Management strategies for patients with pulmonary hypertension in the intensive care unit.

OBJECTIVE: Pulmonary hypertension may be encountered in the intensive care unit in patients with critical illnesses such as acute respiratory distress syndrome, left ventricular dysfunction, and pulmonary embolism, as well as after cardiothoracic surgery. Pulmonary hypertension also may be encountered in patients with preexisting pulmonary vascular, lung, liver, or cardiac diseases. The intensive care unit management of patients can prove extremely challenging, particularly when they become hemodynamically unstable. The objective of this review is to discuss the pathogenesis and physiology of

[Patients with ornithine transcarbamylase deficiency. Anaesthesiological and intensive care management].

Ornithine transcarbamylase deficiency (OTCD) is the most common inborn urea cycle disorder. Patients with OTCD are at risk of acute metabolic decompensation with hyperammonemia and subsequent encephalopathy, coma and death. Symptoms may be triggered by infections, drugs and stress, evoked by trauma, pain, fear, surgery and anaesthesia or by episodes of protein catabolism, i.e. fasting-induced, post partum or during gastrointestinal bleeding. Several specific considerations must be made for anaesthetic and intensive care management in patients with this disease in order to avoid metabolic decom

[Cardiorespiratory management of severe injured patients in intensive care units].

Late mortality of severely injured patients could be prevented by the quality of early cardiorespiratory management. Indeed traumatized patients with high risk of multiple organ failure (according to age, ISS, amount of blood transfused, and/or metabolic acidosis) need a pulmonary artery catheterization as soon as possible (postdefinitive phase: during the surgical period or at the admission in the ICU). Such a procedure allows the intensivist to determine therapeutic goals in term of O2 delivery (DO2) and O2 uptake (VO2) in front of a frequent increased peripheral O2 demand, These goals (usua

The Chronic Kidney Disease Care Model: Quantifying Management Strategies for Improving Patient Outcomes in the USA.

PURPOSE: While chronic kidney disease (CKD) afflicts an estimated 14% of US adults, the majority remain unaware of the ailment until later stages. CKD progression can lead to reduced quality of life, cardiovascular disease (CVD), and end-stage renal disease (ESRD). Consensus guidelines highlight novel interventions for the early recognition and management of CKD. Here, we describe the rationale and structure of a policy model for CKD that provides contemporary estimates of long-term effectiveness and cost-effectiveness of medical interventions. The utility of the model is illustrated by assess

Mobile health platform for self-management of pediatric cystic fibrosis: Impact on patient-centered care outcomes.

BACKGROUND: Previously, we adapted a mobile health platform (Genia) to the needs of patients and families in a pediatric CF center in the United States. In this feasibility study, we tested the impact of Genia on measures of patient-centered care. METHODS: In a one-group pre-post study with adolescents with CF and caregivers of children with CF, we tested Genia's effect over 6 months on patient satisfaction with chronic illness care (PACIC) and shared decision-making (CollaboRate). Feasibility and acceptability were assessed with exit interviews and app analytics. RESULTS: The intervention i

Anesthesia and postpartum pain management for placenta accreta spectrum: The patient perspective and recommendations for care.

OBJECTIVE: Placenta accreta spectrum (PAS) is a high-risk complication of pregnancy, which often requires complex surgical intervention. There is limited literature on the patient experience during the perioperative period and postpartum pain management for PAS. Therefore, this study aims to explore the patient perspective of anesthesia care. METHODS: Ethical approval was granted by the hospital ethics committee (EC02.2023). This was a descriptive survey study, including women with a history of pregnancy complicated by PAS who were members of two patient advocacy groups. The survey, consistin

The Impact of Pain Invisibility on Patient-Centered Care and Empathetic Attitude in Chronic Pain Management.

OBJECTIVES: The use of interdisciplinary patient-centered care (PCC) and empathetic behaviour seems to be a promising avenue to address chronic pain management, but their use in this context seems to be suboptimal. Several patient factors can influence the use of PCC and empathy, but little is known about the impact of pain visibility on these behaviours. The objective of this study was to investigate the influence of visible physical signs on caregiver's patient-centered and empathetic behaviours in chronic pain context. METHODS: A convenience sample of 21 nurses and 21 physicians participat

Interprofessional management of a complex continuing care patient admitted with 18 pressure ulcers: a case report.

Interprofessional practice (IP)--ie, collaborative practice--involves interaction and knowledge-sharing between professionals from different disciplines in order to meet the needs of the patient. This approach to care is well suited to patients with pressure ulcers, whose complex and varying presentations require the monitoring and consultation of an IP team. A 44-year-old man with anoxic brain injury was admitted to a complex continuing care facility with 18 wounds, 17 of which were pressure ulcers. The patient was at high risk for further skin breakdown as a result of immobility, incontin

Dental care management of a young patient with extensive lymphangioma of the tongue: a case report.

Lymphangiomas are benign tumors resulting from a congenital malformation of the lymphatic system. Relatively uncommon, lymphangiomas are usually diagnosed at birth and develop within the first years of life. When these tumors occur in the oral cavity, the tongue is the most frequently affected site. Lymphangioma of the tongue is a common cause of macroglossia in children, which may lead to a dry/cracked tongue with ulcerating secondary infections, difficulty in swallowing and mastication, speech disturbances, exclusive nasal breathing, airway obstruction, mandibular prognathism and other possi

Chronic care management for substance dependence in primary care among patients with co-occurring disorders.

OBJECTIVE: Co-occurring mental and substance use disorders are associated with worse outcomes than a single disorder alone. In this exploratory subgroup analysis of a randomized trial, the authors hypothesized that providing chronic care management (CCM) for substance dependence in a primary care setting would have a beneficial effect among persons with substance dependence and major depressive disorder or posttraumatic stress disorder (PTSD). METHODS: Adults (N=563) with alcohol dependence, drug dependence, or both were assigned to CCM or usual primary care. CCM was provided by a nurse care

Redesigning the care of fragility fracture patients to improve osteoporosis management: a health care improvement project.

OBJECTIVE: To develop new processes that assure more reliable, population-based care of fragility fracture patients. METHODS: A 4-year clinical improvement project was performed in a multispecialty, community practice health system using evidence-based guidelines and rapid cycle process improvement methods (plan-do-study-act cycles). RESULTS: Prior to this project, appropriate osteoporosis care was provided to only 5% of our 1999 hip fracture patients. In 2001, primary physicians were provided prompts about appropriate care (cycle 1), which resulted in improved care for only 20% of patients.

Integrated Chronic Care Management For Patients With Atrial Fibrillation - A Rationale For Redesigning Atrial Fibrillation Care.

Atrial Fibrillation (AF) is a highly prevalent heart rhythm disturbance, often associated with underlying (cardio)vascular disease. Due to this the management of AF is often complex and current practice calls for a more comprehensive, multifactorial and patient-centred approach. Therefore an Integrated Chronic Care approach in AF was developed and implemented in terms of a nurse-led specialized outpatient clinic for patients with AF. A randomised controlled trial comparing the nurse-led approach with usual care demonstrated superiority in terms of cardiovascular hospitalization and death as we

End-of-life issues in caring for patients with dementia: the case for palliative care in management of terminal dementia.

The number of people suffering with dementia is increasing in the general population and the trend is projected to continue as people live longer, especially in countries with developed economies. The most common cause of dementia (among the many other causes) is Alzheimer's dementia, which is considered a terminal illness. The disease could eventually lead to death, or death could occur as a consequence of co-morbid physical complications. The problem of end of life (EOL) care for patients suffering from dementia though spoken of and written about, does not get the attention and system suppor
