Predictive value of veno-arterial carbon dioxide partial pressure difference and central venous oxygen saturation for fluid administration and mortality in critically ill patients with extensive burns.

OBJECTIVES: Guiding fluid therapy in severely burned patients presents complex challenges, combining elements of both distributive and hypovolemic shock, accompanied by cardiovascular dysfunction. Whereas parts of hemodynamic assessment in septic shock such as central venous oxygen saturation (ScvO2) and the arterial-venous carbon dioxide (CO2) partial pressure difference (ΔpCO2 = pcvCO2 - paCO2) can both be used as indicators of hypoperfusion, they are not part of the standard clinical assessment of severely burned patients. The aim of this retrospective study was thus to investigate whether

Elevated Arterial-Central Venous Carbon Dioxide Partial Pressure Difference Indicates Poor Prognosis in the Early Postoperative Period of Open Heart Surgery in Infants with Congenital Heart Disease.

BACKGROUND: Elevated arterial-central venous carbon dioxide partial pressure difference (AVCO2) may be an important marker to predict tissue and organ hypoperfusion in adults. We analyzed the hemodynamic data of infants with congenital heart disease who underwent corrective repair with cardiopulmonary bypass (CPB) to identify whether AVCO2 has clinical significance in early postoperative tissue hypoperfusion, occurrence of complications, and clinical outcomes. METHODS: Infants with clinical conditions of hypoperfusion, without volume responsiveness and with ineffective initial treatment, with

[The influence of arterial partial pressure of co2 and of the arterial value of ph on endogenous catecholamines during extracorporal circulation in regard to oxygen consumption of the whole body (author's transl)].

Ten patients with coronary heart diseases were studied during total hypothermic extracorporal circulation relating changes of the endogenous catecholamines epinephrine and nor-epinephrine to arterial partial pressure of CO2 and to the arterial value of pH. Both of the catecholamines increased significantly during respiratory or metabolic acidosis whereas respiratory or metabolic alkalosis was accompanied by significantly lower catecholamine levels in blood. The changes in total peripheral resistance due to the endogenous blood levels of catecholamines depended proprotionally. Total peripheral

Association between the first post-resuscitation partial pressure of arterial carbon dioxide and neurologic outcomes in children with out-of-hospital cardiac arrest: a multicenter retrospective cohort study.

BACKGROUND: The prognostic significance of the first post-ROSC PaCO2 in pediatric out-of-hospital cardiac arrest (OHCA) remains uncertain. We evaluated its association with 30-day outcomes. METHODS: We conducted a multicenter retrospective cohort study using the Japanese OHCA registry (2014-2022). Pediatric patients aged <18 years with return of spontaneous circulation (ROSC) were included, and the primary analysis was restricted to patients with first post-ROSC blood gas sampling within 60 min. Patients were categorized as hypocapnia (<35 mmHg), normocapnia (35-45 mmHg), or hypercapnia (>45

Species, cell proliferation state and oxygen partial pressure are major factors in the in vitro toxicity of repurposed potential ADPKD drugs in human and murine renal proximal tubule cells.

Unexpected nephrotoxicity during clinical stages of drug development, due to undetected toxicity in in vitro and in vivo settings, can lead to drug attrition and jeopardize drug development. The latter highlights the importance of in vitro renal toxicity testing that can safely be translated to humans. Two factors commonly overlooked in the latter are the continuous cell proliferation that occurs in regeneration processes after renal insult and the physiological O2 tension in the kidney. Autosomal Dominant Polycystic Kidney Disease is a genetic disorder characterized by the development of rena

The Partial Support of the Left Ventricular Assist Device Shifts the Systemic Cardiac Output Curve Upward in Proportion to the Effective Left Ventricular Ejection Fraction in Pressure-Volume Loop.

Left ventricular assist device (LVAD) has been saving many lives in patients with severe left ventricular (LV) failure. Recently, a minimally invasive transvascular LVAD such as Impella enables us to support unstable hemodynamics in severely ill patients. Although LVAD support increases total LV cardiac output (COTLV) at the expense of decreases in the native LV cardiac output (CONLV), the underlying mechanism determining COTLV remains unestablished. This study aims to clarify the mechanism and develop a framework to predict COTLV under known LVAD flow (COLVAD). We previously developed a gener

[The behavior of arterial and mixed venous oxygen and carbon dioxide partial pressure and the pH value during and following intubation apnoea. Studies on the occurrence of the Christiansen-Douglas-Haldane effect].

The Christiansen-Douglas-Haldane effect describes the reduced CO2 binding capacity of oxygenated compared to deoxygenated haemoglobin. Under the condition of a "closed system", for example hyperoxic apnoea after adequate preoxygenation (continuous O2 uptake with lack of CO2 delivery), specific effects on the arterial and mixed venous blood gas status, due to the Haldane effect, are seen: within 30 s after onset of apnoea, "paradoxical pCO2" (paCO2 exceeds pvCO2) and "pH reversal" (pHa falls under pHv) can be observed. It was the aim of this study to demonstrate how fast arterial and mixed veno

Permissive Hypoxemia With Partial Pressure of Oxygen in Arterial Blood (PaO2) at Less Than 50 mmHg: A Case of Successful Ventilator Weaning for COVID-19-Associated Acute Respiratory Failure.

Permissive hypoxemia is a ventilation strategy that intentionally tolerates mild to moderate hypoxemia to minimize lung injury. Unlike conventional management, which aims for normal oxygen levels, permissive hypoxemia maintains a partial pressure of oxygen in arterial blood (PaO2) of 55-60 mmHg and an arterial oxygen saturation (SaO2) of 88-92%. This approach ensures sufficient oxygen delivery for vital functions while reducing mechanical stress on the lungs from high oxygen concentrations and airway pressures, which can lead to lung injury. This flexible, individualized strategy prioritizes l

Partial pressure of oxygen is lower in the left upper pulmonary vein than in the right in adults with atrial septal defect: difference in P(O2) between the right and left pulmonary veins.

BACKGROUND: The right-to-left shunt at the atrial level is responsible for arterial hypoxemia in patients with atrial septal defect. OBJECTIVES: This study investigated the mechanism of arterial hypoxemia in patients with atrial septal defect by measuring the P(O2) in both the right and left upper pulmonary veins. SUBJECTS AND METHOD: We prospectively measured the P(O2) in the femoral artery and the right and left upper pulmonary veins during cardiac catheterization in 13 adults (median age, 53 years) and 7 children (median age, 7 years) with secundum atrial septal defect. The adults and chi

Post-Treatment Partial Pressure of Carbon Dioxide for Prognostic Stratification and Non-Invasive Ventilation Guidance in Elderly Patients with Community-Acquired Pneumonia and Heart Failure: The "Subgroup with Impaired Cardiac Response".

BACKGROUND: Prognostic stratification in elderly patients with co-existing community-acquired pneumonia (CAP) and heart failure (HF) is challenging due to the limitations of static admission scores. OBJECTIVE : To evaluate the prognostic value of the 72-hour post-admission partial pressure of carbon dioxide (PCO2_post) and its utility in guiding non-invasive ventilation (NIV) therapy. METHODS: We analyzed 100 elderly patients with CAP and HF. Six-month mortality was the primary endpoint. Propensity score matching (PSM) was used to explore mechanistic differences between risk strata. RESULTS

Arterial flow focalization could increase tissue oxygen partial pressure, or trigger endothelial shear stress - a new concept to overcome cancer hypoxia-induced radiotherapy resistance, or stimulate liver regeneration during fulminant hepatitis.

Malignant tumor hypoxia, which occurs due to abnormal and poor circulation of the tumoral vasculature, is the major cause of tumor aggressivity and failure of oncology therapeutics, particularly radiotherapy. Indeed, radio-sensitivity is reduced up to 3-fold in tumoral tissues with a lower oxygen partial pressure (PtO2). As such, there is no efficient means for increasing tumor PtO2 during radiotherapy sessions. Through a concept called "arterial flow focalization", by controlled temporary endo or peri-vascular occlusion of the collateral arterial branch(es) upstream of the tumor (organ), it i

Utility of peripheral blood oxygen saturation (SpO2)/fraction of inspired oxygen (FiO2) ratio and arterial partial oxygen pressure (PaO2)/FiO2 ratio in the diagnosis of acute respiratory distress syndrome: A comparative study.

Our study determined the reliability of using the ratio of peripheral blood oxygen saturation (SpO2) and the fraction of inspired oxygen (FiO2) rather than the ratio of arterial partial oxygen pressure (PaO2) and FiO2 for diagnosing acute respiratory distress syndrome (ARDS). Pulse oximetry and arterial blood gas analysis were performed to obtain both ratios, respectively, in 70 patients who fulfilled the Berlin definition criteria of ARDS. A detailed history, clinical examination, and relevant evaluation were done. There was a significant correlation between the two ratios. The former had a l
