Surgery of otosclerosis in the elderly.

Auditory results following stapes surgery in 26 patients with otosclerosis >60 years of age at the time of surgery were compared with those obtained in 140 patients <60 years undergoing stapes surgery during the same time period. Stapedotomy was performed in all cases. The mean follow-up period was 29 months. Hearing results as judged by postoperative air-bone gaps were as good in the older age group as in the younger patients. Moreover, complications of surgery, such as postoperative formation of perilymphatic fistula, did not occur more frequently in elderly vs younger patients. On the basis

Stapes mobilization in otosclerosis.

We performed a retrospective chart study (including surgeon's notes and audiometric results) and an analysis of the archival temporal bones from a patient who had undergone surgery for stapes mobilization in both ears. The stapes footplate was submerged into the vestibule on the right (as a complication of surgery) and absent on the left. One interesting finding was that the patient's hearing had improved on the right despite the presence of the depressed footplate and that the air-bone gap had widened on the left despite the absence of complications on that side.

Genetic correlation in otosclerosis.

PURPOSE: The aim of this study was to determine the relationship of HLA typing in patients with otosclerosis. MATERIALS AND METHODS: We used a prospective study in which HLA typing of 100 surgically confirmed otosclerosis patients were compared with age and sex matched normal individuals. RESULTS: HLA-A3, HLA-A9, HLA-A11, and HLA-B13 were found to be significantly higher (P < .05, .01, and .01, respectively). HLA-A9 and HLA-A11 were found to be higher (P < .01) in patients with a positive family history, indicating genetic heterogenicity. CONCLUSIONS: Higher values of HLA-A9, HLA-11, and HL

[My adventure with otosclerosis].

The author presents his own experience in surgical treatment of otosclerosis in 373 patients. He particularly exposes results concerning hearing level after surgical procedures and discusses progress of symptoms which accompany otosclerosis like vertigo or dizziness and tinnitus in the context of bibliography data. He also looks at the complications occurring during procedures and following them. Worth noticing is presentation of long term results after 10-year follow-up.

MRI of active otosclerosis.

Our aim was to determine whether MRI reliably shows pathology in patients with active otosclerosis (otospongiosis). We studied five patients with clinical and audiometric signs of this disorder and positive findings on high-resolution CT and tympanocochlear scintigraphy. Contrast enhancement of otospongiotic lesions was found in all affected ears, and could be topographically related to demineralised otospongiotic foci on CT. In lesions in the lateral wall of the labyrinth MRI sometimes showed the pathology better than CT, where partial-volume effects could be troublesome.

[Cytogenetic study in otosclerosis].

In 50 patients (32 females and 18 males) with otosclerosis who underwent stapedectomy, a cytogenetic study of chromosomal abnormalities was made. No description of G banding in patients with otosclerosis was found in the literature. No chromosomal abnormalities were found in the karyotype using this technique.

[Pathogenetic aspects of otosclerosis].

Basic otosclerosis pathogenetic factors are reviewed with a special emphasis on heredity. The inheritance in otosclerosis is autosomal dominant with incomplete gene expression. The involvement of HLA antigens is detailed. Autoimmune origin of the disease is confirmed morphologically and immunologically in perilymph and serum tests. There are infiltrates with lymphocytes, plasmatic and mast cells in basis stapedial mucoperiostal tissue. Perilymph contained significantly elevated levels of IgG and IgM, of antibodies to native DNA and type II collagen, especially in patients with active otosclero

Hearing aids and otosclerosis.

The purpose of this article on the use of hearing aids in patients with otosclerosis is to emphasize that all otosclerosis patients cannot be assumed to be good users of hearing aid amplification. Patients with losses of 60 dB or less and with purely conductive lesions may be able to use aids well. Individuals with mixed-type impairments due to otosclerosis may have significant problems utilizing amplification. These cases demonstrate the use of hearing aid amplification in patients with mixed-type hearing impairment due to otosclerosis. This is a very different group of patients from the grou

[Otosclerosis--diagnosis and therapy].

Otosclerosis (synonym: otospongiosis) is a focal or diffuse spongifying disease of the bony labyrinth. So far pathogenesis is unknown, some recent investigations assume a paramyxovirus infection. But there are no doubts about hereditary and genetic factors, females are twice often affected as males with a maximum incidence between 20 and 40 years. If the disease invades the oval window niche it causes fixation of the stapes with conducting hearing loss. In some cases otospongiosis is associated with and presumably causes cochlear degeneration alone with sensoneurale hearing impairment of varyi

[Stapedectomy and a piston in otosclerosis].

At the Otolaryngological Clinic, Faculty Hospital Bratislava since 1971 to 1987 673 microsurgical operations on the stapes were made in patients with otosclerosis, incl. 73 using the method of stapedotomy and interposition of a teflon piston. The method was very successful. Hearing improved in 91.7% of the patients. In no instance marked deterioration of hearing or deafness developed. The surgical operation is associated with a smaller risk of damage to the inner ear. Patients tolerate stapedotomy well and the results are better than after stapedoplasties according to Schuknecht. Immunohistologic findings in otosclerosis.

The cause of otosclerosis is still unclear. Between 1981 and 1988 we obtained 186 biopsy specimens of otosclerotic bone during the course of stapedectomies. Serial sections of the specimens were examined histologically (H & E) and immunohistologically (peroxidase-antiperoxidase technique) for immunoglobulin G, immunoglobulin M, and C1q and C3 complements. Active otosclerotic foci were found in 61 cases, 43 of which had IgG deposits. IgM antibodies were absent. In specimens with inactive otosclerosis, IgG and IgM were not evident, C1q and C3 complements were demonstrated together with IgG, but

Fluoride therapy for otosclerosis.

From the extensive morphological studies performed, otosclerosis appears to be ubiquitous, and in many instances subclinical. Despite reports of the efficacy of fluoride for this condition, at present its use should be regarded as experimental.

The surgical concept for otosclerosis.

The aggregate results of the stapedectomy operation for hearing improvement are highly satisfactory in the short and long term when carried out by an experienced and skilled surgeon, using a variety of techniques in carefully selected candidates. An initial bone-air gap of 10 dB or less is achieved in 80-90% of patients. The outcome of stapedectomy is dependent on the patient's level of sensorineural reserve which is age dependent, and on the limitations imposed by stapedial footplate pathology. The advantages of various types of piston prostheses for reconstruction have become increasingly cl

A tympanometric approach to otosclerosis.

A formula has been elaborated to estimate the resonance frequency of the middle ear from non-simultaneously recorded susceptance and conductance tympanograms at 220 and 660 Hz. The formula is tested on 30 normal and 29 otosclerotic ears. From these experimental results one can conclude that the middle-ear model (proposed by Lutman), from which the formula was deduced, is too crude to assess the resonance frequency. Nevertheless a ratio of phase angles functions at 220 and 660 Hz, introduced in that formula, is always qualitatively linked to the resonance frequency. Experimental data proved tha
