Paraspinal muscle cross-sectional area predicts low back disability but not pain intensity.

BACKGROUND AND CONTEXT: The lumbar paraspinal muscles, including the erector spinae and multifidus, play an important role in movement and control of the spine. However, our understanding of their contribution to low back pain and disability is unclear. Systematic reviews have reported conflicting evidence for an association between paraspinal muscle size and low back pain, and a paucity of data examining muscle cross-sectional area (CSA) and low back disability. PURPOSE: To investigate the relationship between paraspinal muscle CSA and both low back pain intensity and disability. STUDY DESI

Do variations in paraspinal muscle morphology and composition predict low back pain in men?

This longitudinal study aimed to clarify the longstanding controversy over whether variations in paraspinal muscle morphology (e.g., size, composition and asymmetry) are predictors of low back pain (LBP). A sample of 99 Finnish men were included in this population-based longitudinal study. Data were collected through a structured interview, physical examination and magnetic resonance imaging (MRI). Baseline measurements of the lumbar multifidus and erector spinae muscles were obtained from T2-weighted axial images at L3-L4 and L5-S1, and interview data were obtained at baseline, 1- and 15-year

Factors associated with paraspinal muscle asymmetry in size and composition in a general population sample of men.

BACKGROUND: Paraspinal muscle asymmetry in cross-sectional area (CSA) and composition have been associated with low back pain and pathology. However, substantial multifidus muscle asymmetry also has been reported in men who were asymptomatic, and little is known about other factors influencing asymmetry. OBJECTIVE: The goal of this study was to identify behavioral, environmental, and constitutional factors associated with paraspinal muscle asymmetry. DESIGN: A cross-sectional study of 202 adult male twins was conducted. METHODS: Data were collected through a structured interview, physical e

Postoperative changes in paraspinal muscle thickness after various lumbar back surgery procedures.

Damage to the paraspinal muscle after various lumbar back surgery procedures was evaluated by measuring the paraspinal muscle thickness preoperatively and postoperatively in 89 patients, 61 males and 28 females. There were 42 single interlaminar level procedures (SL group), 13 multiple interlaminar level procedures (ML group), and 34 posterolateral fusion procedures (PLF group). Changes in paraspinal muscle thickness were evaluated at more than 10 months after surgery, because muscle swelling continued as long as 10 months after surgery, and then reduced as the edema subsided. Postoperative se

Lumbar paraspinal muscles in patients with chronic non-specific low back pain and the effect of exercise: comparative and interventional study.

BACKGROUND: Lumbar paraspinal muscles (LPM), which belong to lumbar extensor muscles, play a key role in stabilising the lumbar spine and maintaining spine health. Although chronic non-specific low back pain (CNLBP) represents a major health problem worldwide, there is a knowledge gap regarding the function, morphology of LPM, and the effect of regular exercise on them in patients with CNLBP. The purpose of the study was to evaluate functional and quantitative MRI (qMRI) parameters of LPM in CNLBP patients, compare them to matched healthy volunteers (HV), and assess the effect of exercise ther

Age- and sex-dependent differences in the morphology and composition of paraspinal muscles between subjects with and without lumbar degenerative diseases.

BACKGROUND: The quality of the paraspinal muscles has been recommended as a surrogate marker for the evaluation of the severity of the lumbar degenerative diseases (LDD). The purpose of this study is to determine the age- and sex-dependent differences in the morphology and composition of the paraspinal muscles between LDD and asymptomatic subjects. METHODS: We analyzed data from 370 patients and 327 asymptomatic volunteers aged between 18-85 years. The measurement of the cross-sectional area (CSA) of the erector spinae, multifidus, and psoas at the L4/5-disc level was performed by the magneti

Do cerebral potentials to magnetic stimulation of paraspinal muscles reflect changes in palpable muscle spasm, low back pain, and activity scores?

OBJECTIVE: Previous studies have shown that cortical-evoked potentials on magnetic stimulation of muscles are influenced by muscle contraction, vibration, and muscle spasm. This study was carried out to determine whether these potentials correlate with palpatory muscle spasm, patient symptoms, and disability in patients with low back pain. METHODS: A prospective observational study was performed on 13 subjects with a history of low back pain visiting an orthopedic hospital-based clinic. Patients were screened for serious pathologic conditions by an orthopedic surgeon. The patients were then e

Altered myogenic and lipid metabolism gene expression in paraspinal muscles of patients with adult spinal deformity who develop proximal junctional failure.

PURPOSE: Adult spinal deformity (ASD) is prevalent in aging populations and often managed with multi-level spinal fusion surgery. A major postoperative complication is proximal junctional failure (PJF), a condition at the upper end of spinal constructs linked to poor muscle quality. While risk factors such as age, alignment, body mass index, and bone density are known, the molecular basis-particularly in paraspinal muscles-remains unclear. This study examines morphological and genetic characteristics of paraspinal muscles in ASD patients to identify predictors of PJF. METHODS: Thirty-four ASD

[Comparison of MRI of Paraspinal Muscles in Patients after Open and Minimally Invasive Percutaneous Stabilisation of Thoracic and Lumbar Spine Fractures].

PURPOSE OF THE STUDY: Magnetic resonance imaging (MRI) was used to evaluate paraspinal muscle damage after classic open surgery (OPEN) and minimally invasive spine surgery (MISS) after injury. In a randomised prospective study, we evaluated our hypothesis that MISS would result in a lower extent of atrophy and fibrosis as detected by MRI. MATERIAL AND METHODS: MRI was performed after the injury and not earlier than 3 months after the removal of implants. 16 OPEN and 16 MISS patients underwent the final follow-up. Both atrophy and muscle fibrosis were assessed based on the progression of the c

Asymmetry in the Onset of Paraspinal Muscles Activity Differs in Adolescents With Idiopathic Scoliosis Compared With Those With a Symmetrical Spine.

BACKGROUND: Adolescent idiopathic scoliosis (AIS) is characterized by an asymmetrical formation of the spine and ribcage. Recent work provides evidence of asymmetrical (right versus left side) paraspinal muscle size, composition, and activation amplitude in adolescents with AIS. Each of these factors influences muscle force generation. The timing of paraspinal muscle activation may also contribute to an asymmetry in the timing of forces applied to the spine. QUESTIONS/PURPOSES: The main objectives were to determine (1) whether the timing and asymmetry of erector spinae muscle activation durin

Paraspinal muscles in individuals undergoing surgery for lumbar spine pathology lack a myogenic response to an acute bout of resistance exercise.

BackgroundLumbar spine pathology (LSP) is a common source of low back or leg pain, and paraspinal muscle in these patients demonstrates fatty and fibrotic infiltration, and cellular degeneration that do not reverse with exercise-based rehabilitation. However, it is unclear of this lack of response is due to insufficient exercise stimulus, or an inability to mount a growth response. The purpose of this study was to compare paraspinal muscle gene expression between individuals with LSP who do and do not undergo an acute bout of resistance exercise.MethodsParaspinal muscle biopsies were obtained

Functional magnetic resonance imaging reveals dysfunction of the paraspinal muscles in patients with chronic low back pain: a cross-sectional study.

BACKGROUND: Patients with chronic low back pain (CLBP) undergo structural changes of the paraspinal muscles; however, it is unclear if functional changes also occur. This study aimed to examine the metabolic and perfusion function changes in the paraspinal muscles of patients with CLBP as indirectly reflected by blood oxygen level-dependent (BOLD) imaging and T2 mapping. METHODS: All participants were consecutively enrolled at our local hospital from December 2019 to November 2020. Patients were diagnosed with CLBP in the outpatient clinic, and asymptomatic participants were considered to be

Risk Factors for Distal Junctional Problems Following Long Instrumented Fusion for Degenerative Lumbar Scoliosis: Are they Related to the Paraspinal Muscles.

PURPOSE: Although the incidence of distal junctional problems (DJPs) following long construct-based treatment for degenerative lumbar scoliosis (DLS) is lower, affected patients are more likely to require revision surgery when they occur. So the aim of this study is to identify risk factors associated with DJPs to avoid its occurrence by at least 1-year follow-up. METHODS: A total of 182 DLS patients undergoing long instrumented fusion surgery (≥4 levels) between February 2011 and March 2022 were retrospectively analyzed. Patients were placed into the DJP group if a DJP occurred at the final
