Primary Total Knee Arthroplasty for Treating Osteoarthritic Knees with Neglected Patellar Dislocation.

Background and Objectives: Neglected patellar dislocation in the presence of end-stage osteoarthritis (OA) is a rare condition characterized by the patella remaining laterally dislocated without reduction. Due to the scarcity of reported cases, the optimal management approach is still uncertain. However, primary total knee arthroplasty (TKA) can serve as an effective treatment option. This study aimed to present the clinical and radiological outcomes achieved using our surgical technique. Materials and Methods: A retrospective review of 12 knees in 8 patients with neglected patellar dislocatio

Consensus-Based Guidelines for Management of First-Time Patellar Dislocation in Adolescents.

BACKGROUND: The management of first-time patellar dislocation remains variable, with limited evidence to support or compare different operative and nonoperative modalities. The primary aim was to establish consensus-based guidelines for different components of nonoperative treatment following a first-time patellar dislocation. The secondary aim was to develop guidelines related to management after failed nonoperative treatment. The tertiary aim was to establish consensus-based guidelines for the management of first-time patellar dislocation with a concomitant osteochondral fracture. METHODS:

Habitual patellar dislocation 40 years after failed conservative treatment: A case report.

Patellar dislocation is often caused by congenital bone morphological abnormalities or joint laxity and is rarely traumatic. We report a case of long-term habitual patellar dislocation due to trauma 40 years ago without abnormal bone morphology, which required a combined procedure, including the Elmslie-Trillat (ET) procedure, massive lateral release, and medial patellofemoral ligament (MPFL) reconstruction, resulting in favorable outcomes. A 52-year-old male, with no specific underlying disease, dislocated his right patella due to falls when he was 13 years old and underwent conservative trea

Patellar Instability and Dislocation: Optimizing Surgical Treatment and How to Avoid Complications.

Patellar instability is a common problem seen by the orthopedic surgeon. Surgery is indicated in recurrent dislocation to improve patellar tracking and ligamentous restraint in order to decrease risk of recurrence, osteochondral injury, and eventual progression to arthritis. Preoperative imaging studies identify anatomic risk factors that increase risk of patellar dislocation to inform surgical decision making. Surgical management starts with medial patellofemoral ligament reconstruction, which is effective in many cases. Tibial tubercle osteotomy realigns the extensor mechanism and is useful

Clinics in diagnostic imaging (163). Transient lateral patellar dislocation with trochlear dysplasia.

A 14-year-old girl presented with left knee pain and swelling after an injury. Magnetic resonance (MR) imaging showed a transient lateral patellar dislocation with patellar osteochondral fracture, medial patellofemoral ligament tear and underlying femoral trochlear dysplasia. Open reduction and internal fixation of the osteochondral fracture, plication of the medial patellar retinaculum and lateral release were performed. As lateral patellar dislocation is often clinically unsuspected, an understanding of its characteristic imaging features is important in making the diagnosis. Knowledge of th

Return to sport after patellar dislocation or following surgery for patellofemoral instability.

Patellofemoral instability may occur in a young population as a result of injury during sporting activities. This review focuses on return to sport after one episode of dislocation treated no operatively and as well after surgery for chronic patellofemoral instability. With or without surgery, only two-thirds of patients return to sports at the same level as prior to injury. A high-quality rehabilitation programme using specific exercises is the key for a safe return to sporting activities. To achieve this goal, recovery of muscle strength and dynamic stability of the lower limbs is crucial. T

Bilateral recurrent patellar dislocation in a patient with isolated patella aplasia-hypoplasia.

Patella aplasia-hypoplasia is a rare condition characterized by the congenital absence or marked reduction of the patellar bone. It is well known that patella aplasia-hypoplasia occurs in nail-patella syndrome, small patella syndrome, and several other syndromes. However, isolated patella aplasia-hypoplasia without associated clinical or radiologic anomalies, first described by Kutz in 1949, is an extremely rare condition. This report describes 12 years' follow-up after medial patellofemoral ligament reconstruction surgery performed for bilateral recurrent patellar dislocation in a 29-year-old

Traumatic patellar dislocation in children and adolescents: treatment update and literature review.

PURPOSE OF REVIEW: Traumatic patellar dislocation in children and adolescents is a complex problem resulting from a range of anatomic and mechanical conditions. A careful review of the literature demonstrates certain risk factors that predispose children to recurrent dislocation. With this understanding, these conditions can be managed more aggressively in the hope of an improved outcome. Additionally, a vast number of surgical procedures to correct patellar instability have been described, and recent cadaveric studies are now guiding surgical interventions. RECENT FINDINGS: Risk factors for

Total knee arthroplasty for neglected permanent post-traumatic patellar dislocation--case report.

Permanent post-traumatic patellar dislocation is a rare entity resulting from recurrent trauma. This uncommon condition is often confused clinically with congenital dislocation of the patella. Treatment options, although not well defined, include observation, various soft tissue and bony procedures for realigning the patella and patellectomy. To our knowledge, this is the first case reporting the use of conventional non-constrained total knee arthroplasty with patellar resurfacing and soft tissue realignment procedure for long-standing patellar dislocation in a single stage. An interesting fea

[Interim results of surgical therapy of patellar dislocation by Insall proximal reconstruction].

Between 1984 and 1991, 57 patients with diagnosis of an acute or recurrent patellar dislocation were treated operatively using proximal realignment with vastus medialis transfer and lateral release (Insall operation). A total of 45 patients (15 with acute and 30 with recurrent patellar dislocation) were reviewed at an average follow-up of 6.5 years (2-9.6 years). The average age at injury was 21.5 years, with a predominance of female patients. Follow-up examination included routine knee examination, clinical review using the modified knee scoring scale of Larsen and Lauridsen, sports activity

Rotational knee strain resulting in patellar dislocation. An experimental study in rabbits.

The right lower extremities of 64 young rabbits were immobilized by a plaster spica. The animals developed a gait pattern, which included internal tibial rotation and adduction of the left (unimmobilized) tibia. Twenty-one of the animals developed medial patellar dislocation in the unimmobilized lower extremity. The mechanism of the patellar dislocation in this experimental model was possibly overstretching of the lateral colateral ligament and the lateral side of the joint capsule, associated with medial rotation of the tibia and the tibial tubercle. The direction of patellar pull when glidin

Knee dislocation associated with patellar tendon rupture: case report and literature review.

Knee dislocations and multiligament knee injuries represent a significant clinical and surgical challenge, particularly when associated with patellar tendon rupture, a rare combination with a high functional impact that may initially go unrecognized. Management of these injuries requires meticulous planning to restore anatomy, joint stability, and extensor mechanism function in order to optimize long-term functional outcomes. We report the case of a patient who sustained a knee dislocation associated with patellar tendon rupture, which was surgically managed using a two-stage approach. Current

Predictors of Osteochondral Fractures Following Patellar Dislocation in the Pediatric Emergency Department.

BACKGROUND: Patellar dislocations are frequently seen in the emergency department (ED). Almost all children with a reduced patellar dislocation will have a knee radiography, while only 10% have a fracture identified on x-ray. OBJECTIVE: The primary purpose of the study was to identify factors predicting osteochondral fractures among children with patellar dislocation reduced in the ED. METHODS: This was a retrospective cohort study of all children aged between 1 and 18 years old with patellar dislocation who needed a reduction in a tertiary care pediatric ED between 2019 and 2024. Patient-reported Outcomes Following Nonoperative Treatment of Pediatric First-Time Patellar Dislocation.

BACKGROUND: Patellar instability is a common pediatric knee injury. The purpose of this study was to analyze patient-reported outcome measures (PROMs) after nonoperative treatment following a first-time pediatric patellar dislocation. METHODS: In this single-center, retrospective study, patients <18 years old between January 2012 and December 2019 with a first-time patellar dislocation were evaluated. From this cohort, we included all those patients who initially underwent nonoperative management and followed up for at least 1 year. Measures of patellar height and patellofemoral dysplasia wer

Evaluating tibial rotation in recurrent patellar dislocation with four-dimensional computed tomography.

OBJECTIVE: This study is to investigate the three-dimensional (3D) kinematic changes in the knee joint in patients with recurrent patellar dislocation using four-dimensional computed tomography (4DCT) imaging and the 3D-3D surface registration technique. MATERIALS AND METHODS: Ten knees from nine patients with recurrent patellar dislocation and seven knees from seven controls (unaffected side of patients with unilateral anterior cruciate ligament injury) were analyzed using 4DCT. The patients were asked to extend their knees from 60° of flexion to full extension for 10 s in the CT gantry. We

[A rare cause of an acutely locked knee joint: superior patellar dislocation].

The case of a 56-year-old male patient with locking of the knee in extension without trauma is presented. Patient history, clinical examination and radiographs confirmed a superior dislocation of the patella. Closed reduction led to full weight bearing with free range of motion without further complaints. Knowledge of this uncommon cause of a locked knee joint leads to immediate treatment avoiding complex imaging modalities, such as computed tomography (CT) or magnetic resonance imaging (MRI). No further treatment was necessary in the majority of the cases published in the literature.

Computational approach to correcting joint instability in patients with recurrent patellar dislocation.

Patellar dislocation is a debilitating injury common in active adolescents and young adults. Conservative treatment after initial dislocation is often recommended, but almost half of these patients continue to suffer from recurrent dislocation. The objective of this study was to compare preoperative patellofemoral joint stability with stability after a series of simulated procedures, including restorative surgery to correct to pre-injury state, generic tibial tubercle osteotomy, patient-specific reconstructive surgery to correct anatomic abnormality, less invasive patient-specific surgery, and
