Long-term effectiveness and overdiagnosis in the NHS breast screening programme: a cohort study with up to 33 years of follow-up.

BACKGROUND: Population-based mammographic screening programmes have been operating for over 30 years and whilst many studies have assessed the short- to medium-term effectiveness of screening, few have assessed their long-term effectiveness. The aim of this study was to assess the long-term effectiveness of the NHS Breast Screening Programme in England and Wales. METHODS: 2,509,384 women aged 49-64 years and uninvited to screening upon study entry were studied, with screening status changing on invitation and attendance. Women were followed for breast cancer diagnosis and death for a maximum

Evolving epidemiological patterns of thyroid cancer and estimates of overdiagnosis in 2013-17 in 63 countries worldwide: a population-based study.

BACKGROUND: The incidence of thyroid cancer has been increasing in many countries, mainly due to overdiagnosis. Given these rapid changes, we aim to assess the specific features of the thyroid cancer diagnosis epidemic and provide estimates of overdiagnosis across countries spanning five continents and identify areas in which coping strategies are needed. METHODS: Two types of data were retrieved from the International Agency for Research on Cancer (IARC) Global Cancer Observatory database. The long-term annual incidence of thyroid cancer by sex and 5-year age group for all ages from 1980 to

Overdiagnosis and overtreatment of nipple and breast candidiasis: A review of the relationship between diagnoses of mammary candidiasis and Candida albicans in breastfeeding women.

BACKGROUND: Breastfeeding mothers commonly experience nipple pain accompanied by radiating, stabbing or constant breast pain between feeds, sometimes associated with pink shiny nipple epithelium and white flakes of skin. Current guidelines diagnose these signs and symptoms as mammary candidiasis and stipulate antifungal medications. AIM: This study reviews existing research into the relationship between Candida albicans and nipple and breast pain in breastfeeding women who have been diagnosed with mammary candidiasis; whether fluconazole is an effective treatment; and the presence of C. albic

Global health inequities in thyroid cancer among adolescents and young adults from overdiagnosis to inadequate care by the socio-demographic index.

INTRODUCTION: Thyroid cancer is the most common and rapidly increasing malignancy among adolescents and young adults (AYAs, aged 15-39 years). While this trend is well-documented, a critical gap exists in understanding how the disease burden varies with socioeconomic development. This study analyzes global disparities in the AYAs thyroid cancer burden across Socio-demographic Index (SDI) levels, identifying ecological patterns that suggest the dual challenges of overdiagnosis and inadequate care. METHODS: We explored the global disparity in disease burden of thyroid cancer among AYAs utilizin

Trends in lung cancer incidence and mortality and the potential for overdiagnosis associated with computed tomography screening across 21 countries on six continents.

BACKGROUND: Lung cancer remains a leading cause of cancer-related deaths worldwide. While low-dose computed tomography (LDCT) screening has proven effective in reducing deaths among high-risk populations, its extension to average-risk individuals raises concern of potential overdiagnosis. Recent evidence from Shanghai, China, indicates this risk. However, cross-national ecological assessments remain limited. This study was therefore conducted to investigate population-level signals of potential lung cancer overdiagnosis across multiple countries. METHODS: Using the Global Burden of Disease 20

Estimating the rate of overdiagnosis with prostate cancer screening: evidence from the Finnish component of the European Randomized Study of Screening for Prostate Cancer.

PURPOSE: Screening for prostate cancer may have limited impact on decreasing prostate cancer-related mortality. A major disadvantage is overdiagnosis, whereby lesions are identified that would not have become evident during the man's lifetime if screening had not taken place. The present study aims to estimate the rate of overdiagnosis using Finnish data from the European randomized trial of prostate cancer screening. METHODS: We used data from 80,149 men randomized to a screening or a control group, distinguishing four birth cohorts. We used the "catch-up method" to identify when the differe

Potential overdiagnosis of long QT syndrome using exercise stress and QT stand testing in children and adolescents with a low probability of disease.

BACKGROUND: Long QT syndrome (LQTS) is a dangerous arrhythmia disorder that often presents in childhood and adolescence. The exercise stress test (EST) and QT-stand test may unmask QT interval prolongation at key heart rate transition points in LQTS, but their utility in children is debated. OBJECTIVE: To determine if the QT-stand test or EST can differentiate children with a low probability of LQTS from those with confirmed LQTS. METHODS: This retrospective study compares the corrected QT intervals (QTc) of children (<19 years) during the QT-stand test and EST. Patients were divided into th

Clonal proliferation of B lymphocytes in the germinal centers of human reactive lymph nodes: possibility of overdiagnosis of B cell clonal proliferation.

Clonal expansion of the germinal center B cells of human reactive lymph nodes was analyzed. By micromanipulation, 28 germinal centers were microdissected from three nonneoplastic lymph nodes that had been fixed with formalin. Immunoglobulin heavy chain variable (V) region gene rearrangement was examined by seminested polymerase chain reaction (PCR) using two sets of primers (FR2-J and FR3A-J). An oligoclonal development (one to five clones) was found in each germinal center. Depending on the primer used, four or five (16%) of the germinal centers showed a single rearrangement band. The average

Effect of implementing population-based prostate-specific antigen screening on testing rates and prostate cancer overdiagnosis in England: a statistical modelling study.

OBJECTIVE: To determine the potential effect of implementing population-based prostate-specific antigen (PSA) screening in England on overdiagnosis and testing rates compared with the current opportunistic testing policy. DESIGN: Statistical modeling study. English data on rates of prostate cancer by stage, symptomatic and asymptomatic PSA testing, and life expectancy were merged with epidemiological assumptions on lead time to evaluate plausible overdiagnosis and PSA testing rates from an organized population-based program, in comparison with the current opportunistic policy. The Revenge of Unintended Consequences of Anaphylaxis-Risk Overdiagnosis: How Far We Have Come and How Far We Have to Go.

Overdiagnosis of anaphylaxis risk is an underappreciated aspect of anaphylaxis prevention. Whereas the benefits of anaphylaxis-risk prevention are well known, potential harms resulting from preemptive approaches to mitigate anaphylaxis-risk are not insignificant. Still, great progress has been made in recent years to avoid the unintended consequences of anaphylaxis-risk overdiagnosis. Reflection on recent advances in the use of diagnostic testing, as well as the application of diagnostic labels, provides an important perspective to understand how far the specialty of allergy and immunology has

It depends on where you stand on screening: health professionals' awareness, perspectives, and approaches to overdiagnosis in cancer screening - a qualitative interview study.

BACKGROUND: Overdiagnosis is a major challenge in screening, leading to healthy individuals being diagnosed and treated for conditions that would never have caused harm. While evidence on overdiagnosis is growing, little is known about how health professionals (HPs) understand and engage with this phenomenon in practice. This study examines health professionals' awareness, understanding and approaches to overdiagnosis in breast, lung, and prostate cancer screening. METHODS: This is a qualitative study with semi-structured interviews, using Positioning Theory as an analytical lens, among HPs i

No overdiagnosis in the Norwegian Breast Cancer Screening Program estimated by combining record linkage and questionnaire information in the Norwegian Women and Cancer study.

BACKGROUND: The Norwegian Breast Cancer Screening Program (NBCSP) was implemented across the country in 2005 and has been criticised for potential 'overdiagnosis', i.e. a breast cancer diagnosis that otherwise would not have been detected or treated in a woman's lifetime. We aimed to estimate overdiagnosis in the NBCSP based on the Norwegian Women and Cancer (NOWAC) study using both questionnaire information and record linkage information from NBCSP. METHOD: For 124,978 women aged 49-79 years from the NOWAC study, information on screened women could be cross-validated from the NBCSP database.

Molecular Detection of the Carriage Rate of Four Intestinal Protozoa with Real-Time Polymerase Chain Reaction: Possible Overdiagnosis of Entamoeba histolytica in Nigeria.

Diarrhea remains the second largest killer of children worldwide, and Nigeria ranks number two on the list of global deaths attributable to diarrhea. Meanwhile, prevalence studies on potentially diarrheagenic protozoa in asymptomatic carriers using molecular detection methods remain scarce in sub-Saharan countries. To overcome sensitivity issues related to microscopic detection and identification of cysts in stool concentrates, real-time polymerase chain reaction (PCR) was used to analyze genomic DNAs extracted from stool samples from 199 healthy school children for Entamoeba histolytica, E. d

Left ventricular noncompaction in a patient with fabry disease: overdiagnosis, morphological manifestation of fabry disease or two unrelated rare conditions in the same patient?

We report a clinical case of a young female with Fabry disease but without left ventricular hypertrophy, which fulfills the diagnostic criteria of left ventricular noncompaction (LVNC). To our knowledge, this is the first report of LVNC in a patient with Fabry disease. The possibility of an overdiagnosis of LVNC is discussed based on the limitations of the current diagnostic criteria. This case was further investigated by genetic analysis, which came to demonstrate the limited usefulness of genetic testing in the diagnosis of LVNC. Assuming a true trabecular pattern of LVNC, the hypothesis tha

Effect of Implementing Population-Based Prostate-Specific Antigen Screening on Testing Rates and Prostate Cancer Overdiagnosis in England: A Statistical Modelling Study.

We undertook a statistical modeling study to determine the effect of implementing population-based prostate-specific antigen (PSA) screening in England on overdiagnosis and PSA testing rates in comparison with the current opportunistic testing policy. Our model merged English data on life expectancy, rates of PSA testing and incidence of prostate cancer by stage with epidemiological data on lead time. In the base scenario, introduction of population-based screening led to an approximate 25% reduction in both PSA testing and overdiagnosis in the population compared with the current policy. This

Does mammographic density predict survival in women with invasive breast cancer? The need to account for potential confounding from cancer stage and overdiagnosis.

• The negative findings from the linked study suggest breast density may not affect prognosis after a breast cancer diagnosis. • Cancer stage and overdiagnosis are potential confounders of the relationship between breast density and health outcomes. • The prognostic value of changes in mammographic density over time may be explored in future research.
