Whether the stomach should be preserved during total pancreatectomy(TP)is controversial. Therefore, we examined the correlation between stomach-preserving procedures on TP and postoperative gastric complications. Seven consecutive cases underwent TP(standard TP: 1 case, SSPTP: 3 cases, PPTP: 3 cases)for pancreatic cancer during the time period 2011-2019 at our hospital. There was no clinical case of postoperative gastric ulcer nor bleeding. Delayed gastric emptying(DGE)was observed in 4 cases of Grade A and 2 cases of Grade C. One of the Grade C cases was considered to be secondary DGE due to

PURPOSE: To evaluate the long-term outcomes of total pancreatectomy in a modern cohort of pancreatic cancer patients and to establish whether any factors identified prior to pancreatic resection were related to poor survival.
METHODS: We analyzed, retrospectively, patients who underwent total pancreatectomy for pancreatic cancer between 2007 and 2016. The short- and long-term outcomes were investigated and Cox regression analysis was used to evaluate the prognostic factors identified before resection.
RESULTS: The subjects were 49 patients with a mean age of 65 years, who underwent total pan

INTRODUCTION: Benign and malignant lesions of the pancreas located at the body and tail of the pancreas are managed by the standard procedure of distal pancreatectomy (DP). The mortality associated with this procedure is reported as less than 5% in high-volume centers. The major proportion of morbidity is comprised of pancreatic fistula with a reported incidence of 5% to 60%. The most considered risk factors associated with pancreatic fistula formation are soft pancreatic texture, diameter of the pancreatic duct <3 mm, intraoperative blood loss >1000 ml and surgical techniques. Among all these

Spleen preserving laparoscopic distal pancreatectomy is considered as first choice operation for symptomatic benign or small malignant lesions located at the body or tail of the pancreas. The two main surgical techniques that have been proposed and widely adopted for spleen preserving laparoscopic distal pancreatectomy are the Warshaw and Kimura techniques. A novel modified approach for laparoscopic spleen preserving distal pancreatectomy is presented. The technique was initially performed in a 57-year-old female patient with mucinous cystadenoma. Following the surgical planes created by the f

Unlike other solid-organ transplants, whole pancreas transplantation in children is relatively rare, and it occurs more frequently in the context of multivisceral or composite organ transplantation. Because children only infrequently suffer severe sequelae of type 1 diabetes mellitus, pancreas transplantation is rarely indicated in the pediatric population. More commonly, pediatric pancreas transplant occurs in the setting of incapacitating acute recurrent or chronic pancreatitis, specifically islet autotransplantation after total pancreatectomy. In this clinical scenario, total pancreatectomy

Pancreatic surgery represents one of the most challenging areas in digestive surgery. In recent years, an increasing number of laparoscopic pancreatic procedures have been performed and laparoscopic distal pancreatectomy (LDP) has gained world-wide acceptance because it does not require anastomosis or other reconstruction. To date, English literature reports more than 300 papers focusing on LDP, but only 6% included more than 30 patients. Literature review confirms that LDP is a feasible and safe procedure in patients with benign or low grade malignancies. Decreased blood loss and morbidity, e

We performed total pancreatectomy with segmental duodenectomy preserving the gastrocolic trunk and right gastroepiploic vein, to prevent gastric venous congestion, for treatment of pancreatic tumor. This is believed to be the first report of such a procedure. The patient was a 58-year-old man with high serum levels of carbohydrate antigen 19-9 and carcinoembryonic antigen. He was examined by abdominal ultrasonography and computed tomography, and diagnosed with cancer of the pancreatic body. No distant metastasis was found. We decided to perform distal pancreatectomy. After surgery, the cut edg

OBJECTIVE: The advantage of retroperitoneoscopy in renal and adrenal gland surgery has been widely acknowledged. Retroperitoneoscopy may also be a useful approach to the pancreas, a retroperitoneal organ. This study aims to evaluate the feasibility of retroperitoneoscopic pancreatectomy performed in an animal model and a small patient cohort.
METHODS: This study was divided into two stages. Initially, retroperitoneoscopic distal pancreatectomy was performed in 6-month-old Yorkshire breed piglets (n = 5; mean weight, 50 ± 5 kg). Subsequently, seven patients with suspected diagnosis of distal p

Subtotal pancreatectomy is an exceptional solution for the pain in chronic pancreatitis affecting the whole pancreas, which evolves with non-dilated Wirsung duct or in cases in which drainage procedures fail to achieve improvement of symptoms. If it is possible to avoid splenectomy, this type of surgical procedure enters in the modern concept of treatment of chronic pancreatitis named "organ sparing", basically being a Beger procedure combined with a distal pancreatectomy with spleen preservation. The case reported here is of a young male (29 years old) with idiopathic chronic pancreatitis pro

We report the case of a female with chronic pancreatitis who underwent multiple surgical procedures leading to total resection of the pancreas with successful islet auto-transplantation. Postoperatively, she developed heparin-induced thrombocytopenia. Heparinization prior to islet infusion is currently recommended to prevent portal vein thrombosis. Antibodies against heparin are not standard pre-operative testing for islet transplantation. Timely screening for heparin-dependent anti-platelet antibody should be kept in consideration for elective surgical cases (especially following previous sur

A 32-year-old woman with a choledochal cyst (Todani type I) developed recurrent acute pancreatitis leading to calcific chronic pancreatitis. She had previously been treated with two cyst drainage procedures and subtotal cyst excision. This association between choledochal cyst and chronic pancreatitis has not been previously reported. Severe continuing symptoms led to pylorus-preserving proximal pancreatoduodenectomy, which was undertaken to prevent future carcinoma in the cyst remnant and progression of the chronic pancreatitis.

The classical Whipple procedure for chronic pancreatitis has been associated with significant long term postoperative morbidity. The pylorus-preserving procedure of Longmire has reduced but not eliminated the long-term morbidity. Preservation of the whole duodenum with total pancreatectomy has been introduced for the treatment of patients with end-stage chronic pancreatitis after favourable experience with this procedure in infants for nesidioblastosis. Fourteen patients with chronic pancreatitis have had a total pancreatectomy with preservation of the duodenum and the bile duct. All patients

BACKGROUND: Neoadjuvant chemoradiotherapy (NACRT) has become a standard in the treatment of borderline and locally advanced pancreatic ductal adenocarcinomas (PDAC). Left pancreatectomy (LP) is the procedure of choice for body and tail PDAC. Only a few data compare the outcomes of laparoscopic and open LP (LLP, OLP) after NACRT.
METHODS: A monocentric retrospective cohort of patients who underwent LLP or OLP for PDAC after NACRT between 2012 and 2024.
RESULTS: We included 57 patients, 22 LLP (39%) and 35 OLP (61%) with no difference regarding demographic characteristics. LLP compared to OLP

Pancreatic neuroendocrine tumours (pNETs) are rare and can be difficult to manage due to their variable presentations. Surgical resection remains the standard treatment for symptom control and reduced risk of recurrence. Minimally invasive robotic surgery has become an alternative to open approaches due to its enhanced three-dimensional visualisation and precision. Most documented cases, however, involve solitary tumours or multifocal disease in patients with genetic syndromes such as MEN-1. We present the case of a woman in her 20s who underwent robotic subtotal pancreatectomy for multifocal

Total pancreatectomy (TP) is a complex and rarely performed procedure. With advances in surgical techniques, perioperative care, and islet auto transplantation, TP has seen renewed clinical interest. This narrative review explores the current evidence on outcomes following TP, with a focus on age-related differences in morbidity, mortality, metabolic challenges, quality of life, and survival. Relevant literature from peer-reviewed journals was reviewed to compare perioperative and long-term outcomes in younger versus older patients undergoing TP, including the role of total pancreatectomy with

BACKGROUND AND METHODS: The incidence of pancreatic cancer is increasing, and up to 55% of patients present with metastatic disease at the time of diagnosis. Many patients also develop metastatic disease following surgical resection. The impact of metastatic patterns on outcomes has not been described. A retrospective chart review was conducted of patients with pancreatic adenocarcinoma treated at a tertiary care center from 2012 to 2023. Patients who presented with metastatic disease or developed metastatic disease during their treatment course were identified. Univariate analysis was perform
