Gradual reduction of distal radial malunion using distraction osteogenesis.

PURPOSE: To evaluate gradual distraction lengthening or distraction osteogenesis as a technique for treating malunions of the distal radius. METHODS: Twenty patients with clinical and radiographic evidence of distal radius malunion were treated with osteotomy of the distal radius using distraction osteogenesis. At the follow-up evaluation each patient was evaluated for healing rates, complications, resolution of pain, and radiographic alignment. Surgical treatment consisted of an application of a nonbridging external fixator that could be distracted to correct the deformity. A loosely set scr

Maxillo-mandibular distraction osteogenesis for hemifacial microsomia in children.

To obviate or lessen the long period of use of orthodontic appliances after distraction for hemifacial microsomia in the mixed dentition patients, simultaneous maxillo-mandibular distraction osteogenesis has been performed in 10 patients aged 7 to 12 years with hemifacial microsomia. In these patients, there were two cases of type I, five cases of type IIa, and three cases of type IIb. A uniplanar internal distraction device was used in all cases. After maxillary Le Fort I osteotomy and mandibular ramus osteotomy, a uniplanar internal distraction device was attached to the mandible. On days 5

Determining the optimal time for consolidation after distraction osteogenesis.

The consolidation period, one of the biological factors participating in the distraction osteogenesis phenomenon, has not been elucidated. Parameters to assess new bone formation and consolidation in craniofacial distraction have been borrowed from the orthopedic experience. Up until now, the decision to discontinue distraction has been based more on personal experience than on objective data. By using Scintigraphy with Tc99 DP a method has been developed to accurately assess the termination of the consolidation phase. Scintigraphic evidence of consolidation was seen between the fourth and the

Orthodontic management of the patient undergoing mandibular distraction osteogenesis.

Distraction osteogenesis is a gradual incremental bone-lengthening technique that requires very precise treatment planning and surgical execution. Subtle variation in the position of the corticotomy/osteotomy or in the position of the distraction device will affect the ultimate position of the tooth-bearing (distal) segment. Other functional and/or anatomic factors (that at this time are not well understood) also have an influence on the direction in which the tooth-bearing segment moves during distraction. These factors combine to create a discrepancy between the planned direction and the act

Simultaneous distraction osteogenesis and microsurgical reconstruction for facial asymmetry.

Restoring facial balance in patients with severe facial asymmetry is a challenging problem for the craniofacial team. Attention to bony reconstruction as well as soft-tissue contouring is required for patients with moderate to severe deformities. Traditionally, facial skeletal reconstruction was performed with osteotomies and bone grafting. More recently, distraction osteogenesis has proven to be successful in achieving bone lengthening. For select cases, distraction osteogenesis has lessened the need for major skeletal procedures and has allowed earlier surgical intervention. The reconstructi

Distraction osteogenesis in congenital limb length discrepancy: a review.

This paper reviews the technique of callotasis used to correct limb length discrepancy due to congenital causes. Many more lower than upper limb lengthening procedures have been reported. Despite the low complication rate reported by the originators of the technique, patients undergoing limb lengthening because of congenital problems are at significant risk of pin tract sepsis, joint contracture, nerve palsies, angular deformities and fracture through the regenerate bone. A minority of studies focus specifically on limb lengthening for congenital defects. Most series simply include some congen

Reconstruction of mandibular defects following tumor ablation by vertical distraction osteogenesis using intraosseous distraction devices.

OBJECTIVES: Distraction osteogenesis has recently evolved a challenging technique to overcome major drawbacks of conventional augmentation procedures. We, therefore, report the application of miniaturized intraosseous distraction devices for the rehabilitation of mandibular defects due to ablative tumor surgery. METHODS: In 10 patients who had undergone box-shaped or segmental resections, augmentation of the residual mandibular bone or of full thickness iliac crest grafts was performed by intraosseous implant-shaped distractors. Distraction and reconsolidation was monitored by ultrasonography

Development of Multidirectional Cranial Distraction Osteogenesis for the Treatment of Craniosynostosis.

BACKGROUND: Previously, the authors developed a new method of distraction osteogenesis for the treatment of craniosynostosis, multidirectional cranial distraction osteogenesis (MCDO). The purpose of this study is to review the authors' experience of MCDO for remodeling of the anterior cranium in the patients of craniosynostosis. METHODS: Forty-five patients with craniosynostosis underwent MCDO for anterior cranial remodeling from 2003 to 2017. The ages of the patients ranged from 8 to 72 months (mean: 23.9 months, median: 13 months). The involved sutures included the bicoronal suture in 14 pa

The Use of Distraction Osteogenesis in the Treatment of Rickets-Associated Craniosynostosis.

BACKGROUND: Craniosynostosis has a known association with rickets. Because of abnormal bone development and a tendency for patients with rickets to present at an older age than most patients with craniosynostosis, repair may be complicated by inadequate cranial expansion and greater risk of sutural re-fusion. We present 2 cases of rickets-associated sagittal craniosynostosis and show the effectiveness of distraction osteogenesis in the surgical management of this condition. CASE DESCRIPTION: Two 3-year-old boys with rickets presented with sagittal synostosis and marked scaphocephalic deformit

Clinical evaluation of distraction osteogenesis in the treatment of mandibular hypoplasia.

BACKGROUND: Mandibular hypoplasia is one of the most frequently encountered craniofacial anomalies with a variety of etiologies, including congenital, developmental, and acquired. It can lead to significant functional issues at birth by creating an obstruction of the hypopharynx with the retropositioning of the base of the tongue, which lead to respiratory and feeding difficulties at birth. Later in life, mandibular hypoplasia may have a severe impact on the quality of life of the patient, affecting mastication, speech, and appearance. Distraction osteogenesis (DO) of the craniofacial skeleton

Distraction osteogenesis: role and clinical applications in the maxillofacial region.

Distraction osteogenesis or callostasis is a technique of new bone formation by gradual separation of bony fragments. The method was first developed for limb lengthening but recently this process was widely applied to the cranio-maxillofacial bones. The application included unlimited bone lengthening and reconstruction of segmental defects. Several designs of extra-oral and intra-oral distraction devices were invented to suit different areas of craniofacial bone. Nevertheless intraoral distractors have several advantages including minimal scarring and being less cumbersome. Clinical cases usin

Distractor breakage in cranial distraction osteogenesis for children with craniosynostosis.

Cranial distraction osteogenesis has been applied as a mode of therapy to patients with various types of craniosynostosis. Several minor complications during distraction have been reported in the previous literature, including infection, device exposure, and dislocation and distortion of the device. To our knowledge, this is the first report of 2 cases of spontaneous device breakage during cranial distraction osteogenesis. Two infant patients were diagnosed with shunt-induced microcephalies. The ages of the 2 patients were 8 and 12 months. Their head circumferences were 3 percentiles below tha

Lengthening of short bones by distraction osteogenesis--results and complications.

We performed bone lengthening surgery on 12 metacarpals and 14 metatarsals of 15 patients. The mean age for metacarpal and metatarsal lengthening was 14.5 (10-21) and 17.5 (10-25) years, respectively. We used a unilateral or a circular external fixator. The mean healing index of the metacarpals and metatarsals was 1.6 (1.1-2.3) and 1.6 (1.0-2.0) months/cm, respectively. The mean increase in metacarpal and metatarsal length was 17.6 (13-26) and 24.3 (20-30) mm, respectively. The functional scores of the metatarso-phalangial (MTP) joint of lengthened metatarsals for the lesser toe were excellent

Distraction osteogenesis of the cranial vault for the treatment of craniofacial synostosis.

From January 2000 to December 2001, six patients with craniosynostosis were treated. Involved sutures were coronal sutures in three patients, coronal and metopic sutures in one patient, multiple sutures (brachycephaly and oxycephaly) in one patient, and multiple sutures with a cloverleaf skull deformity in one patient. The age distribution of the patients was 4 months to 3 years. Four were male, and two were female. A frontal craniotomy was performed in four patients with brachycephaly. In one patient with brachycephaly, the osteotomies were made across the nasofrontal junction, across the roo

Treatment of condylar hypoplasia with distraction osteogenesis: a case report.

This report describes the surgical-orthodontic treatment procedures of a patient with condylar hypoplasia. Condylar hypoplasia is a major factor in any facial abnormality in a growing child. This case report describes a patient with a facial anomaly with an acquired unilateral condylar hypoplasia. His asymmetry was corrected by using functional therapy for the correction of muscle tonus and distraction osteogenesis for lengthening the mandibular ramal height. Fixed orthodontic appliances were used for conventional orthodontic therapy and final occlusal adjustment.

[Distraction osteogenesis in a newborn infant with Pierre-Robin sequence].

BACKGROUND: The prognosis for Pierre Robin sequence is often closely correlated with direct postnatal asphyxia (ARDS) and permanent cerebral damage. Unfortunately, all of the well-known treatments entail many problems, which often lead to medical care of the patient for many years. Distraction osteogenesis was recently introduced as a very promising procedure. We revised this concept for newborn children aged up to 3 months. MATERIALS AND METHODS: The first patient was operated with an extraoral distractor at an age of approx. 3 months. The second patient was operated at an age of approx. 2 m
