which ensured parity between practitioners while maximising clinical precision. METHODS: Second year optometry students (n = 127, 19.5 ± 1.4 years, 55 % female) and qualified eye care practitioners (n = 61, 40.2 ± 14.8 years, 52 % female) had 30 s to grade each of bulbar, limbal and palpebral hyperaemia of the upper lid of 4 patients imaged live with a digital slit lamp under 16× magnification, diffuse illumination, with the image projected on a screen. The patients were presented in a randomised sequence 3 times in succession, during which

Standardized patient methodology to assess refractive error reproducibility.

PURPOSE: Standardized patient (SP) methodology is the gold standard for evaluating clinical practice. This approach was used to investigate the content of typical optometric eyecare in England and the reproducibility of refractive error measurement using prescriptions obtained by three SPs. METHODS: The three SPs were independently examined by three to four expert optometric clinicians to obtain "benchmark" estimates of refractive error. One hundred two community optometrists consented to be visited by three SPs who were trained to provide accurate responses during the examinations. The spect

Schirmer test. I. A review.

Since its introduction by Schirmer in 1903, the Schirmer test has been widely used for the assessment of the adequacy of tear production. Although the faults of the Schirmer test have been well documented, it is nevertheless still being used to assess tear production, especially in the diagnosis of keratoconjunctivitis sicca (KCS) and for screening potential contact lens wearers. We summarize the findings and conclusions of some of the previous studies.

Effect of Fresnel prism dispersion on contrast sensitivity function.

Fresnel prisms are often used in orthoptics for binocular anomalies and in low vision for visual field defects. These prisms are made of optical polyvinyl chloride (PVC) and this material increases chromatic dispersion and produces a loss of contrast. In this study, the effect of chromatic dispersion on contrast sensitivity is determined. Contrast thresholds over a wide range of spatial frequencies are obtained on a number of subjects with the use of Fresnel prisms ranging from 5 to 30 prism dioptres. As predicted there is a decrease in visual acuity and cut-off spatial frequency threshold as

Medicare program; coverage of optometrists' services--Health Care Financing Administration. Notice of proposed rulemaking.

These regulations would expand Medicare coverage of services furnished by optometrists to include examination services related to the condition of aphakia (absence of the natural lens of the eye). Under our current regulations, Medicare coverage of optometric services is limited to dispensing services in connection with the actual fitting and provision of prosthetic lenses. The regulations are necessary to conform our existing Medicare regulations to a statutory change made by section 937 of the Omnibus Reconciliation Act of 1980.

Approaching headaches and facial pains in eye care practice.

Headaches and facial pains are among the most frequent ailments seen in outpatient or emergency settings. Given the fact that some of the primary headaches and facial pains mimic the characteristic patterns seen in ocular diseases and related conditions, it is fairly common for these situations to be sent to an ophthalmology or optometry clinic and misdiagnosed as ocular headaches. This may result in a delay in starting an appropriate therapy, therefore extending the patient's illness. This review article aims to help the practitioners in understanding common causes of headaches and facial pai

Diagnostic accuracy of optic nerve OCT and ultrasound in a large paediatric cohort referred with suspected papilloedema and very low rates of raised ICP.

BACKGROUND/OBJECTIVES: The Honey Rose trial increased referrals for suspected papilloedema. We audited such referrals from community optometrists and used long-term patient outcomes to investigate the diagnostic accuracy of OCT retinal nerve fibre layer (RNFL) thickness and B scan ultrasound optic nerve sheath diameter (ONSD) for raised intracranial pressure (rICP). SUBJECTS/METHODS: Audit of 337 children seen in a 'blurry disc' clinic. Long-term outcomes (6-42 months) were used to classify children as with or without raised ICP. Receiver operating curves investigated diagnostic accuracy of R

A Pattern-Based OCT Metric for Glaucoma Detection.

PURPOSE: To develop and test a novel optical coherence tomography (OCT) metric for the detection of glaucoma based on a logistic regression model (LRM) and known patterns of glaucomatous damage. METHODS: The six variables of the LRM were based on characteristic patterns of damage seen on the OCT thickness maps of the ganglion cell layer plus inner plexiform layer (GCL+) and retinal nerve fiber layer (RNFL). Two cohorts were used to develop the LRM. The healthy cohort consisted of 400 individuals randomly selected from a real-world reference database (RW-RDB) of OCT widefield scans from 4932 e

von Hippel-Lindau disease (angiomatosis of the retina and cerebellum).

von Hippel-Lindau disease is a multi-system disorder characterized by hemangioblastic lesions of the central nervous system and visceral organs. Angiomatosis retinae is often the first observable manifestation of von Hippel-Lindau disease. Optometry may be the initial point of entry into the health care system for the von Hippel-Lindau patient. As such, the eye care practitioner should be aware of the diffuse nature of the disease and initiate the search for silent, possibly lethal, lesions associated with the disease. Management may involve consultation with a neurosurgeon, internist, and ret

The burgeoning presbyopic population: an emerging 20th century phenomenon.

Improved social welfare in the wake of the 'industrial revolution' set in train in mid-18th century Britain an escalation in population numbers which has been sustained through to the end of the 20th century--and is projected to continue into the 21st century. However, within the total population envelope the percentage-contribution of certain age groupings shows a striking pattern when viewed across nearly five centuries. For 350 years, up to the end of the 19th century, the over--40 year old section of the population comprised a steady 25% of the total population count of England (latterly E

Another joint statement regarding learning disabilities, dyslexia, and vision--a rebuttal.

Several medical organizations have published yet another joint statement trivializing vision therapy and vision disorders in the learning-disabled population. A review of the references in the joint statement as well as other references find that the joint statement is misleading because of inappropriate citations and selected references, as was the case with previous joint statements. The most current joint statement ignores the results of evidence-based research and makes recommendations regarding the treatment of convergence insufficiency that have no scientific validity. When care meets commerce: Patient resistance in optometric consultations.

We use conversation analysis to investigate clients' responses to treatment recommendations in optometric eye examinations (n = 66). As in other healthcare settings, in optometry, clients exercise their right to reject professionals' recommendations. In our analysis, it transpires that the reason for visit influences the emergence of rejections to recommendations: those who attend optometric examinations because they have noticed changes to their eyes/vision align with recommendations given by optometrists; those, however, who attend eye examinations on routine grounds resist recommendations.

Enhancing eye care training: a comprehensive rubric for retinoscopy.

CLINICAL RELEVANCE: Retinoscopy is an important ophthalmic technique for objectively assessing refractive errors and detecting ocular abnormalities, especially in patients unable to undergo subjective refraction. The development and standardisation of retinoscopy rubrics foster a more equitable and consistent learning experience for trainees, ultimately leading to improved patient care. BACKGROUND: Retinoscopy is an essential skill in ophthalmic examination. Despite the use of various rubrics in eye care, standardised rubric for retinoscopy is still unavailable. This study aims to develop and

Glaucoma Agreement in New Zealand (GAINZ).

CLINICAL RELEVANCE: With an ageing population, ophthalmologists are becoming burdened with glaucoma management, and patient care can be delayed. Therefore, the use of optometrists in glaucoma management can help alleviate the burden. BACKGROUND: The ageing population and subsequent rise of glaucoma prevalence are putting a strain on the public health system in New Zealand. Glaucoma collaborative care between optometrists and ophthalmologists has been gaining support with the aim to reduce this burden on ophthalmologists. There has been little investigation of the agreement in care and managem
