Intraoperative adjustments to optimize active middle ear implant performance.

CONCLUSION: After initial contact of the active middle ear implant (AMEI) on the incus, significant increases in device performance can be achieved intraoperatively without affecting residual hearing by additional static loading of the incus with 62 μm (quarter turn) to 125 μm (half turn) increments via an adjustment screw. OBJECTIVES: To assess the performance gains of driving the incus with an AMEI under increasing static loads in cadaveric temporal bones. METHODS: Incus drive efficacy was assessed using laser Doppler velocimetry measurements of stapes velocities over a frequency range of

Hearing Results in Patients With No Ossicular Chain Reconstruction and Factors Influencing the Outcome.

INTRODUCTION: Ossiculoplasty is aimed at improving or maintaining the conductive portion of hearing in patients subjected to otologic surgery. However, satisfying hearing is frequently observed after tympanoplasty even in the absence of ossicular chain reconstruction. Aims of this article would be to evaluate hearing outcomes, in terms of pure tone average air-bone gap (PTA-ABG), in patients subjected to tympanoplasty for middle ear disease whose ossicular chain has not been reconstructed, and then to investigate factors influencing those outcomes. MATERIALS AND METHODS: Retrospective chart r

[Laser stapedotomy].

In addition to hearing aids, stapesplasty represents the standard treatment of otosclerosis-induced hearing loss. In this procedure, the stapes superstructure is replaced by a prosthesis that is attached to the long process of the incus and communicates through a perforation in the footplate with the perilymphatic space of the inner ear. The removal of the stapes superstructure and perforation of the footplate are the critical steps of this surgical procedure. With the introduction of laser-assisted perforation techniques, the surgical safety of this method has been improved compared to conven

Design-Dependent Calculation of the Prosthesis Length in Malleostapedotomy.

OBJECTIVE: The results of a recently published micro-CT study suggested a correlation of the distance between long incus process and stapes footplate and the required prosthesis length in malleostapedotomy. The goal of this study was to test the reliability of that assumption. METHODS: Rectangular and bent prostheses were tested in 11 cadaveric human temporal bone specimens; 1 of them showed a stapedial artery. Prosthesis length was calculated based on the distance between long incus process and stapes footplate. The rate of acceptable prosthesis insertion into the vestibule was investigated.

Active middle ear implants.

Active middle ear implants are surgically implanted prosthesis, which intend to stimulate the ossicular chain or the inner ear fluids through the oval or round windows. These implants may be useful for the treatment of certain patients with sensorineural hearing loss as well as for conductive or mixed hearing loss. This clinical guide attempts to summarize the current knowledge concerning the basic characteristics and indications of the most commonly used middle ear implants, including Vibrant Soundbrige (Med-el, Innsbruck), Carina (Cochlear, Australia), and CodacsTM. (Cochlear, Australia).

A New Intraoperative Real-time Monitoring System for Reconstructive Middle Ear Surgery: An Experimental and Clinical Feasibility Study.

HYPOTHESIS: Electromagnetical excitation of ossicular vibration is suitable for middle ear transmission measurements in the experimental and clinical setting. Thereby, it can be used as a real-time monitoring system for quality control in ossiculoplasty. BACKGROUND: Positioning and coupling of middle ear prosthesis are a precondition for good postoperative hearing results, but at the same time completely dependent upon the surgeon's subjective judgment during surgery. We evaluated an electromagnetically driven measurement system that enables for in vitro and in vivo transmission measurements

Extrusion and Dislocation in Titanium Middle Ear Prostheses: A Literature Review.

Titanium middle ear (ME) prostheses are widely used in surgical practice due to their acoustic properties. However, they present a significant drawback shared by all synthetic materials currently in use for ME reconstruction: they can be rejected by the organism of the host. In this study, we aim to review the current literature on titanium partial ossicular replacement prostheses (PORPs) and total ossicular replacement prostheses (TORPs) extrusion and dislocation. Eighty articles were analysed after a full article review based on the inclusion and exclusion criteria. The most common indicatio

Use of Glass Ionomer Cement for Incudostapedial Rebridging Ossiculoplasty.

INTRODUCTION: The ossicles of the middle ear are affected by the erosive effect of pathology in chronic otitis media. Ossicular reparation can be done via autologous incus or with alloplastic materials. Glass ionomer cement (GIC) is simple to use and saves considerable operative time and expenses especially in developing countries where costly ossicular prosthesis are not affordable for the majority of the patients. MATERIALS AND METHODS: Twenty-five chronic otitis media patients who underwent surgery were included in this study. The reconstruction material used in this study was glass ionome

[Use of glass ionomer cement in surgery of the incus-stapedial joint. An initial report of experiences].

Chronic inflammations of the middle ear are characterised by perforation of the tympanic membrane as well as hearing loss, due to disintegration in the incudostapedial joint. Usually the perforation is easily restored by means of available modern procedures; on the other hand, failure of the reconstruction of the ossicular chain, especially of the incudostapedial joint, is not uncommon. Hence, the improvement of hearing capacity is transitory, since later, due to scars, graft lateralisation results in a dislocation of the primarily reconstructed ossicular chain. The patient's hearing capacity

Reconstruction of the ossicular chain in the middle ear with glass ionomer cement.

OBJECTIVES: The aim of this study is to describe a method to repair small defects (1 to 2 mm) of the ossicular chain by means of glass ionomer cement (GIC) alone. Larger defects can be repaired with a combination of GIC and a platinum wire prosthesis. GIC is a two-component hybrid material consisting of inorganic glass particles surrounded by an insoluble hydrogel matrix. GIC has been used by dentists for many years, but only a few reports describe the use of GIC in human middle ear surgery. Many investigators have proved that most GICs are biocompatible, biostable, and well tolerated by bone

Transcanal tympanoplasty: a 15-year report.

The data presented would seem to support the following conclusions: 1. Transcanal tympanoplasty employing connective tissue underlay grafts, ossicular bone prostheses, and avoidance of mastoid exposure, unless irreversible disease is present, seems to meet contemporary standards of efficacy, safety, and long-term stability when performed on patients with either chronic suppurative otitis or atelectatic otitis. 2. Although chronic suppurative otitis and atelectatic otitis have differing characteristics and are of seemingly different pathogenesis, no statistical differences were found in the pat
