The impact of the next pandemic influenza is likely to be far greater, by orders of magnitude, than most bioterrorism (BT) scenarios. A written pandemic emergency plan and establishment of emergency management teams are critical to mounting a coordinated and effective response to what will be a catastrophic event. Members of these teams should include public health, medical, emergency response and public safety officials, organized at each local, state and federal level. The tragic events of September 11, 2001 and the subsequent anthrax attacks have substantially increased funding and support

BACKGROUND: Hospitals should plan a staffing surge capacity to prepare for a new pandemic or disaster. As medical students are a possible surge capacity resource, this study investigated whether including students in surge capacity plans is desirable and under which conditions it is feasible.
METHODS: We performed two retrospective multicentre observational trials in 2023 by enrolling medical students from 20 German faculties and leadership from critical areas of inpatient care at two university hospitals. We investigated the integration of medical students in the COVID-19 effort, assessed th

Historically, pandemics constitute a major nuisance to public health. They have a debilitating impact on global health with previous occurrences causing major mortalities worldwide. The adverse outcomes are not limited to health outcomes but ravage the social, economic, and political landscapes. The World Health Organization (WHO) stands at the front of the pandemic response, strategizing to contain and mitigate the impacts on humans and the environment. It also intervenes in regional disease outbreaks that pose a threat to global health through strategic technical guidance, resource allocatio

The use of wastewater-based surveillance (WBS) in infectious disease surveillance and early warning of new disease outbreaks have gained increased interest and relevance following the COVID-19 pandemic. During the pandemic, countries around the world strengthened their disease surveillance systems by exploring innovative approaches such as WBS. However, maintaining these systems in the "inter-pandemic period" is challenged by limited resources and funding. This pilot study explores the feasibility and value of a joint bilateral approach for WBS to ensure an "ever warm" system using shared reso

BACKGROUND: Covid-19 has reinforced health and economic cases for investing in pandemic preparedness and response (PPR). The World Bank and World Health Organization (WHO) propose that low- and middle-income governments and donor countries should invest $31.1 billion each year for PPR. We analyse, based on the projected economic growth of countries between 2022 and 2027, how likely it is that low- and middle-income country governments and donors can mobilize the estimated funding.
METHODS: We modelled trends in economic growth to project domestic health spending by low- and middle-income gove

Because H5N1 influenza viruses continuously threaten the public health, the WHO has prepared various clades of H5N1 mock-up vaccines as one of the measures for pandemic preparedness. The recent worldwide outbreak of H5Nx virus which belongs to clade 2.3.4.4 and of which H5N6 subtype belongs and already caused human infection also increases the need of pandemic vaccine for such novel emerging viruses. In this study, we evaluated the protective efficacy and immunogenicity of an egg-based and inactivated whole-virus H5N8 (IDCDC-RG43A) developed by CDC containing HA and NA gene of the parent virus

BACKGROUND: National rates of COVID-19 infection and fatality have varied dramatically since the onset of the pandemic. Understanding the conditions associated with this cross-country variation is essential to guiding investment in more effective preparedness and response for future pandemics.
METHODS: Daily SARS-CoV-2 infections and COVID-19 deaths for 177 countries and territories and 181 subnational locations were extracted from the Institute for Health Metrics and Evaluation's modelling database. Cumulative infection rate and infection-fatality ratio (IFR) were estimated and standardised

The COVID-19 pandemic generated an unprecedented volume of spatially and temporally resolved data, enabling rapid development of spatio-temporal models for surveillance, forecasting, and policy support. However, the evolution, geographic distribution, and equity implications of these models remain insufficiently synthesized. This study presents a global systematic review of 363 peer-reviewed studies published between January 2020 and August 2025 using publicly available data. Following PRISMA 2020 guidelines, studies were classified by geographic scale, modeling approach, data streams, and ana

This Perspective discusses how European Union (EU)-level action could support public health laboratories in preparing for future threats from respiratory infectious diseases. It draws extensively from a study that looked at the experience of public health laboratories in EU/ European Economic Area (EEA) countries during the COVID-19 pandemic. This study included a literature review and consultation with stakeholders from public health laboratories in EU/EEA countries and from EU institutions. We discuss how to address the challenges faced by public health laboratories during the COVID-19 pande

BACKGROUND: The emergence of novel reassortant influenza A virus strains and zoonotic transmission to the human population is a critical factor in the development of a global pandemic, as exemplified by the 2009 swine flu pandemic and the ongoing threat of an H5N1 avian flu pandemic. Small, unconjugated synthetic peptide vaccines comprising multiple, highly conserved epitopes may provide an important strategy towards a cost-effective, easily scalable, supra-seasonal universal influenza vaccine that can target across human, swine, and avian strains, advancing pandemic preparedness.

BACKGROUND: Heterogeneity in national SARS-CoV-2 infection surveillance capabilities may compromise global enumeration and tracking of COVID-19 cases and deaths and bias analyses of the pandemic's tolls. Taking account of heterogeneity in data completeness may thus help clarify analyses of the relationship between COVID-19 outcomes and standard preparedness measures.
METHODS: We examined country-level associations of pandemic preparedness capacities inventories, from the Global Health Security (GHS) Index and Joint External Evaluation (JEE), on SARS-CoV-2 infection and COVID-19 death data com

The COVID-19 pandemic exposed significant gaps in healthcare systems' preparedness and response capabilities including workforce coordination and collaborative practice. Although pandemic preparedness is often framed in terms of infrastructure and policy, the pandemic highlighted that health system responsiveness depends on how healthcare workers are educated and trained to collaborate, adapt, and make decisions. Healthcare workers operate within volatile, uncertain, complex, and ambiguous (VUCA) environments, necessitating new approaches to education and practice. In this scoping review we wi

BACKGROUND: The emergence of highly pathogenic avian influenza A (H5N1) clade 2.3.4.4b in dairy cattle and human cases raises urgent pandemic concerns. A critical question is whether seasonal influenza vaccines elicit cross-reactive immunity against this novel zoonotic strain, potentially contributing to pre-existing population immunity.
METHODS: We combined structural bioinformatics and serological analysis. Hemagglutinin (HA) protein sequence and structural conservation were assessed between World Health Organization WHO-recommended 2024-2025 vaccine strains (H1N1 A/Victoria/4897/2022, H3N2

INTRODUCTION: Between January and March 2022, the UK Public Health Rapid Support Team delivered a master's-level short course, Pandemics: Emergence, Spread and Response, designed to strengthen interdisciplinary knowledge and skills for pandemic preparedness. This study evaluated the course's perceived usefulness, outcomes, and longer-term impacts on professional practice, one year after completion.
METHODS: A participatory convergent mixed-methods design was used, involving course alumni as co-evaluators. Quantitative data were collected through an online survey (n = 31; 62% response rate), w

The rapid onset and progression of the COVID-19 pandemic highlighted the critical necessity for standardized, timely epidemiological investigations to generate actionable evidence for public health policy. The World Health Organization (WHO) Global Influenza Surveillance and Response System (GISRS) and Unity Studies and Investigations initiative (Unity Studies) provides the standardized framework to address these critical knowledge gaps. This manuscript reflects upon the Hamdard Institute of Medical Sciences and Research (HIMSR)'s experience as an active site implementing three WHO Unity proto
