Clinical evaluation of diazepam for relief of postoperative pain.

Morphine 10 mg or diazepam 10 mg or a mixture of morphine 5 mg and diazepam 5 mg were given i.m. to three groups of 35 patients for relief of pain in the period immediately after operation. The patients had undergone upper abdominal operation for which a uniform regime of premedication and anaesthesia had been used. Pain was assessed by an observer before and after treatment and by the subjects, using a five-point scoring scheme. The pain scores before treatment and at 30-, 60-, 90- and 120-min intervals after treatment were compared by Ridit analysis. All three treatments produced significant

A multi-level implementation strategy to increase adoption of chiropractic care for low back pain in primary care clinics: a randomized stepped-wedge pilot study protocol.

INTRODUCTION: Limited adoption of first line treatments for low back pain (LBP) in primary care settings may contribute to an overreliance on pain medications by primary care providers (PCPs). While chiropractic care typically includes recommended nonpharmacologic approaches (e.g., manual therapy, exercise instruction, advice on self-care), implementation strategies to increase adoption of chiropractic care for LBP in primary care clinics are understudied, particularly in underserved communities. METHODS: We will use a stepped-wedge cluster randomized controlled pilot trial design to evaluate

Urine Drug Testing at an Ambulatory Pain Clinic in Patients Receiving Opioids for Cancer Pain.

Importance: Urine drug test (UDT) can help screen and monitor aberrant controlled substance use among patients requiring opioid therapy for cancer pain. However, data on UDT utilization in patients with cancer pain remains limited. Objective: To determine the rate of UDT ordering and the factors associated with UDT order placement and the frequency and type of abnormal UDT findings. Methods: This retrospective study included adults (≥18 yrs) with a cancer diagnosis receiving prescription opioids for pain in an ambulatory pain clinic from November 2017 through April 2018. Descriptive statistics

Clinical Phenotypes and the Effects of Interdisciplinary Pain Treatment in Pediatric Complex Regional Pain Syndrome.

BACKGROUND: This investigation aimed to replicate phenotypes in pediatric complex regional pain syndrome (CRPS) previously identified in adult CRPS patients. The impact of an intensive pain therapeutic approach in pediatric CRPS was also evaluated. METHODS: Neurological examination data were obtained from two samples of children with CRPS (cohort #1; N = 24 and cohort #2; N = 14). The structure among CRPS signs was analyzed on pooled data from cohorts #1 and 2 using hierarchical clustering. A phenotype score was quantified for patients and then submitted to unsupervised k-means clustering. Pa

Sensory and Clinical Characterization of Acute and Chronic Pain in Hemophilia: Identification of Distinct Pain Phenotypes.

BACKGROUND: Pain is a major clinical problem in hemophilia, traditionally assessed in terms of intensity and frequency. However, the sensory characteristics of pain and patient-perceived differences between acute and chronic pain remain insufficiently explored. This study aimed to systematically characterize the sensory and clinical features of acute and chronic pain in people with hemophilia and to determine whether clinically meaningful differences exist between these pain types. METHODS: In this cross-sectional study, people with hemophilia followed at a comprehensive care center were eval

Clinical Significance of a Pain Scoring System for Deep Endometriosis by Pelvic Examination: Pain Score.

Endometriosis-associated pain is an essential factor in deciding surgical indications of endometriosis. However, there is no quantitative method to diagnose the intensity of local pain in endometriosis (especially deep endometriosis). This study aims to examine the clinical significance of the pain score, a preoperative diagnostic scoring system for endometriotic pain that can be performed only with pelvic examination, devised for the above purpose. The data from 131 patients from a previous study were included and evaluated using the pain score. This score measures the pain intensity in each

Clinical features of chronic primary pain in individuals presenting painful temporomandibular disorder and comorbidities.

BACKGROUND: The diagnosis of chronic primary pain (CPP), according to the recently released International Classification of Disease (ICD-11) criteria, refers to conditions with complex aetiologies. CPP is characterized by specific clinical features such as generalized sensory hypersensitivity and widespread pain, and is associated with functional disability and emotional distress. OBJECTIVE: This study investigated clinical features of CPP in individuals with painful temporomandibular disorders (TMD) and comorbidities (fibromyalgia, migraine and/or tension-type headache). METHODS: This cross

Thermal grill illusion of pain in patients with chronic pain: a clinical marker of central sensitization?

The thermal grill illusion of pain (TGIP) is a paradoxical burning pain sensation elicited by the simultaneous application of innocuous cutaneous warm and cold stimuli with a thermode ("thermal grill") consisting of interlaced heated and cooled bars. Its neurophysiological mechanisms are unclear, but TGIP may have some mechanisms in common with pathological pain, including central sensitization in particular, through the involvement of N-methyl- d -aspartate receptors. However, few studies have investigated TGIP in patients with chronic pain and its clinical relevance is uncertain. We hypothes

Early Neonatal Pain-A Review of Clinical and Experimental Implications on Painful Conditions Later in Life.

Modern health care has brought our society innumerable benefits but has also introduced the experience of pain very early in life. For example, it is now routine care for newborns to receive various injections or have blood drawn within 24 h of life. For infants who are sick or premature, the pain experiences inherent in the required medical care are frequent and often severe, with neonates requiring intensive care admission encountering approximately fourteen painful procedures daily in the hospital. Given that much of the world has seen a steady increase in preterm births for the last severa

Assessment and treatment at a pain clinic: A one-year follow-up of patients with chronic pain.

BACKGROUND AND AIMS: Pain is one of the most common reasons for patients to seek primary health care. Pain relief is likely to be achieved for patients suffering from acute pain, but for individuals with chronic pain it is more likely that the condition will persist. These patients have the option of being referred to specialised pain clinics. However, the complexity surrounding chronic pain patients is not well studied in these settings. This study aimed to describe patients with chronic pain referred to a pain clinic by using the information submitted during their first visit and one year la

Clinically significant differences in acute pain measured on self-report pain scales in children.

OBJECTIVES: The objective was to determine the minimum and ideal clinically significant differences (MCSD, ICSD) in pain intensity in children for the Faces Pain Scale-Revised (FPS-R) and the Color Analog Scale (CAS) and to identify any differences in these estimates based on patient characteristics. METHODS: This was a prospective study of children aged 4 to 17 years with acute pain presenting to two urban pediatric emergency departments. Participants self-reported their pain intensity using the FPS-R and CAS and qualitatively described their changes in pain. Changes in pain score reported u

Duration of clinically meaningful improvement in pain as a patient-centered endpoint in acute pain trials.

Current US Food and Drug Administration (FDA) guidance recommends the sum of pain intensity differences (SPID) as the primary efficacy endpoint for acute pain trials, which lacks validated thresholds for clinically meaningful change and requires imputation of pain scores after rescue medication use. We introduce the responder outcome over time (ROOT), defined as the proportion of study time a participant experiences clinically important improvement in pain intensity without recent rescue medication use or early discontinuation. The ROOT combines the clinical interpretability of a responder-bas

Pain reprocessing therapy in adults with chronic widespread pain: A pilot randomized clinical trial.

INTRODUCTION: Chronic Widespread Pain (CWP) is a disabling condition with no established cure. As a nociplastic pain condition without active tissue damage, it is considered potentially reversible. Pain Reprocessing Therapy (PRT) is a mechanism-based psychological intervention shown to reduce or eliminate pain in chronic low back pain but has not been tested in CWP. METHODS: This 12-week, two-arm pilot randomized clinical trial enrolled 53 participants with CWP, randomized 1:1 to PRT (n = 27) or psychoeducation control (n = 26). The PRT group received one health evaluation and education sessi

Intraprocedural Pain During Intradetrusor Onabotulinum Toxin-A Injections: A Clinical Evaluation of Pain Distribution.

OBJECTIVES: This study aimed to evaluate the distribution and intensity of intraprocedural pain during intradetrusor onabotulinumtoxinA injections by assessing differences across anatomical bladder regions and diagnostic subgroups. Specifically, the study investigated whether pain perception varied based on gender, diagnosis, and injection site in patients with idiopathic detrusor overactivity (IDO), neurogenic detrusor overactivity (NDO), and wet overactive bladder (OAB-wet). METHODS: In this prospective observational study, 119 adult patients who underwent intradetrusor onabotulinumtoxinA i
