Bilateral cleft lip and palate with a severe hypoplastic and backward rotated premaxilla and lack of soft tissues is a rare congenital facial deformity. No treatment protocol for this type of cleft is widely accepted. In patient with bilateral cleft lip and palate, the premaxilla was protracted by nasoalveolar molding before lip surgery. The nasal tip was elevated and the columella lengthened by nasal components incorporated into the palatal guidance plate. After 4 months of nasoalveolar molding, surgery could be performed without complications. Postoperative use of a guidance plate prevented

A case history of a bilateral cleft lip and palate male patient is presented. He was operated late at the age of his twelve, without any previous orthodontic treatment. Surgical reconstruction included, lip, hard and soft palate, removal of the premaxilla, and later on the pharingoplasty. At the age of twenty years he was prosthetically rehabilitated. Nine inner telescope crowns were made in "Palador" silver alloy, and nine outer in chrom-cobalt splinted together and connected with the skeletal frame. Intermaxillary relations were recorded and established in a manner used in the construction o

Speech is the most basic yet invaluable mode of expression for an individual. Alterations in speech can have vast effects on the psychological well-being of a person, hampering social interactions. Congenital or traumatic defects of the hard and soft palate result in velopharyngeal dysfunction, which often results in abnormal and aberrant speech. Apart from these, it is also a common outcome following surgical repair of cleft palate. Prosthodontic management of such cases with velopharyngeal obturators to improve speech and function is well documented and known to give optimal results. In this

The dynamic of velopharyngeal sphincter mechanism is a complex motor skill involving coordination of soft palate and posterior and lateral pharyngeal walls. At rest, the soft palate drapes downward so that the oropharynx and the nasopharynx open allowing for normal breathing. However, during deglutition and certain speech, sounds such as plosives require complete or nearly complete velopharyngeal closure, whereas during utterance of vowels, the port needs to be open at varying degrees. Defects in velopharyngeal mechanism lead to hypernasality and decreased intelligibility of speech. The aim of

Maxillectomy is done for a variety of disease conditions. Reconstruction following maxillectomy is done to restore the form and function. One of the important goals that are to be achieved in reconstruction is the separation of the oral and nasal cavities. In this article, we report the use of palatal flap by preserving the descending palatine artery during bilateral inferior partial maxillectomy, for separating the nasal cavity from the oral cavity. This technique eliminates the need for an obturator or another free or local flap for this purpose.

OBJECTIVES: To present the state of knowledge and surgical practice concerning cleft lip and palate leading to the modern era.
DESIGN: Bibliographical review.
SETTING: Analysis of the scientific medical, surgical, and odontological literature from the 16th to 18th centuries.
RESULTS: Texts and art confirm the existence of cleft lip and palate from antiquity in all cultures; however, the first certain references in Western scientific literature did not appear until the works of Celsus (ca. 25 BC to AD 50) and Galen (AD 129 to 200). Indications for cleft lip surgery appear in the Middle Ages

INTRODUCTION: The purpose of this article was to show the importance of the knowledge of the anatomy of root canals. Unusual root and root canal morphologies associated with both buccal roots of upper molars have been recorded in several studies in the literature. However, scientific information focusing on variations of the palatal root is rare.
METHODS: In this report, four cases are presented involving the root canal treatment of maxillary first and second molars with unusual morphologic configurations of the palatal root canals.
RESULTS: During root canal treatment, type IV and V configu

Prosthodontic rehabilitation of patients with bimaxillary resection involving the maxillae, hard and soft palates, and paranasal sinuses presents a significant challenge in restoring speech, deglutition, mastication, and respiration. This clinical report describes the prosthodontic management of a young girl treated for leukemia who required a bilateral maxillectomy secondary to mucormycosis. Distraction osteogenesis, bone grafts, osseointegrated implants, and magnet attachments were used to provide retention, support, and stability of a large definitive obturator.

Mucoepidermoid carcinoma is a malignant tumor that arises from the salivary glands. The recommended treatment strategy typically involves surgical intervention, sometimes complemented by radiotherapy, depending on the histological grade of the tumor. A case of a 22-year-old female patient without medical history was described. The clinical examination revealed a bluish lesion located on the hard palate. The histological examination confirmed the diagnosis of a low-grade mucoepidermoid carcinoma. Resection of the lesion was performed and oro-nasal communication was immediately closed by a prost

Maxillary surgical defects resulting from resection of oral neoplasms vary in size from small perforations of the hard and soft palate to complete removal of these structures. Osteogenic sarcoma is of unknown etiology and is a rapidly growing tumor that may produce pain, paresthesia, and anesthesia. The recommended treatment of radical resection often results in defects that produce significant orofacial disfigurement. These defects, regardless of size, present significant functional disability, with compromised esthetics, mastication, and deglutition. Restoration with a maxillary obturator pr

The dynamic palatograph is an electrical apparatus that generates a visual display of constantly changing palatolingual contact as a function of time, using an artificial palatal plate with affixed electrodes. This paper describes a technique of speech therapy incorporating dynamic palatography for a cleft palate patient. The patient, a 6-year-old Japanese girl with a repaired unilateral cleft lip and palate, had been judged to demonstrate articulation disorders involving contact of the tongue with the hard palate or alveolus following surgical improvement of velopharyngeal function. PURPOSE: Postoncologic reconstruction of the palate represents a major surgical challenge with respect to the thin intraoral and intranasal lining. Current reconstructive methods have ranged from obturative closure of the defect to microsurgical free tissue transfer. The final choice of treatment will be influenced by the size and location of the defect and surgeon experience. The goals of palate repair include optimizing palatal function for speech and eating, and avoiding dehiscence or postoperative fistulas. This study assessed the reliability of locoregional flaps for reconstructing maxill

BACKGROUND: Surgical treatment of tumours of the maxilla and hard palate is often associated with persistent functional and psychosocial impairment, making quality-of-life assessment an important component of rehabilitation evaluation. The objective of the present study was to assess the applicability and psychometric characteristics of an adapted bilingual (Russian-Kazakh) version of the UW-QOL v4.1 questionnaire in patients after surgical treatment of tumours of the maxilla, hard palate, and adjacent anatomical regions, as well as to analyse quality-of-life outcomes depending on the method o

This study determined the effect of a speech aid prosthesis on resting breathing. Nasal cross-sectional area was measured during inspiration and expiration in eight cleft palate patients. The measurements were made for the unobturated defect during both phases of respiration and then repeated while the defect was obturated by a speech aid prosthesis. The results of the study revealed that the presence of a speech aid prosthesis significantly decreased the cross-sectional region of the nasal airway. In 50% of the subjects, the cross-sectional region was less than 0.40 cm2 with concomitant impai

Nasal alveolar molding is used effectively to reshape the nasal cartilage and mold the maxillary arch before cleft lip repair and primary rhinoplasty. It provides aesthetic and functional benefits of nasal tip and alar symmetry and improved dental arch form. At The Craniofacial Center at the University of Illinois at Chicago, the authors have developed a modification of a nasal alveolar molding appliance previously described in the literature. The key modification is the use of an orthodontic wire from the palatal prosthesis with an acrylic bulb positioned inside the nose, underneath the apex
