Influence of orthognathic surgery for symptoms of temporomandibular dysfunction.

OBJECTIVE: To evaluate the influence of orthognathic surgery on the clinical signs and symptoms of temporomandibular disorders (TMDs). STUDY DESIGN: In a cohort study, 54 patients undergoing orthognathic surgery were evaluated with regard to the signs and symptoms of TMDs through subjective and objective assessments. These evaluations were performed 1 week preoperatively (T1), 1 month postoperatively (T2), and 6 months postoperatively (T3). The evaluations included patient variables and surgery. Univariate analyzes were performed to verify the association of the variables (P < .05). RESULTS:

[Orthognathic surgery for patients with cleft lip and palate].

Patients with cleft lip and palate frequently develop dento-facial deformity requiring orthognatic surgery. The origin of this deformity is therapeutic and surgeons are currently trying to prevent this iatrogenicity. The maxillary dento-facial deformity in these patients is a retrognathia with infragnathia, associated with endognathia, obliquity of the occlusal plane, with deviation of the superior incisive midline in case of unilateral clefts. The difficulties in the treatment of these skeletal deformities are due to the palatal, labial, and pterygomaxillary scar tissue. Orthognathic surgery

The emotional impact of orthognathic surgery and conventional orthodontics.

Previous research by the authors has pointed to depressive reactions among orthognathic surgery patients during the fixation-removal stage and up to 9 months later. However, less is known about emotional shifts among persons who choose to undergo conventional orthodontic treatment after considering surgical orthodontics. In the current study, a standard measure of mood states was applied to 90 surgical patients and 66 who had considered surgery but decided against it. Of these, 33 were undergoing orthodontic treatment and 33 were having no treatment. The mood scale and measures of personality

Retrospective study of eligibility for orthognathic surgery using the Index of Orthognathic Functional Treatment Need (IOFTN).

To assess the functional needs of orthognathic patients who had been accepted for treatment by Christchurch Hospital, New Zealand, we made a retrospective assessment of 80 consecutive patients using the Index of Orthognathic Functional Treatment Need (IOFTN). Eligibility was based on the Severity and Outcome Index (SOI) score of ≤3 derived from seven lateral cephalometric measurements (three skeletal, three dental, and one soft tissue), which made allowances for asymmetrical or reported important functional issues. The IOFTN grades 4 or 5 indicated "great" or "very great" need for treatment, a

Orthognathic positioning system: intraoperative system to transfer virtual surgical plan to operating field during orthognathic surgery.

PURPOSE: To introduce the concept and use of an occlusal-based "orthognathic positioning system" (OPS) to be used during orthognathic surgery. MATERIALS AND METHODS: The OPS consists of intraoperative occlusal-based devices that transfer virtual surgical planning to the operating field for repositioning of the osteotomized dentoskeletal segments. The system uses detachable guides connected to an occlusal splint. An initial drilling guide is used to establish stable references or landmarks. These are drilled on the bone that will not be repositioned adjacent to the osteotomy line. After mobili

Ultrasonic surgery--an alternative way in orthognathic surgery of the mandible. A pilot study.

The aim of this report is to present preliminary results and experiences using an ultrasonic bone-cutting device in bilateral sagittal split osteotomies of the mandible (BSSRO) with particular attention to possible damages to the inferior alveolar nerve (IAN). Seven patients with class II or class III malocclusion were treated by BSSRO with a conventional combined orthognathic and surgical approach. The osteotomy was carried out using an ultrasonic bone-cutting device. Subjective neurosensory deficits of the inferior alveolar nerve were assessed on 14 sides. Compared to the conventional techni

The "Surgery-Only" Paradigm in Orthognathic Surgery: A Contemporary Analysis of Three Indications.

Orthognathic surgery is applied to correct significant skeletal discrepancies in the maxillofacial region. In recent years, increasing attention has been directed toward the Surgery-First Approach (SFA), which is believed to offer notable advantages. This approach primarily addresses underlying skeletal irregularities. Given the substantial impact of facial appearance on psychosocial well-being, the enhancement of facial aesthetics is recognized as one of the primary goals of orthognathic treatment. Within this context, the SFA has emerged as an alternative to the conventional approach due to

Skeletal stability in orthognathic surgery with the surgery first approach: a systematic review.

The surgery first approach (SFA) is a therapeutic strategy used in orthognathic surgery that is constantly evolving. With this approach, the pre-surgical orthodontic treatment can be eliminated, the maxilla and the mandible are surgically repositioned into the desired position, and the therapy is ended with a short orthodontic phase. Several studies have reported that the SFA is an acceptable approach, but postoperative stability is unclear. In this study, a systematic review on the SFA was performed. The PubMed, Google Scholar, Scopus, LexisNexis, Web of Science, and Cochrane Library database

The surgery-first approach in orthognathic surgery: a retrospective study of 50 cases.

The surgery-first approach (SFA), without presurgical orthodontic treatment, has become favoured in the treatment of dentomaxillofacial deformities. This approach has been applied in our institution since 2012. The purpose of this study was to report our experience with the SFA for skeletal malocclusion. Fifty patients with skeletal malocclusions were enrolled in this study (11 bimaxillary protrusion, 27 skeletal class III malocclusion, and 12 facial asymmetry). After orthognathic-orthodontic consultation, suitability for SFA was determined and a treatment plan drawn up. Patients then underwen

[Postoperative stability in orthognathic surgery: from predictors to challenges4].

Postoperative stability is crucial for maintaining the long-term functional and aesthetic outcomes of orthognathic surgery. What is the current state of the science concerning skeletal and soft tissue stability following orthognathic surgery, and what are the predictors, clinical manifestations of relapse and implications for clinical action? Sagittal skeletal stability is generally high and well predictable, although specific patient- and surgery-related risk factors, including progressive condylar resorption, can result in clinically relevant relapse. Evaluation of soft tissue stability is l

Secondary Inferior Alveolar Nerve Repair During Revisional Orthognathic Surgery.

Revisional orthognathic procedures are increasingly requested by patients who remain dissatisfied with facial aesthetics after primary correction of dentofacial deformities. Neurosensory disturbance of the inferior alveolar nerve (IAN) is a well-recognized complication of sagittal split ramus osteotomy (SSRO), and inadvertent IAN transection can result in severe permanent neurosensory deficits if not properly repaired. The authors report a 41-year-old woman who presented with persistent "prognathic appearance" and left lower lip hypesthesia 7 years after mandibular setback surgery using bilate

Orthognathic surgery-related nerve injuries: analysis and treatment modalities.

BACKGROUND: Orthognathic surgery, while highly effective in correcting dentofacial deformities, inherently poses a risk of neurosensory complications. Despite the growing volume of these surgical procedures, real-world data remain limited concerning the precise incidence, clinical course, and efficacy of various therapeutic interventions for nerve injuries in this specific context. This study aims to delineate the incidence, anatomical distribution, and treatment outcomes of trigeminal nerve injuries observed after orthognathic surgery within a tertiary care center. METHODS: This retrospectiv

Integrating Orthognathic Surgery Into Lip Lift for Subnasal Aesthetics.

Lip lift is a versatile procedure to shorten the upper lip and improve the morphology and proportion of the lip. It is commonly performed as part of facial rejuvenation in Western patients, whereas in Asian countries, younger people tend to desire a lip lift because a shorter upper lip is considered more attractive. The lip lift carries a risk of an unfavorable aesthetic outcome when underlying skeletal deformities, such as maxillary protrusion and vertical excess, are not addressed, resulting in an undesirable degree of dental exposure. This article described our experience of integrating ort
