Effect of portal pressure gradient reduction on outcomes after transjugular intrahepatic portosystemic shunt in portal hypertension patients.

BACKGROUND: Portal hypertension (PHT), a complication of liver cirrhosis, is sometimes managed with transjugular intrahepatic portosystemic shunt (TIPS) to reduce portal pressure. Although effective, TIPS poses risks, including hepatic encephalopathy (HE). This study investigates whether a significant reduction in the portal pressure gradient (PPG) after TIPS improves outcomes in PHT patients. AIM: To evaluate the impact of post-TIPS PPG reduction on clinical outcomes and explore the relationship between PPG reduction and portal vein diameter. METHODS: This retrospective cohort study include

Stratified analysis of the correlation between wedged hepatic venous pressure and portal venous pressure in patients with portal hypertension.

To evaluate the differences in the agreement between wedged hepatic venous pressure (WHVP) and portal venous pressure (PVP) at different hepatic venous pressure gradient (HVPG) levels to identify specific HVPG thresholds where WHVP can reliably estimate PVP, thus enhancing the accuracy of risk stratification and treatment decision-making for portal hypertension (PHT) patients. A multicenter study of 616 patients with PHT from three centers was stratified into five groups by their HVPG: HVPG < 12 (group A), 12 ≤ HVPG < 16 mmHg (group B), 16 ≤ HVPG < 20 mmHg (group C), 20 ≤ HVPG < 24 mmHg (group

Parachute technique for portal vein reconstruction during pancreaticoduodenectomy with portal vein resection in patients with pancreatic head cancer.

BACKGROUND: The most appropriate venous reconstruction method remains debatable when a long section of portal vein (PV) and/or superior mesenteric vein (SMV) must be resected in patients undergoing a pancreaticoduodenectomy (PD). The aim of the present study was to describe the technical details of the parachute technique, a modified end-to-end anastomotic maneuver that can be used in the above-mentioned circumstances, and to investigate its safety and feasibility. STUDY DESIGN: Patients who underwent venous reconstruction using the parachute technique after receiving a PD with PV resection f

Effects of surgical procedures on the occurrence and development of postoperative portal vein thrombosis in patients with cirrhosis complicated by portal hypertension.

OBJECTIVE: We have compared the influences of three surgical strategies on the occurrence and development of portal vein thrombosis (PVT) in patients with liver cirrhosis complicated by portal hypertension (PHT) in this study. METHODS: Total 116 patients who respectively underwent pericardial devascularization (PCDV) with splenectomy (PDS group: n = 50), selective PCDV (SPD group: n = 28) and splenorenal shunt (SRS) combined with PCDV (combined group: n = 38) were investigated in this study. The incidence of PVT before and after operation was monitored. The incidence of Grade II-IV PVT was us

Pretransplant Portal Vein Recanalization-Transjugular Intrahepatic Portosystemic Shunt in Patients With Complete Obliterative Portal Vein Thrombosis.

BACKGROUND: Chronic, obliterative portal vein (PV) thrombosis (PVT) represents a relative contraindication to liver transplantation (LT) in some centers. When PV thromboembolectomy is not feasible, alternative techniques (portacaval hemitransposition, portal arterialization, multivisceral transplantation) are associated with suboptimal outcomes. In cases where a chronically thrombosed PV has become obliterated, we developed PV recanalization (PVR)-transjugular intrahepatic portosystemic shunt (TIPS) to potentiate LT. We evaluated the impact of PVR-TIPS on liver function, transplant eligibility

Successful pregnancy in a patient with portal hypertension secondary to portal vein thrombosis due to essential thrombocythaemia: a rare case.

Essential thrombocythaemia (ET) is a disease characterized by an increased platelet count, megakaryocytic hyperplasia and a hemorrhagic or thrombotic tendency. Pregnancy in patients with ET can have a favorable outcome. However, ET has also been reported to complicate pregnancy by recurrent abortions, intrauterine death, and fetal growth retardation due to placental infarctions. ET has an unusual prevalence of intraabdominal (hepatic, portal and mesenteric) vein thrombosis, especially in young patients, which can lead to portal hypertension. There are ample cases in the literature of both esse

Thromboelastographic evaluation of coagulation in patients with extrahepatic portal vein thrombosis and non-cirrhotic portal fibrosis: a pilot study.

BACKGROUND AND AIMS: Non-cirrhotic portal hypertension due to extrahepatic portal vein thrombosis (EHO) and non-cirrhotic portal fibrosis (NCPF) is a major cause of upper gastrointestinal hemorrhage in India. Hypercoagulability has been proposed to explain the thrombosis in the portal vein in EHO and intrahepatic portal vein radicals in NCPF. However, some authors have reported hypocoagulability in these patients. Thromboelastography (TEG), which gives a dynamic assessment of coagulation, has potential for evaluating coagulation in these patients but has not been used so far. It was the aim of

Outcome of patients with pre-existing portal vein thrombosis undergoing arterialization of the portal vein during liver transplantation.

Arterialization of the portal vein is being propagated as a technical possibility in liver transplant recipients with pre-existing portal vein thrombosis. In our own small series, portal vein arterialization (PVA) was carried out in four patients undergoing orthotopic liver transplantation. In three of these cases, the portal vein was anastomosed to the aorta via an interposed iliac artery, and in one case, directly to the hepatic artery. After PVA, all transplants showed regular initial function. Two patients died postoperatively after 19 and 50 days, of intra-abdominal haemorrhage and liver

Scintigraphic visualisation of extrahepatic portal circulation in patients with portal hypertension using 99mTc-tin-colloid and 99mTc-HIDA.

Rapid dynamic sequential scintigraphy of the abdomen with 99mTc-tin-colloid and 99mTc-HIDA has been used to evaluate two patients with portal hypertension. 99mTc-tin-colloid was useful for visualisation of diliated vessels in the abdomen. Radionuclide hepatobiliary imaging was useful in such cases in evaluation of the hepatobiliary system.

Patient Perspectives on AI-Drafted Electronic Portal Messages.

IMPORTANCE: Patient portal messaging volume has increased substantially and is linked to clinician burnout. In response, health systems are increasingly adopting large language models (LLMs) to generate draft replies to patient messages, yet little is known about how patients interpret and evaluate artificial intelligence (AI) involvement in this communication channel. OBJECTIVE: To understand how patients perceive AI-drafted responses to electronic patient portal messages and to identify implications for patient-centered implementation. DESIGN, SETTING, AND PARTICIPANTS: This qualitative st

Overview of Non-Cirrhotic Portal Hypertension in Pediatric Patients.

Non-cirrhotic portal hypertension (NCPHT) is defined as portal hypertension (PHT) occurring in the absence of cirrhosis. Major etiological causes of NCPHT include immunological disorders, chronic infections, exposure to medications or toxins, prothrombotic conditions, and several genetic syndromes, highlighting that NCPHT is not a single disease but a shared phenotype arising from diverse underlying pathways. NCPHT is frequently misdiagnosed, largely due to inconsistent nomenclature and limited scientific literature. The broader term non-cirrhotic portal fibrosis (NCPF) or porto-sinusoidal vas

[Management of patients with cirrhosis and portal hypertension-related bleeding].

Acute variceal bleeding treatment in patients with portal hypertension combines vasoactive drugs, prophylactic antibiotics, hepatic encephalopathy prophylaxis and upper gastrointestinal endoscopy for variceal treatment (elastic band ligation and/or tissue adhesive injection), on top of non-specific measures. Failure to control bleeding must lead to salvage TIPS (Transjugular Intrahepatic Portosystemic Shunt) discussion, after esophageal tamponade (balloon or stent).Pre-emptive TIPS within 72h must systematically be considered in high-risk patients to reduce rebleeding and mortality. Pre-emptiv

Transient Extensive Portal Venous Gas in a Dialysis Patient.

Hepatic portal venous gas (HPVG) is a serious radiologic indicator that can suggest bowel ischemia and lead to fatal outcomes. The medical field now identifies transient HPVG without ischemia more often because CT technology has spread throughout the healthcare system. We report a 70-year-old woman with end-stage renal disease (ESRD) on hemodialysis who developed acute abdominal pain during dialysis. The CT scan results showed HPVG extending into the superior mesenteric venous system. The medical team performed an urgent diagnostic laparoscopy because of ischemia concerns, which showed that th

Update in the treatment of cirrhotic patients with portal vein thrombosis.

Portal vein thrombosis (PVT) is characterized by the formation of a thrombus (blood clot) within the portal vein system, including main portal vein and its intrahepatic portal vein branches, and may extend to the superior mesenteric vein or splenic vein. The emergence of PVT is linked to diverse risk factors, encompassing liver conditions with cirrhosis, abdominal infections, previous abdominal surgeries, malignancies, inherited or acquired thrombophilias, and systemic hypercoagulable conditions. Recent studies revealed a possible connection between the occurrence of PVT and either contracting

Textbook outcomes after splenectomy in patients with portal hypertension.

The Textbook Outcome (TO) is a composite outcome measure that integrates commonly utilized independent outcome measures to comprehensively reflect the optimal post-surgical treatment result. It is used to assess the quality of surgeries and has been widely employed in hepatobiliary and pancreatic surgeries. However, its utility in splenic surgery remains unknown. Patients with cirrhosis and portal hypertension are often managed with splenectomy. The objective of this study was to investigate the perioperative textbook outcomes of splenectomy and identify the risk factors associated with achiev

Chronic Extrahepatic Portal Vein Occlusion in a Young Patient.

Extrahepatic portal vein obstruction (EHPVO) is a rare condition characterized by the occlusion or narrowing of the portal vein outside the liver. We present a case report of a patient with EHPVO secondary to combined protein C and S deficiency and pancytopenia secondary to hypersplenism, highlighting the clinical presentation, diagnostic challenges, and management strategies. Early recognition of this condition and prompt initiation of appropriate treatment can prevent life-threatening complications such as variceal bleeding and portal hypertension. This case underscores the need for a high i
