Although there has been a growing body of literature about bisphosphonates since 1969, it was not until 2003 that treatment with this medication was associated with osteonecrosis of the jaw. Presented herein is such a case.

Intravertebral clefts are considered markers for osteonecrosis. We tested the hypothesis that bisphosphonate therapy was associated with vertebral osteonecrosis. A retrospective review of a vertebroplasty database was performed. Exposure to, duration of, and type of bisphosphonate therapy was documented. UNLABELLED: Paget's disease of bone, although common in the long bones and pelvis, is less common in the head and neck. A 20-year follow-up is reported of a patient who was diagnosed with Paget's disease following the investigation of her initial oral complaint. Following surgical extraction of two teeth in her maxilla, the sockets have failed to heal, despite numerous attempted interventions. This case highlights both the cranio-maxillofacial and oral manifestations of Paget's disease of bone, its effects on surgical exodontia and the possible effects of bisphosphonate therapy.

Osteonecrosis is recognized increasingly as a complication of the treatment of cancer in young people, especially those with acute lymphoblastic leukemia and non-Hodgkin lymphoma, reflecting a probable pathogenetic role of steroid therapy. Children >/=10 years of age and Whites are at particular risk, and genetic predispositions have been identified. The disorder affects weight-bearing joints predominantly. It is commonly bilateral and multi-articular. Treatment options include both surgical and non-surgical interventions.

We report the PET-CT appearance of Zoledronate-related osteonecrosis of the mandible in a 54-year-old woman with right breast cancer with osseous and pulmonary metastases. Her treatment included surgery, chemotherapy and Zoledronate, and external beam radiation treatment. We present a case of idiopathic osteonecrosis of the humeral capitellum in a 44-year-old female in the absence of any associated risk factors. Arthroscopy was undertaken to remove the loose bodies and debride the capitellum, with a satisfactory outcome.

The new generation of bisphosphonates are often used in the treatment of osteoporosis or for certain tumors with bone defects.
