Wound repair and regeneration : official publication of the Wound Healing Society [and] the European Tissue Repair Society2015eng
Muscle repair is a crucial component of palatoplasty but little is known about muscle regeneration after cleft palate repair. We hypothesized that the formation of new myofibers is hampered by collagen accumulation after experimental injury of the soft palate of rats. One-millimeter excisional defects were made in the soft palates of 32 rats. The wound area was evaluated after 3, 7, 28, and 56 days using azocarmine G and aniline blue to stain for collagen and immunohistochemistry to identify myofibroblasts and to monitor skeletal muscle differentiation. To evaluate age effects, 16 unwounded an

BACKGROUND: In this study, we report a rare case of an adult patient with essential palatal tremors (EPT) presenting as nasal clicks, instead of otic clicks or objective pulsatile tinnitus in common EPT.
CASE PRESENTATION: Nasal endoscopic examination and EMG recordings of the soft palate muscles were performed to confirm the source of the clicks. Initial treatment with lidocaine provided symptomatic relief for four hours. The patient was then treated with four simultaneous injections of 12.5 U of botulinum toxin in four different sites of the soft palate. Palatal tremors and clicks completel

We describe two cases of palatal myoclonus (PM), one essential and another secondary to a stroke. Case 1: a 64 years old female who developed clicking sounds in both ears after a stroke and three years later on noticed a progressive involuntary movement of the throat associated with rhythmic contractions of the soft palate, muscles of tongue and throat. MRI showed an ischemic area in brainstem. The patient had a partial response to the use of sumatriptan 6 mg subcutaneously. Case 2: a 66 years old female who began with ear clicking at left ear that worsed slowly associated with tinnitus and ar

Fifty-two patients with velopharyngeal incompetency without overt cleft palate underwent pharyngeal flap surgery. Surgery included exploration of the soft palate and reconstruction of the levator muscles. Four patients had a completely normal hard and soft palate. In two patients, the palatal musculature was normal but a U-shaped defect of the posterior hard palate was palpable. Seventeen patients had a diagnosis of submucous cleft palate made preoperatively and of these thirteen presented with the classic triad of a bifid uvula, a palpable abnormality of the hard palate and diastasis of the s

OBJECTIVE: (a) Pharyngeal flap and sphincter pharyngoplasty are the procedures most frequently chosen by craniofacial surgeons for surgical management of velopharyngeal insufficiency. Both operations may be complicated by obstructive breathing and even sleep apnea. (b) The purpose of this study is to evaluate the efficacy of a palatopharyngeal sling in the treatment of velopharyngeal insufficiency in cases with weak palatal mobility and its effect on breathing.
METHODS: Seventeen cases of post-palatoplasty velopharyngeal insufficiency were subjected to treatment by palatopharyngeal sling. Thi

In an effort to clarify the morphologic characteristics of the palatopharyngeal muscle, we examined its origin, insertion, and positional relationship with other muscles. We found that the origin of the palatopharyngeal muscle was both the oral and the nasal side of the soft palate; it was also attached to both the palatal aponeurosis and the soft palate median. However, in some cases the muscle originated on the nasal side was lacked. When the palatopharyngeal muscle originated from both the oral and the nasal side, it traveled through its insertion via the levator muscle of the palatine velu

BACKGOUND: Pierre Robin sequence (PRS) has worse speech outcomes than isolated cleft palate. We aimed to search for possible associations of phonological outcomes with PRS status (isolated vs syndromic), clinical severity, soft palate muscles deficiency, or surgical procedure.
METHODS: We designed a retrospective study of 130 children (male/female ratio: 0.4) with isolated (96) or syndromic (34) PRS with cleft palate. Grading systems were used to classify retrognathia, glossoptosis, and respiratory and feeding disorders. Electromyography was used to investigate levator veli palatini muscles.

Objective tinnitus refers to a tinnitus that can also be heard by the examiner. It is a relatively rare condition, and can be misdiagnosed or neglected. Some causes of objective tinnitus are head and neck vascular malformations, or muscular myoclonus of the tensor tympani, stapedial, or palatal muscles. The case of an 11-year-old girl with an objective tinnitus lasting from 1 year is herein presented, and the diagnostic workup performed in this unusual case is described.

A 40-year-old woman with myoclonic contractions bilaterally in the infrahyoid neck muscles, especially in the left cricothyroid muscle, was presented and successfully treated with botulinum toxin injections. The patient had a wide, aberrant vessel curving into the left dorsolateral reticular formation of the medulla oblongata. Based on our observations, we propose that symptomatic bulbar and palatal myoclonus is caused by pathology in the dorsolateral reticular formation, and not by inferior olivary dysfunction as is currently thought.

The primary function of the palate is to separate the nasal and oral cavities and facilitate the production of normal speech. The velar levator veli palatini (LVP) muscular sling is the palate speech motor. In the cleft palate, the LVP muscles are aberrantly sagittally oriented, which prevents their normal cranial, posterior, and lateral pull on the soft palate, resulting in velopharyngeal incompetence. Palatoplasty techniques enlist the same 3 principles: closure of the nasal mucosa, reorientation and repair of the LVP muscle sling, and closure of the oral mucosa. The primary outcome measures

Adenoid cystic carcinoma (ACC) is a rare malignancy arising from the exocrine glands. It most commonly involves the minor salivary glands. In the oral cavity, palate is the commonest site. For palatal ACC, surgical resection is the primary treatment of choice. However the tumour location makes complete resection demanding, due to its associated complications of velopharyngeal insufficiency, oronasal fistula, foreign body sensation and pain. Various techniques like coblation, CO2 laser and robotic surgery are available for the trans-oral resection of oral and oropharyngeal tumours. Recently, th

PURPOSE: The purpose of this study is to evaluate the effect of the different surgical techniques of expansion sphincter pharyngoplasty (ESP) on the dimensions of the oropharyngeal airway.
METHODS: The techniques that were evaluated included the preservation and transection of the palatopharyngeus (PP) and superior pharyngeal constrictor (SPC) muscle attachment and transposition of the PP muscle to the hamulus of the medial pterygoid plate and the palatal musculature. Surgical techniques were applied in twenty half heads.
RESULTS: The preservation of the PP-SPC attachment inhibited the trans

To clarify the aspects and role of oropharyngeal closure, soft palate movements during sucking were observed and then assessed by electromyographic and fluoroscopic analyses. Three patterns of sucking movements by the palatoglossus and levator veli palatini muscles were analyzed with electromyographic means in seven healthy adults. Furthermore, a forced sucking maneuver, which required special effort to produce a strong sucking pressure by opening the jaw, was analyzed using lateral fluoroscopy in three of the subjects. During all tested sucking movements, the palatoglossus muscle showed conti

In an attempt to improve speech results following palate repair while allowing adequate maxillary growth, a palatoplasty using two opposing Z-plasties of the soft palate, one of the oral and one of the nasal layers, has been used in 22 infants. Eight patients had unilateral cleft lip and palate, eight had bilateral cleft lip and palate, and six had cleft palate. The Z-plasties facilitate effective dissection and redirection of the palatal muscles to produce an overlapping muscle sling and lengthen the velum without using tissue from the hard palate, which permits hard palate closure without pu

BACKGROUND: Velopharyngeal inadequacy (VPI) after primary palatal repair remains a persistent problem despite advanced surgical techniques. Velopharyngeal inadequacy has significant negative impacts on patients' speech intelligibility and quality. Previous studies have produced conflicting results regarding the causes of VPI, which are likely due to limitations in two-dimensional imaging techniques used as well as failure to include matched groups with repaired cleft palate and differing velopharyngeal outcome. The present study aimed to investigate the characteristics of the velopharyngeal st

The role of fetal surgery in the management of congenital anomalies and intrauterine abnormalities is appropriately restricted on the basis of feasibility and risk-to-benefit analyses of intrauterine intervention. Recently, the authors demonstrated that in utero cleft palate repair of the congenital caprine model is technically feasible and results in scarless healing of the mucoperiosteum and velum, with subsequent development of a potentially functional bilaminar palate with distinct oral and nasal mucosal layers, following single-layer repair of the fetal mucoperiosteal flaps. A slight inde

INTRODUCTION: Schwannomas are benign tumors derived from Schwann cells. However, schwannomas in the soft palate is considered rare.
PRESENTATION OF CASE: We report a case of a 17-year-old girl who presented with a 20-mm nodular lesion in the soft palate. After surgical resection, a buccinator musculomucosal flap was used for soft palate reconstruction. At 6 months post-operation, velopharyngeal insufficiency was not observed.
DISCUSSION: In this report, palatal muscles are examined, and the necessity of palatal muscle preservation and reconstruction of the defect in the soft palate mucosa af

Since 1984, we have used the buccinator muscle mucosal flap for the treatment of mucosal defects after tumor resection, osteomyelitis of the mandible, closure of cleft palate fistulas, primary closure of very wide cleft palates, and lengthening of the soft palate. A study was made in the facial regions of 14 cadavers, and a comparison was made to descriptions found in the anatomic literature. It was confirmed in our dissections that the buccal artery, which reaches the posterior half of the muscle, is the major arterial pedicle of the buccinator and that it runs very close to the buccal nerve.
