Effect of level of sedation on outcomes in critically ill adult patients: a systematic review of clinical trials with meta-analysis and trial sequential analysis.

BACKGROUND: Sedation is routinely administered to critically ill patients to alleviate anxiety, discomfort, and patient-ventilator asynchrony. However, it must be balanced against risks such as delirium and prolonged intensive care stays. This study aimed to investigate the effects of different levels of sedation in critically ill adults. METHODS: Systematic review with meta-analysis and trial sequential analysis (TSA) of randomised clinical trials including critically ill adults admitted to the intensive care unit. CENTRAL, MEDLINE, Embase, LILACS, and Web of Science were searched from their

Neuromuscular paralysis for newborn infants receiving mechanical ventilation.

BACKGROUND: Ventilated newborn infants breathing in asynchrony with the ventilator are at risk for complications during mechanical ventilation, such as pneumothorax or intraventricular hemorrhage, and are exposed to more severe barotrauma, which consequently could impair their clinical outcome. Neuromuscular paralysis, which eliminates spontaneous breathing efforts of the infant, has potential advantages in this respect. However, a number of complications have been reported with muscle relaxation in infants, so that concerns exist regarding the safety of prolonged neuromuscular paralysis in ne

Rib cage deformities alter respiratory muscle action and chest wall function in patients with severe osteogenesis imperfecta.

BACKGROUND: Osteogenesis imperfecta (OI) is an inherited connective tissue disorder characterized by bone fragility, multiple fractures and significant chest wall deformities. Cardiopulmonary insufficiency is the leading cause of death in these patients. METHODS: Seven patients with severe OI type III, 15 with moderate OI type IV and 26 healthy subjects were studied. In addition to standard spirometry, rib cage geometry, breathing pattern and regional chest wall volume changes at rest in seated and supine position were assessed by opto-electronic plethysmography to investigate if structural m

A longitudinal latent class growth analysis of PTSD symptom development in mechanically ventilated ICU patients.

ICU patients exhibit a high incidence of post-traumatic stress disorder (PTSD). During treatment, procedural pain (e.g., suctioning, ventilator asynchrony) and communication barriers can trigger significant stress responses, exacerbating feelings of fear and helplessness, ultimately contributing to PTSD post-extubation. Current research predominantly relies on cross-sectional studies, failing to capture the dynamic progression of PTSD. As recovery evolves, patients' psychological and physical states continuously change, necessitating adaptive intervention strategies. There is an urgent need fo

Feasibility and Clinical Outcomes of a Step Up Noninvasive Respiratory Support Strategy in Patients with Severe COVID-19 Pneumonia.

The best noninvasive respiratory strategy in patients with Coronavirus Disease 2019 (COVID-19) pneumonia is still discussed. We aimed at assessing the rate of endotracheal intubation (ETI) in patients treated with continuous positive airway pressure (CPAP) and noninvasive ventilation (NIV) if CPAP failed. Secondary outcomes were in-hospital mortality and in-hospital length of stay (LOS). A retrospective, observational, multicenter study was conducted in intermediate-high dependency respiratory units of two Italian university hospitals. Consecutive patients with COVID-19 treated with CPAP were

Neurally adjusted ventilatory assist compared to other forms of triggered ventilation for neonatal respiratory support.

BACKGROUND: Effective synchronisation of infant respiratory effort with mechanical ventilation may allow adequate gas exchange to occur at lower peak airway pressures, potentially reducing barotrauma and volutrauma and development of air leaks and bronchopulmonary dysplasia. During neurally adjusted ventilatory assist ventilation (NAVA), respiratory support is initiated upon detection of an electrical signal from the diaphragm muscle, and pressure is provided in proportion to and synchronous with electrical activity of the diaphragm (EADi). Compared to other modes of triggered ventilation, thi

Pleural and transpulmonary pressures to tailor protective ventilation in children.

This review aims to: (1) describe the rationale of pleural (PPL) and transpulmonary (PL) pressure measurements in children during mechanical ventilation (MV); (2) discuss its usefulness and limitations as a guide for protective MV; (3) propose future directions for paediatric research. We conducted a scoping review on PL in critically ill children using PubMed and Embase search engines. We included peer-reviewed studies using oesophageal (PES) and PL measurements in the paediatric intensive care unit (PICU) published until September 2021, and excluded studies in neonates and patients treated w

Thoracoabdominal Asynchrony in Very Severe COPD: Clinical and Functional Correlates During Exercise.

INTRODUCTION: Patients with severe Chronic Obstructive Pulmonary Disease (COPD) often experience breathlessness and exercise limitations due to expiratory flow limitation. Pulmonary rehabilitation programmes, including exercise with non-invasive ventilation (NIV) or high-flow nasal therapy (HFT), aims to improve quality of life and exercise tolerance. This study investigates the relationship between thoracoabdominal asynchrony (TAA) during supported (NIV and HFT) and unsupported (conventional oxygen therapy - COT) exercise and clinical and functional parameters in severe COPD patients awaiting

Acute effects of transcutaneous electrical diaphragmatic stimulation on respiratory pattern in COPD patients: cross-sectional and comparative clinical trial.

BACKGROUND: Transcutaneous electrical diaphragmatic stimulation (TEDS) has been used to improve respiratory muscle strength in patients with respiratory muscle weakness. However, this physical therapy resource has not been studied in chronic obstructive pulmonary disease (COPD). OBJECTIVE: To evaluate the respiratory pattern during one session of TEDS in COPD patients. METHOD: Fifteen COPD patients participated in one TEDS session for plethysmographic analysis and assessment of peripheral oxygen saturation (SpO2) and heart rate (HR). After the session, patients were divided into two groups:

Severe, Non-apneic Respiratory Dysfunction and Hypoxia following Generalized Convulsive Seizures.

OBJECTIVE: Sudden unexpected death in epilepsy (SUDEP) is a devastating consequence of some generalized convulsive seizures (GCS). Recent work has focused on seizure related apnea as a biomarker of SUDEP risk, frequently without characterizing the adequacy of non-apneic ventilation or identifying other dysfunctional breathing patterns. We hypothesized that GCS frequently induce immediate, severe, non-apneic respiratory dysfunction that can induce critical hypoxia and bradycardia and sought to characterize breathing patterns after GCS. METHODS: Adult patients admitted to an epilepsy monitoring

Residual Sleep-Related Respiratory Events During Early Versus Long-Term NIV Follow-Up: Burden and Relationship with Gas Exchange.

Background: A recent study published by our group demonstrated that patient-ventilator asynchrony (PVA) is independently associated with impaired gas exchange during long-term non-invasive ventilation (NIV). We aimed to determine whether the burden and phenotype of residual PVA vary according to disease category, NIV initiation context and follow-up timing, and whether the relationship between PVA and gas exchange differs according to follow-up timing. Methods: We performed a secondary analysis of 123 NIV-treated patients included in the primary study. PVA burden, upper-airway obstructive even

Does less invasive surfactant administration during high-flow nasal cannula oxygen treatment reduce the need for invasive ventilation in late preterm and early term born infants with respiratory distress? A study protocol for a single-centre study with a treatment and control arm.

INTRODUCTION: Up to 40% of late preterm and early term infants require neonatal admission with respiratory disease being the most common cause. Less invasive surfactant administration (LISA) has previously been used with good success in the very preterm population, but it is unclear whether late preterm infants suffering from respiratory distress syndrome and early term infants suffering from secondary surfactant deficiency might also benefit from LISA. Continuous positive airway pressure (CPAP) in that population can increase asynchrony and air leaks, whereas these may be avoided by using hig

Bronchodilator Drug Delivery With Pressurized Meter-Dose-Inhaler Versus Jet Nebulizer With Noninvasive-Ventilation in Acute-Exacerbation of COPD: A Randomized Controlled Pharmacokinetic Endpoint Trial.

BACKGROUND AND OBJECTIVE: Non-invasive ventilation (NIV) and inhaled bronchodilator therapy are foundational in the management of acute exacerbation of chronic obstructive pulmonary disease (AECOPD). However, optimal drug-delivery strategies during NIV remain uncertain. This study compared bronchodilator delivery via a pressurized metered-dose inhaler (pMDI) interface versus standard jet nebulization during NIV. METHODS: This randomized, open-label trial enrolled adults with hypercapnic AECOPD requiring NIV. Participants were randomized 1:1 to receive ipratropium bromide via pMDI using a modi

Acute Effects of Positive Expiratory Pressure on Breathing Pattern and Chest Wall Volumes During Cycle Exercise in COPD Patients.

BACKGROUND: Patients with COPD experience air flow limitation, reduced expiratory capacity, and dynamic hyperinflation (DH), which causes dyspnea and decreased exercise tolerance. Expiratory positive airway pressure (EPAP) is a potential intervention that can reduce DH and improve respiratory muscle efficiency, but its impact on exercise tolerance remains unclear. The aim was to evaluate the acute effects of EPAP during exercise on the respiratory pattern, chest wall volumes, thoracoabdominal asynchrony (TAA), and DH in individuals with COPD using optoelectronic plethysmography. METHODS: This

Noninvasive positive pressure ventilation for acute respiratory failure following upper abdominal surgery.

BACKGROUND: Each year, more than four million abdominal surgeries are performed in the US and over 250,000 in England. Acute respiratory failure, a common complication that can affect 30% to 50% of people after upper abdominal surgery, can lead to significant morbidity and mortality. Noninvasive ventilation has been associated with lower rates of tracheal intubation in adults with acute respiratory failure, thus reducing the incidence of complications and mortality. This review compared the effectiveness and safety of noninvasive positive pressure ventilation (NPPV) versus standard oxygen ther
