Navigating Reproductive Care in Patients With Inflammatory Bowel Disease: A Comprehensive Review.

Inflammatory bowel disease [IBD] is often diagnosed in patients during their reproductive years. It is crucial that both healthcare providers and patients are adequately informed to avoid misguided decisions regarding family planning. One of the most important aspects during conception and pregnancy is to maintain disease remission, as disease activity is associated with adverse pregnancy outcomes. Apart from methotrexate, most conventional drugs used in IBD are considered low risk during conception and pregnancy. For newer agents, evidence is still limited. If needed, surgery must not be post

Accuracy and Precision of Acetabular Component Placement With Imageless Navigation in Obese Patients.

BACKGROUND: Obesity is a risk factor for acetabular component malposition when total hip arthroplasty is performed with manual techniques. The utility of imageless navigation in obese patients remains unknown. This study compared the accuracy and precision of imageless navigation for component orientation between obese and nonobese patients. METHODS: A total of 459 total hip arthroplasties performed for osteoarthritis using imageless navigation were reviewed from a single surgeon's institutional review board-approved database. Einzel-Bild-Roentgen Analyse determined component orientation on 6

Evidence for spatial navigational impairments in hydrocephalus patients without spina bifida.

The cognitive sequelae of hydrocephalus have mostly been explored with standardised clinical tasks. The aim of the present research was determine whether impairments on these abstract tasks extend to everyday spatial and navigational behaviour. Patients with hydrocephalus, but without spina bifida, were compared to a control group on tests of searching behaviour, landmark memory, route learning, and path integration. Participants with hydrocephalus displayed reduced sensitivity to spatial cueing, less accurate route-learning, and significantly less accurate spatial updating. These data represe

Navigation-assisted total knee arthroplasty for patients with extra-articular deformity.

PURPOSE: Since the existence of an extra-articular deformity seriously alters the normal geometry and kinetics around the knee joint, difficulties are often encountered in total knee arthroplasty (TKA) using a standard surgical technique. The purpose of this study was to evaluate the usefulness of surgical navigation system as a treatment option for osteoarthritic knees with extra-articular deformity. MATERIALS AND METHODS: The authors retrospectively reviewed medical records of the patients who underwent primary TKA between 2007 and 2012. Knees with preoperative radiography showing an angula

The effect of navigator on length of stay and rehabilitation for total hip arthroplasty patients.

BACKGROUND: Navigated THA is a new procedure in Thailand that has been performed since 2012. The previous studies have reported that navigated THA was a safe and reliable procedure that resulted in a more consistent cup placement compared to the conventional free hand technique. Furthermore, it decreased complications of THA, especially dislocation. Perioperative protocols are based on the surgeon's concern about stability of the prosthesis and the patient's health condition. Assuming that the navigator can improve the alignment and stability of THA, the time to start rehabilitation and the po

Decreased accuracy of acetabular cup placement for imageless navigation in obese patients.

BACKGROUND: Optimal acetabular cup position is an important determinant of the success of total hip arthroplasty (THA), and navigation systems have been developed and applied to improve placement precision. However, the registration method touching bony landmarks through soft tissues may decrease the accuracy in obese cases and in patients with acetabular dysplasia. METHODS: Placement accuracy in 69 joints in which THA was performed with imageless navigation was calculated by comparing the placement angle in the anterior pelvic plane and the placement angle in the x-ray table plane with the p

Application of patient navigation programs across the entire continuum of breast cancer care: a GRADE-assessed systematic review with meta-analysis.

BACKGROUND: Patient navigation programs (PNPs) have been increasingly applied in breast cancer (BC) care of underserved clients, as they can significantly assist them with coordinating services across the care continuum. However, existing evidence has produced inconclusive findings regarding the empirical adequacy of BC-related caring interventions developed from PNPs. Hence, the aim of the present systematic review and meta-analysis was to clarify the benefits of PNPs across all phases of the BC care continuum. Primary outcomes comprised care-continuum indicators extracted predominantly from

Overcoming Barriers to Accessing Surgery and Rehabilitation in Low and Middle-Income Countries: An Innovative Model of Patient Navigation in Nepal.

BACKGROUND: Injury and disability are prominent public health concerns, globally and in the country of Nepal. Lack of locally available medical infrastructure, socioeconomic barriers, social marginalization, poor health literacy, and cultural barriers prevent patients from accessing surgical and rehabilitative care. Overcoming these barriers is an insurmountable challenge for the most vulnerable and marginalized, resulting in absence of treatment or even death. METHODS: Sundar Dhoka Saathi Sewa (SDSS), a non-government organization, provides a patient navigation service which facilitates refe

Understanding the implementation of patient navigation for adults living with HIV: A scoping review of components, equity considerations and lessons learned.

BACKGROUND: People living with HIV navigate complex medical and social challenges that impact engagement in care, including stigma, poverty and systemic inequities. Patient navigation has emerged as a promising approach to improve linkage, retention and adherence across the HIV care continuum. While evidence suggests that navigation enhances care engagement and health outcomes, little is known about how these programs are implemented in real-world settings, particularly with respect to equity. AIM: This scoping review mapped the literature on the implementation of HIV patient navigation progr

Communities Helping the Hearing of Infants by Reaching Parents (CHHIRP) through patient navigation: a hybrid implementation effectiveness stepped wedge trial protocol.

INTRODUCTION: As the most common neonatal sensory disorder in the USA, infant hearing loss has an incidence of 1.7 per 1000 births. The consequences of delayed diagnosis and failure to obtain timely intervention include significant communication impairment and negative socioeconomic effects. Early Hearing Detection and Intervention (EHDI) national standards dictate that all infants should be screened and diagnosed by 3 months of age and there is a need for interventions that promote adherence to timely diagnosis. Patient navigation (PN) has been shown to be efficacious to decrease non-adherenc

One-stage femoral osteotomy and computer-assisted navigation total knee arthroplasty for osteoarthritis in a patient with femoral subtrochanteric fracture malunion.

Extra-articular femoral deformity in total knee arthroplasty (TKA) is realigned by either intra-articular correction or extra-articular osteotomy. The more distant the deformity is away from knee joint, the more likely it is corrected by the former method. No report described the use of antegrade cephalomedullary femoral nail to fix the osteotomy followed by computer-assisted navigation TKA. This report described the unusual use of this method to manage a 64-year-old man with femoral subtrochanteric fracture malunion and osteoarthritis of knee. He demonstrated a satisfactory functional outcome

Navigated brain stimulation for preoperative cortical mapping in paretic patients: case report of a hemiplegic patient.

BACKGROUND AND IMPORTANCE: Navigated brain stimulation (NBS) is an emerging technology that can be used for preoperative mapping of the motor cortex. It combines conventional transcranial magnetic stimulation with neuronavigation and achieves high precision by taking into account all relevant physical factors. In contrast to functional imaging technologies, NBS does not rely on voluntary patient movements for cortical mapping. Thus, NBS can be used even on patients with severe motor impairment. CLINICAL PRESENTATION: This article presents the case of a hemiplegic elderly woman with a brain tu

NAVIGATE: improving survival in vulnerable patients with lung cancer through nurse navigation, symptom monitoring and exercise - study protocol for a multicentre randomised controlled trial.

INTRODUCTION AND AIM: Low socioeconomic position (SEP) has been shown to be strongly associated with impaired lung cancer survival. Barriers related to receiving recommended treatment among patients with lung cancer with low SEP may include adverse health behaviour and limited physical and psychosocial resources influencing the ability to react on high-risk symptoms and to navigate the healthcare system. To address the underlying factors that drive both decisions of treatment, adherence to treatment and follow-up in vulnerable patients with lung cancer, we developed the Navigate intervention.

Is there a difference between navigated and non-navigated robot cohorts in robot-assisted spine surgery? A multicenter, propensity-matched analysis of 2,800 screws and 372 patients.

BACKGROUND CONTEXT: Robot-assisted spine surgery continues to rapidly develop as evidenced by the growing literature in recent years. In addition to demonstrating excellent pedicle screw accuracy, early studies have explored the impact of robot-assisted spine surgery on reducing radiation time, length of hospital stay, operative time, and perioperative complications in comparison to conventional freehand technique. Recently, the Mazor X Stealth Edition was introduced in 2018. This robotic system integrates Medtronic's Stealth navigation technology into the Mazor X platform, which was introduce
